A rare case of spinal cord metastases from oral SCC.

Oral squamous cell carcinoma (SCC) tends to spread by direct extension, lymphatic metastases and has low incidence of haematogenous spread. Intramedullary and intradural spinal cord metastases are rare in general. We present a patient case with an aggressive oral SCC which developed intradural and intramedullary multiple spinal cord metastases.

Intraspinal clear cell meningioma: diagnosis and management: report of two cases.

OBJECTIVE AND IMPORTANCE: Intraspinal clear cell meningioma is a rare morphological variant of meningioma. Only 13 case reports are found in the literature; therefore, no management strategy has been defined for this tumor type. This article describes two patients, reviews the literature, and proposes a treatment plan for clear cell meningioma. CLINICAL PRESENTATION: Two female patients, 22 months and 8 years of age, respectively, presented with localized neck and leg pain that limited their ability to walk. Magnetic resonance imaging revealed intradural tumors, a cervical intramedullary neop

The copy-number events in skull base chordoma stratify tumours into four biologically coherent groups.

Chordoma, a rare sarcoma of notochordal origin, exhibits slow growth and local aggressiveness. While copy-number (CN) events are recognized as key chordoma drivers, no comprehensive classification, based on CN, has yet been developed. Here, we establish a robust, reproducible genomic subtyping of chordoma, based on CN events. Two independent skull base chordoma cohorts (N = 32, N = 71) were analyzed, utilizing distinct analytical platforms, DNA methylation microarrays and whole-genome sequencing, both controlled for B-allele frequencies. Samples were clustered using unsupervised hierarchical m

A case of diplopia masquerading as ptosis: Unveiling a hidden clival chordoma.

IntroductionChordomas are malignant tumours derived from residual notochord tissue. They are characterized by slow growth and local aggressiveness. Visual symptoms commonly arise from the compression of neighbouring structures.Case DescriptionThe authors describe a 77-year-old male patient who presented with ptosis following cataract surgery. The patient described diplopia and limitations in all directions. Computed tomography imaging revealed a soft tissue mass The pathology report showed a diagnosis of chordoma. The lesion was surgically removed. Six months postoperatively, ptosis surgery wa

Reappraisal of the types of trigeminal porus and importance in surgical applications.

OBJECTIVE: The detailed information regarding the types of trigeminal porus (TP) and related surgical approach is lacking in the literature. Therefore, we performed this study to elucidate further the types of TP and the relationships with critical surgical landmarks in the skull base. METHODS: The study was performed on 19 formalin-fixed cadavers of the cranial base (52.6% male, n = 10; 47.4% female, n = 9) on both sides. Calculations were made of the vertical dimension (VD), horizontal dimension (HD), and types of TP, the thickness of the TP, the HD and VD of the internal acoustic meatus, t

Techniques and challenges of the expanded endoscopic endonasal access to the ventrolateral skull base during the "far-medial" and "extreme medial" approaches.

BACKGROUND: Expanding the ventrolateral skull base corridor from the midline of lower clivus to the petroclival fissure is a challenging endonasal surgical task. Resection of lytic lesions like chondrosarcoma can cause cranial nerve morbidities and injury of ICA, necessitating accurate knowledge of correlative endoscopic anatomy with stereotactic landmarks. METHODS: We describe an extended endoscopic endonasal approach (EEA) for a right petroclival chondrosarcoma with the demonstration of ipsilateral surgical landmarks with contralateral normal correlates, using a stepwise comparative image-g

Extended Supraorbital Approach with Modified Eyebrow Incision: Technical Note.

BACKGROUND: The extended supraorbital approach through a modified eyebrow incision is a minimally invasive variant of the frontotemporal or pterional approach that enriches a neurosurgeon's armamentarium for the treatment of pathologies lateral to the anterior clinoid process, by advancing laterally with frontal facial nerve branches monitoring. To demonstrate the steps of the approach, we studied 2 formalin-fixed and artery/vein silicone-injected adult cadaveric heads, and reviewed 1 of the 3 clinical cases operated on to illustrate the applicability of the approach. CLINICAL PRESENTATION: A

The infratranstentorial subtemporal approach (ITSTA): a valuable skull base approach to deep-seated non-skull base pathology.

BACKGROUND: Surgical access to space-occupying lesions such as tumors and vascular malformations located in the area of the tentorial notch, mediobasal temporal lobe, and para-midbrain is difficult. Lesions in this area are typically resected with supratentorial approaches demanding significant elevation of the temporal lobe or even partial lobectomy, or via a supracerebellar transtentorial approach. We introduce an alternative, the skull base infratranstentorial subtemporal approach (ITSTA), which provides excellent exposure of the incisural area while minimizing risk to the temporal lobe. M

Long-term olfaction outcomes in transnasal endoscopic skull-base surgery: a prospective cohort study comparing electrocautery and cold knife upper septal limb incision techniques.

BACKGROUND: Olfactory nerve fibers are at risk of injury during transnasal endoscopic skull-base approaches. Olfactory outcomes for various techniques have not been thoroughly investigated. This study aims to report long-term olfactory outcomes when a cold knife upper septal limb incision technique is used compared to monopolar cautery. METHODS: A prospective cohort study was performed at a tertiary referral center. Adult patients undergoing endoscopic approaches with septal incisions were randomized to cold knife or monopolar cautery groups. Patient demographics, clinical history, surgical d

Parapharyngeal space tumor surgery using a modified cervical-parotid approach.

OBJECTIVES: The cervical approach to parapharyngeal space tumors is blind in cases involving an approach to the vicinity of the skull base from below. We therefore devised a procedure to improve the safety of surgery in the vicinity of the skull base. METHODS: With this surgical technique, the temporal branch from the main trunk is first identified, as in the case of parotid gland tumor surgery. We then approach the skull base by entering the posterior aspect of the parotid gland tissue from above the temporal branch and anterior auricular space, allowing clear visual confirmation of the uppe

Isolated cerebellar tuberculoma mimicking posterior cranial fossa tumour.

Isolated central nervous system (CNS) tuberculoma is a rare disease. This disease is associated with high morbidity and mortality, despite modern methods of detection and treatment. CNS tuberculosis can present as meningitis, arachnoiditis, tuberculomas or the uncommon form of tuberculous subdural empyema and brain abscess. We present the clinical, radiological and pathological findings of cerebellar tuberculoma in an Iranian immunocompetent patient mimicking a malignant tumour.

Tentorial detachment technique in the combined petrosal approach for petroclival meningiomas.

OBJECT: The combined petrosal approach is a suitable technique for the resection of medium-to-large petroclival meningiomas (PCMs). Multiple technical modifications have been reported to increase the surgical corridor, including the method of dural and tentorial opening. The authors describe their method of dural opening and tentorial resection, and detail the microanatomy related to their technique to clarify pitfalls and effects. METHODS: The relationship of temporal bridging veins and cranial nerves (CNs) around the tentorial resection area was examined during the combined petrosal approac

Combined microsurgical and endoscopic removal of extensive suprasellar and prepontine epidermoid tumors.

Epidermoids tend to grow around and adhere to critical neurovascular structures, but total or maximal tumor removal is recommended to reduce the risk of recurrence. We describe our method of combined microscopic and endoscopic resection for extensive epidermoid tumors. Thirteen patients with epidermoid tumors located in the suprasellar, prepontine, or surrounding cisterns underwent microsurgical resection with a rigid endoscope and a high definition camera system. An anterior petrosal and/or a pterional approach was selected in 6 patients and a lateral suboccipital approach in 7 patients. An e

[Linear accelerator-based stereotactic radiation treatment of patients with medial middle fossa meningiomas].

Medial middle fossa meningiomas are challenging for neurosurgical treatment. Invasion of cranial nerves and vessels leads to high risk of complications after removal of such meningiomas. Currently methods of conformal stereotactic radiation treatment are applied wider and wider for the discussed lesions. During a 3.5-year period 80 patients with medial middle fossa meningiomas were treated in Burdenko Moscow Neurosurgical Institute using linear accelerator "Novalis". In 31 case radiation treatment was preceded by surgical resection. In majority of patients symptoms included cranial nerve dysfu
