Somatic symptom disorder, psychiatric comorbidities, and suicide.

BACKGROUND: Multiple studies have indicated an association between somatic symptom disorder (SSD) and suicidal symptoms, particularly suicidal ideation. Nevertheless, whether SSD is associated with a risk of suicide mortality remains unclear. In addition, how SSD-related psychiatric comorbidities such as bipolar disorder and major depressive disorder influence the risk of suicide remains unknown. METHODS: In this study, 38,483 patients with SSD and 153,932 age- and sex-matched non-SSD individuals were followed up from 2003 to 2017. Suicide was evaluated within this study period, and major psy

Derealization and panic attacks: a clinical evaluation on 150 patients with panic disorder/agoraphobia.

One hundred fifty patients with Panic Disorder (PD) with or without Phobic Avoidance were subdivided into two groups on the basis of presence/absence of derealization and/or depersonalization (D-D) during panic attacks. D-D was found in 34.7% of the sample. By comparing the two groups, the patients with D-D were found to be younger and had an earlier onset of the disorder; they had a higher prevalence of avoidance behavior and a higher severity of the agoraphobic spectrum phobias. They were also more frequently subject to concomitant disorders such as Generalized Anxiety, Obsessive-Compulsive,

Childhood trauma, trauma in adulthood, and psychiatric diagnoses: results from a community sample.

This study compared the prevalence rates of various psychiatric disorders in persons with first onset of a potentially traumatic event (PTE) in childhood, persons with first onset of a PTE in adulthood, and those with no history of a PTE in a representative sample of Chileans. The Diagnostic of Statistical Manual of Mental Disorders, Revised Third Edition (DSM-III-R), posttraumatic stress disorder, and antisocial personality disorder modules from the Diagnostic Interview Schedule and modules for a range of DSM-III-R diagnoses from the Composite International Diagnostic Interview were administe

Patients with generalized anxiety disorder and a history of trauma: somatic symptom endorsement.

The authors investigated the types and rates of trauma exposure and differences in symptom endorsement in a clinical sample of patients diagnosed with generalized anxiety disorder (GAD). Fifty-eight patients with GAD were assessed using the Structured Clinical Interview (SCID) and Trauma Assessment for Adults. In order to explore the relationship between specific traumatic event(s) and clinical presentation, the presence of somatic symptoms associated with GAD, including muscle tension, autonomic hyperactivity, and vigilance/scanning clusters (using DSM-III-R criteria), were examined. Patients

The classification of mental disorders in primary care: a guide through a difficult terrain.

BACKGROUND: Primary care physicians traditionally have a strong interest in the mental health of their patients. Three classification systems are available for them to diagnose, label, and classify mental disorders: 1) The ICD-10 approach with three options, 2) The DSM-IV approach with two options, and 3) the ICPC approach with two options. This article lists important similarities and differences between the systems to help potential users choose the option that best meets their needs. METHODS: Definitions for depressive disorder, anxiety disorder, and somatization disorder are compared on f

Obsessive compulsive spectrum disorder.

A wide range of psychiatric and medical disorders have been hypothesized to be related to obsessive compulsive disorder (OCD) and thus, together, to form a family of disorders known as obsessive compulsive (or OCD) spectrum disorder. The grouping of these conditions is based on their phenomenological similarities with OCD (i.e., obsessive thinking and/or compulsive behaviors), as well as their having courses of illness, comorbidity and family history patterns, biological abnormalities, and treatment responses similar to OCD. Proposed OCD spectrum disorders have included body dysmorphic disorde

Attitudes of pediatricians toward psychiatric consultations.

This study attempts to evaluate the psychiatric consultation services in both a university and a non-university children's hospital by requesting opinions from 121 pediatricians. A 26-item questionnaire asked pediatricians to comment on the percentage of their patients with psychological problems and their level of ease in dealing with them, problems making psychiatric referrals, and their level of satisfaction with psychiatric consultations and other consultation activities. There were no significant differences between the attitudes of physicians from the two hospitals. Overall, the level of

Discordance between symptom and physiological criteria for the hyperventilation syndrome.

Hyperventilation is assumed to produce a set of somatic and psychological symptoms, the so-called Hyperventilation Syndrome (HVS). Recognition of symptoms during the hyperventilation provocation test (HVPT) is the most widely used criterion for diagnosing HVS, but additional physiological and symptom criteria have been proposed. The concordance of various diagnostic criteria for HVS is investigated in the present study. Forty-eight psychiatric patients with panic disorder and 90 somatic patients with symptoms suspective of HVS performed a HVPT. There was a strong interrelationship between the

Psychosocial transitions: an emotional health comparison.

Utilizing the standardized Symptom Check List-90 (SCL-90), emotional health scores of those experiencing psychosocial transitions were compared. Parents whose child died approximately 60 months earlier (N = 62) were compared to new immigrants living in absorption centers who arrived approximately 8 months earlier (N = 122). Even 5 years after the event, the emotional health of bereaved parents appeared to be lower than the new immigrant group. The differences appeared more pronounced for females and for the 45 + year old groupings (male and female). Depression seemed to be contrary. In most in

Dissociation in adolescent girls with anorexia: relationship to comorbid psychopathology.

This study investigated the relationship between dissociation and psychological symptoms in adolescent girls with anorexia. First, the psychometric properties of the Adolescent Dissociative Experiences Scale (A-DES) were examined using data from 181 nonclinical adolescent boys and girls. Thereafter, A-DES scores and correlations with a range of psychological symptoms were compared across 20 girls with anorexia, 19 mixed clinical girls, and 86 nonclinical girls. The A-DES had a one-factor structure and good psychometric characteristics. Dissociation scores were significantly correlated with lev

Sick-leave due to minor psychiatric morbidity: role of sex integration.

During the years 1985-1987 all sick-leave spells exceeding 7 days, together with a number of demographic variables, were registered in a Swedish county. Using this register, the 3,100 employed persons with at least one sick-leave per year due to minor psychiatric morbidity were analysed with regard to age, occupation and sex. The annual cumulative incidence was 1.7% of the employed population. In 1985 the female incidence was 2.1% and the male incidence was 1.3%. Incidence was highest in the middle-aged. The mean number of absence days per sick-listed person was 73 in 1985. For women, the corr

An investigation of 'functional' somatic symptoms among patients attending hospital medical clinics in Pakistan--II. Using somatic symptoms to identify patients with psychiatric disorders.

Among 600 medical clinic attenders in Lahore, Pakistan, screened using the Bradford Somatic Inventory in Urdu, significant differences in symptom endorsement between 'organic' and 'mood-disordered' patients were found in 14 BSI items. Counting only those symptoms reported as present on more than 15 days during the previous month, significant differences in symptom endorsement were found in 21 BSI items. Another 60 patients attending the same clinics were recruited to evaluate the BSI as a screening questionnaire for mood disorders. Each patient was interviewed by a psychiatrist and a diagnosis

Elevation of immunoreactive CSF TRH in depressed patients.

The concentration of thyrotropin-releasing hormone (TRH), a tripeptide (pyroglutamylhistidylprolin-amide), in the CSF of drug-free patients with DSM-III major depression, somatization disorder, and peripheral neurological disorders was measured with a sensitive and specific radioimmunoassay. The depressed patients had markedly higher CSF TRH concentrations than the other patient groups, and this finding could not be attributed to any demographic variables. The elevation of TRH in CSF provides further evidence of hypothalamic-pituitary-thyroid dysfunction in depression.

Linguistic dimensions of affect and thought in somatization disorder.

DSM-III has established diagnostic criteria that separate somatization disorder from other overlapping symptom configurations. Nevertheless, information regarding the experience of somatization disorder is far from complete. Terms such as "masked depression" or "alexithymia" imply that a disturbance of affect is a central but guarded issue for at least some somatizing patients. Through content analysis of speech, the authors investigated the self-experience of somatization disorder in relation to affective disorder. Rather than defended depression, a distinctive characteristic found in the lan

Life-Threatening Serotonin Syndrome Precipitated by Discontinuation of Serotonin-Dopamine Antagonist in the Presence of Serotonergic Agents: A Case Report.

INTRODUCTION: Serotonin syndrome is caused by excessive activation of serotonin (5-hydroxytryptamine [5-HT]) neurotransmission. Although the discontinuation of antipsychotics with 5-HT2 receptor antagonistic characteristics could theoretically result in serotonin syndrome, there have been very few reports on the syndrome thus far. CASE PRESENTATION: A 75-year-old woman with somatoform disorder was transferred to our emergency room because of pyrexia, unconsciousness, and myoclonus with hyperreflexia. She had been taking milnacipran and perospirone for 10 years and had started taking duloxetin

The influence of age, gender and socio-economic status on multimorbidity patterns in primary care. First results from the multicare cohort study.

BACKGROUND: Multimorbidity is a phenomenon with high burden and high prevalence in the elderly. Our previous research has shown that multimorbidity can be divided into the multimorbidity patterns of 1) anxiety, depression, somatoform disorders (ADS) and pain, and 2) cardiovascular and metabolic disorders. However, it is not yet known, how these patterns are influenced by patient characteristics. The objective of this paper is to analyze the association of socio-demographic variables, and especially socio-economic status with multimorbidity in general and with each multimorbidity pattern. Psychiatric features of children and adolescents with pseudoseizures.

BACKGROUND: Pseudoseizures may occur as a somatoform disorder in children and adolescents as well as adults. However, few data are available about psychiatric features or outcome in pediatric patients. PATIENTS AND METHODS: We studied 34 patients (25 girls [74%]) who were evaluated by a child psychiatrist at our institution immediately after diagnosis of pseudoseizures by ictal video electroencephalogram (EEG) at ages 9 to 18 years (mean age, 14 years). Each patient had at least 1 pseudoseizure recorded by video EEG that was judged by the patient and family as typical, characterized by unresp

Prediction of self-harm in people with newly-diagnosed depression: development and validation of risk prediction models.

Depression is associated with increased self-harm risk, particularly in the early illness course, yet individualised predictions models remain underexplored. We aimed to develop and externally validate a prediction model for self-harm risk in people with newly-diagnosed-depression. Utilizing a territory-wide electronic health-record (EHR) database spanning Hong-Kong public healthcare services (including all public hospitals, specialists, and general outpatient clinics), we identified individuals aged ≥12 years with first-diagnosed depression between 1-January-2002 and 31-December-2021.
