On the basis of the experience with treatment of more than 500 patients with sigmoid and rectal cancer, the informative value of the instrumental methods of investigation used for more precise definition of a stage of the disease is analysed. A scheme for rational examination of the patients is suggested.

The authors present their experience with perineal infiltrates amputations on account of rectal carcinoma. In the course of 1986 in the Oncological Research Institute in Brno 58 patients with this diagnosis were hospitalized. After amputations three types of surgical complications were observed: 1. early inflammatory infiltrations, 2. late abscesses and 3. locoregional relapses of the primary tumour. The diagnostic efficacy of routine double-contrast barium enema and fiberoptic coloscopy for detection of cancer was retrospectively studied in 303 patients consecutively operated for colorectal adenocarcinoma in a 52-month period from January 1980. Double-contrast barium enema was performed in 180 patients with 184 carcinomas, 157 (85%) of which were revealed by this examination. Binding of FITC conjugated lectins to mucin in benign and malignant colon lesions was studied by fluorescence microscopy. In 145 cases of colonic adenocarcinoma, PNA (peanut agglutinin) was found to be bound to 75% of cases, while DBA (dolichos biflorus agglutinin) to 13% only. Radiographic appearances at double contrast enema of 8 cases of anorectal cloacogenic carcinoma are described. In all cases tumor was radiologically identified as a neoplastic lesion, but the correct histologic diagnosis was not reached in any case. Radiological features of the cloacogenic carcinoma are: the evident submucosal mass, the poor mucosal involvement and the eccentricity of the lesion. However, the characteristics of the tumor are not peculiar and radiologic diagnosis of the nature of tumor can only be suspected.

Thermography results of 126 patients were analysed for the diagnosis of recurrences of rectal cancer. Sixteen patients were examined after radical operations for rectal cancer without signs of recurrence to study the features of a thermal regimen in this area. Sera, duodenal liquid and biopsy materials of small intestine and of mesenteric lymph nodes were collected from 22 patients with colorectal cancer (intestinal lymphoma and adenocarcinoma) and 10 control patients with gastric ulcer were found in all of patients with cancer. We report a clinical case of carcinoma of the rectum. A 45-year-old patient had undergone resection of the rectum and proctostomy 22 months ago. Analysis of a 30 year (1950 to 1979) series of colorectal carcinoma disclosed 29 patients with ulcerative colitis, who were compared in this retrospective study with 1,032 noncolitic patients. Colitic tumors were often multiple and had a predilection for the transverse colon. Nineteen patients were resected with the intention of cure, for a curability rate of 66 percent, which is comparable to the 69 percent rate in noncolitic patients. Seventeen cases of colorectal polyps in the dog were seen at the Ontario Veterinary College over a 12-year period. The mean age of occurrence was 6.9 years, and males and females were almost equally affected. The Collie was the most frequently affected breed (4/17). A 10-month-old boy had episodes of apparent colic with bloody diarrhea. On investigation after prolapse of a rectal mass, a pedunculated polyp was found and removed by transanal ligation. The abdominal pain had been caused by the polyp intussuscepting the sigmoid colon into the rectum. Although rectal bleeding in children under age 1 is rarely caused by rectal polyps, physicians should consider this diagnosis in children of any age when recurrent colic and blood-streaked diarrhea occur.

With good technique it is possible to detect small polyps in the large intestine with a barium enema examination. The radiological appearences and visibility of villous papillomas in 84 barium enema examinations from patients with both benign and malignant villous papillomas in the rectum, rectosigmoideal junction and the sigmoid loop were studied. In 75% of the examinations the tumour was visible. A specific radiological diagnosis of non invasive villous papilloma was made in 33% and the correct malignant diagnosis in 8/13 cases.
