100 consecutive common duct explorations without mortality.

It has been suggested that the incidence of morbidity and mortality after common duct exploration no longer justifies its use in patients with a gallbladder in situ. Therefore endoscopic sphincterotomy has been advocated for removal of common duct stones before cholecystectomy in selected patients. The purpose of this study was to determine our current rate of retained common duct stones and the morbidity and mortality rates associated with common duct exploration. Charts of 100 consecutive patients who underwent cholecystectomy and common duct exploration from January 1982 through December 19…

[The endoscopic treatment of large calculi in the choledochus. The preliminary results with intracorporeal electrohydraulic shock-wave lithotripsy].

Results are reported of endoscopic treatment of: 236 patients with common bile duct calculi treated by endoscopic sphincterotomy with or without hydrostatic balloon extraction and extraction of the calculi, mechanical lithotripsy and endoprosthesis; preliminary results in 12 patients treated by intracorporeal electrohydraulic lithotripsy. Complete removal of calculi from the common bile duct was achieved in 171 of 236 sphincterotomized patients (72 per cent). Complications were observed in 11 per cent of the patients. Mechanical cracking of common bile duct calculi was realized in 60 of 65 pat…

Management of Common Bile Duct Stones: Current Approaches and Evolving Endoscopic Strategies.

PURPOSE OF REVIEW: This review evaluates the evolving management strategies for common bile duct stones (CBDSs), focusing on standard endoscopic techniques, advanced intraductal interventions for refractory stones, and contemporary minimally invasive surgical options. RECENT FINDINGS: While endoscopic retrograde cholangiopancreatography with sphincterotomy remains the cornerstone of therapy, recent evidence highlights the efficacy and safety of endoscopic papillary large balloon dilation and single-operator cholangioscopy-guided lithotripsy for difficult and refractory stones. Concurrently, l…

Meta-analysis of the timing of laparoscopic cholecystectomy after endoscopic retrograde cholangiopancreatography in patients with cholecystolithiasis and choledocholithiasis.

Endoscopic retrograde cholangiopancreatography (ERCP) combined with laparoscopic cholecystectomy plays an important role in the treatment of cholecystolithiasis combined with choledocholithiasis; however, there is no unified standard for the interval of ERCP before laparoscopic cholecystectomy. We conducted a literature search, data extraction and meta-analysis on this topic. Twelve articles with 1142 patients were included, including 567 patients in the E-laparoscopic cholecystectomy group (laparoscopic cholecystectomy performed within 72 h after ERCP) and 575 patients in the D-laparoscopic c

Biliary peritonitis after endoscopic retrograde cholangiopancreatography.

Occurrence of a perforation after endoscopic CPRE sphincterotomy is a complication previously reported in the literature, with frequent repercussions on the duodenum and the periampullary region (Stapfer types 1 and 2); treatment modalities are strictly codified. During an endoscopic procedure, it may be associated with manipulation of a guide wire, generally leading to isolated pneumoretroperitoneum (Stapfer type 4), which is conservatively treated. Secondarily to endoscopic maneuvers, Stapfer type 3 may also occur, causing pancreatic or biliary ductal lesions. When perforation is highlighted…

Is Needle Knife Fistulotomy a Shortcut to Preventing Postendoscopic Retrograde Pancreatitis?

Post-endoscopic retrograde cholangiopancreatography pancreatitis (PEP) remains a common and potentially severe adverse event, with continued research efforts to reduce PEP incidence. Robust evidence exists supporting the selective use of pancreatic duct stent placement, administration of rectal indomethacin, wire-guided cannulation technique, and aggressive fluid hydration using lactated Ringer solution. Jang et al. presented a randomized control trial describing primary needle-knife fistulotomy and its benefit for biliary access and reduced PEP incidence. Although these data are compelling, w

Laparoscopic and endoscopic management of common bile duct stones.

The rapid evolution and acceptance of laparoscopic cholecystectomy for the management of symptomatic cholelithiasis and acute and chronic ch … ommon bile duct stones are reviewed. The role of endoscopic retrograde cholangiopancreatography and sphincterotomy, laparoscopic exploration of the common bile duct, choledochoscopy, and percutaneous transhepatic extraction techniques are discussed.

Endoscopic treatment of acute pancreatitis.

Endoscopic treatment of acute pancreatitis has been proposed in different conditions: acute biliary pancreatitis, pancreatitis related to pancreas divisum or in case of pseudocysts. The efficacy of such endoscopic procedure is discussed.

[Papillotomy in acute pancreatitis: more benefits than risks].

The pathogenesis of acute pancreatitis is still unclear. OPIE's theory that obstruction of the Ampulla of Vater can be an important etiological factor in acute pancreatitis is still relevant for the ongoing discussion whether endoscopic papillotomy can improve acute pancreatitis. Patients with severe acute pancreatitis have to undergo an early ERCP for the detection of the biliary origin of the disease. In case of the detection of choledocholithiasis endoscopic papillotomy should be performed. The rate of complications and the duration of hospitalization can be significantly reduced in contras

An Interesting Case of Cholangitis.

Ascariasis is one of the most common parasitic infections in the world. It is mostly asymptomatic; however, rarely when the worms migrate to the biliary tract, they can cause biliary ascariasis. It typically presents with pain abdomen, jaundice, and fever. This case report is about a patient who presented with fever, icterus, breathlessness, loose stools, and altered sensorium but had no abdominal pain. The patient was diagnosed with biliary ascariasis using ultrasound and endoscopic retrograde cholangiopancreatography (ERCP). The patient was treated with endoscopic sphincterotomy and albendaz…

Endotherapy for Pancreas Divisum.

Pancreas divisum (PD) is a common anatomic variant of the pancreatic duct. Causal association between PD and pancreatitis has been debated for many years. Minor papilla sphincterotomy (miES) is offered in clinical practice to patients with idiopathic acute recurrent pancreatitis (iRAP) and PD. However, available data originate mainly from observational studies with many limitations. An ongoing international, multicenter, sham-controlled trial is evaluating the efficacy of miES in iRAP and PD. Endoscopic therapy for pain relief has limited to no benefit in patients with chronic abdominal pain o…

[Pay attention to endoscopic diagnosis and treatment for confluence disease of pancreaticobiliary ducts].

The confluence of pancreaticobiliary ducts refers to the area between the end of common bile duct, the opening of main pancreatic duct and duodenal papilla. Those confluence diseases of pancreaticobiliary ducts will cause obstruction of biliary and pancreatic outflow tract, cholestasis, pancreatic juice deposition or pancreaticobiliary reflux, and thus induce gallstones, cholangitis, pancreatitis and other diseases. Endoscopic ampullary sphincterotomy is an important way to treat biliary and pancreatic diseases, but the length of incision is determined by the anatomy of Oddi sphincter. The end…

[A case of obstructive jaundice due to the movement of pancreatic calculi].

A 44-year-old man with a history of chronic alcoholic pancreatitis and Crohn's disease presented with abdominal pain. Computed tomography revealed pancreatic calculi in the head of the pancreas and a dilated pancreatic duct. The patient was diagnosed with an acute exacerbation of chronic pancreatitis due to the impact of pancreatic calculi on the main pancreatic duct. During the clinical course, the movement of pancreatic calculi to the major papilla was confirmed, leading to obstructive jaundice. Endoscopic treatment with sphincterotomy of the pancreatic duct was successful. Herein, we report…
