Lateralizing value and semiology of ictal limb posturing and version in temporal lobe and extratemporal epilepsy.

PURPOSE: Unilateral dystonic limb posturing in partial seizures has been shown to be an accurate lateralizing sign indicating seizure onset … ndexes and positive predictive value (PPV) was determined for each sign. RESULTS: In patients with temporal lobe epilepsy (TLE), dystonic posturing occurred in 35.3% (kappa 0.78, positive predictive value (PPV) for the sign being contralateral to seizure onset 92%); tonic limb posturing occurred in 17.7% (kappa 0.36, PPV 40%); unilateral immobile limb occurred …

Asymmetrical blood flow in the temporal lobe in the Charles Bonnet syndrome: serial neuroimaging study.

Clinical features and results of neuroimagings of an 86 year old woman with the Charles Bonnet syndrome are reported. She had become completely blind bilaterally due to cataracts and glaucoma. Shortly after an operation for cataracts, she developed visual hallucinations which lasted for 22 years. She had no deterioration of intelligence. Computed tomography (CT) and magnetic resonance imaging (MRI) showed moderate generalized atrophy, particularly of the temporal lobes. A serial single photon emission computed tomography (SPECT) study during visual hallucinations demonstrated hyperperfusion in…

Atrophy of mesial structures in patients with temporal lobe epilepsy: cause or consequence of repeated seizures?

We studied 70 epileptic patients by using magnetic resonance imaging volumetric measurements of amygdala (AM) and hippocampal formation (HF). Fifty patients presented with intractable temporal lobe epilepsy (TLE), 10 patients had focal extratemporal lobe epilepsy, and 10 had generalized epilepsy. In 91% of the 45 TLE patients without foreign tissue lesions, there was significant smallness of the AM and/or HF coinciding with the side of electroencephalographic seizure onset. No significant smallness or asymmetry was demonstrated in patients with focal extratemporal or generalized epilepsy. We p…

Scene-sensitive Medial Temporal Lobe Subregions Are Recruited for the Integration of Non-scene Stimuli.

A hallmark feature of episodic memory is the ability to flexibly recombine information across episodes to form new associations and guide behavior. This process, termed associative inference, relies on the hippocampus and surrounding medial temporal lobe (MTL) subregions. We previously found that cross-episode binding was improved when episodes were linked by scenes rather than by faces or objects. Here, we tested whether differential recruitment of category-sensitive MTL subregions underlies these behavioral differences. Participants completed study-test phases of the Associative Inference in…

Neuropsychological correlations with fornix and hippocampus in temporal lobe epilepsy demonstrated with diffusion tensor imaging.

OBJECTIVE: Memory dysfunction is a common disabling comorbidity in temporal lobe epilepsy (TLE) and hippocampal sclerosis (HS). While bilateral fornix diffusion abnormalities have been reported in TLE using diffusion tensor imaging (DTI), the impact of fornix pathology on cognitive outcomes remains unclear. This study investigated the relationships between fornix DTI, hippocampal volume, and cognitive outcomes in TLE. METHODS: Thirty-five TLE patients (21 unilateral HS, 11 non-HS, 3 bilateral HS) and 22 controls underwent fornix FLAIR-DTI (1.2 × 1.2 × 2 mm3 resolution) and hippocampal DTI (1 …

Graded extent of hippocampal resection is related to neuropsychological outcomes in temporal lobe epilepsy surgery.

OBJECTIVE: Surgical resection for epilepsy seeks to maximize seizure freedom while minimizing new neurocognitive impairments. Tailored resections guided by anatomoelectroclinical (AEC) hypotheses offer the possibility of sparing parts of the hippocampus. The relationship between the extent of hippocampal resection and postoperative neurocognitive outcomes in this context has not been studied and has important implications for clinical practice. We test this relationship in a series of left and right tailored anterior temporal lobectomy (ATL) surgeries. METHODS: We conducted a retrospective an…

Apoptosis and Glial Marker Profiles in Hippocampus and Amygdala of Medically Intractable Temporal Lobe Epilepsy.

Medically intractable temporal lobe epilepsy is associated with potential neuronal and glial damage in key brain structures, including the hippocampus and amygdala. This study aimed to examine apoptosis, caspase expression, and glial activation markers in these regions, and to evaluate their associations with clinical variables. Hippocampal and amygdala tissues from 17 patients with medically intractable epilepsy and autopsy-derived non-epileptic controls were examined for cellular apoptosis (TUNEL), caspase-3, -9, and -12, as well as GFAP (astrocytic marker) and IBA1 (microglial marker) expre…

Structural and functional alterations of medial temporal lobe in major depressive disorder with anxious distress.

Major Depressive Disorder (MDD) with anxious distress is a severe subtype. The hippocampus and parahippocampal gyrus are implicated in MDD, but findings are inconsistent. This study investigated structural and functional alterations in these regions in a clinical MDD cohort with anxious distress. We analyzed data from 78 MDD patients with anxious distress and 60 healthy controls. Structural MRI volumes and resting-state functional metrics [Amplitude of Low-Frequency Fluctuations (ALFF), fractional ALFF (fALFF), Regional Homogeneity (ReHo)] were extracted for bilateral hippocampus and parahippo

Astrocyte subtype-specific alterations in the dentate gyrus of individuals with mesial temporal lobe epilepsy.

OBJECTIVE: Epilepsy affects approximately 50 million people worldwide and, although primarily attributed to neuronal dysfunction, increasing evidence highlights a critical role of glial cells, particularly astrocytes, in the pathophysiological mechanisms. Mesial temporal lobe epilepsy (MTLE), the most common form of drug-resistant epilepsy, is frequently associated with hippocampal sclerosis (HS) and pronounced astrogliosis. Given the limited efficacy of current anti-seizure medication (ASM) and the side effects of surgical hippocampus removal, there is a need for more specific and effective t…

Transient loss of wakefulness with falls in temporal lobe epilepsy: A retrospective single-centre study.

INTRODUCTION: Focal impaired awareness seizures in temporal lobe epilepsy (TLE) typically involve behavioural arrest and automatisms with preserved posture. However, some patients can exhibit a transient loss of wakefulness with fall without any overt motor manifestations, a phenomenon previously described as "temporal lobe syncope." However, previous reports did not always rigorously exclude FBTCS or ictal asystole. Herein, we aimed to reappraise this semiology by excluding these conditions using simultaneous video-electroencephalogram (VEEG) and electrocardiogram (ECG), clarifying its clinic…

Targeted senotherapy improves electrographic and behavioral outcomes in a mouse model of temporal lobe epilepsy.

OBJECTIVE: Current pharmacotherapy for temporal lobe epilepsy (TLE) is limited to symptomatic treatment and leaves approximately one third of patients with inadequate seizure control. Discovering disease-modifying targets is an unmet clinical need. We have previously identified senescent cells (SCs) as one such target. Many drugs that eliminate SCs (senolytics) interfere with apoptotic resistance proteins, potentially resulting in broad cytotoxicity and numerous side effects. Newer, more targeted therapies, like selective senescence killing compound 1 (SSK1), a gemcitabine prodrug that is sele…

Contribution of the Brown Location Test to the pre-surgical neuropsychological assessment of temporal lobe epilepsy.

The nature of memory deficits in mesial temporal lobe epilepsy (mTLE) can depend on the side of the epileptogenic zone (EZ). Although left mTLE has been consistently associated with impairments in verbal memory, the association between right mTLE and non-verbal memory is less clear, which may be attributable to methodological limitations of existing visuospatial memory tests (e.g., possible use of verbalization strategies, involvement of psychomotor or visuospatial skills, task structure which differs between verbal and non-verbal memory tests). The Brown Location Test (BLT) was developed to e…

The possible role of neurogenesis activators in temporal lobe epilepsy: State of art and future perspective.

Neurogenesis is a complex process by which the neurons and supporting cells of the central nervous system (CNS) are generated by neural stem … ctivates the mobilization of neural stem cells during neurogenesis to substitute for neural loss in temporal lobe epilepsy (TLE), which is the most refractory type of epilepsy. Moreover, recurrent epileptic seizures in TLE trigger neurogenesis in certain brain regions. However, AHN is a transient acute epileptic seizure that terminated with 1-4 weeks following …

Investigating the protective effect of hydroxylated fullerenes on cognitive function in rats with temporal lobe epilepsy.

The objective of this study was to explore the protective effects of hydroxy fullerenes (HFs) on cognitive function in rats with temporal lobe epilepsy (TLE) and to elucidate the underlying mechanisms. Eighteen Sprague-Dawley (SD) rats were randomly selected and administered pilocarpine (50 mg/kg) intraperitoneally to establish a TLE model, and were then randomly assigned to the TLE group and the TLE + HFs group. An additional nine SD rats were served as a normal control group (CON group). The Morris water maze (MWM) test was utilized to assess the spatial learning and memory capabilities of t…

Demonstration of impaired facial emotion perception in temporal lobe epilepsy by theta responses in EEG.

OBJECTIVE: Temporale lobe and occipito-temporal cortical areas play an important role in facial emotion perception (FEP). FEP might be represented by event-related brain oscillations. In patients with temporal lobe epilepsy (TLE), impairment of FEP was shown with behavioral and functional neuroimaging (FMRI, PET, MEG) but not event-related oscillations, which is a well known method for cognitive research studies. The present study aims to explore FEP by analyzing EEG event-related theta oscillations in patients with TLE. METHODS: 21 patients with TLE and 19 healthy volunteers were included. D…

The Effects of Neuronal Fyn Knockdown in the Hippocampus in the Rat Kainate Model of Temporal Lobe Epilepsy.

Previous studies have demonstrated neuronal and microglial Fyn, a Src family kinase (SFK), and how its interactions with tau contribute to epileptogenesis. Saracatinib, a Fyn/SFK inhibitor, modifies disease progression in rat kainate (KA) epilepsy models. In this study, we investigated neuronal-specific fyn knockdown effects on Fyn-tau signaling, neurodegeneration, and gliosis using a calcium/calmodulin-dependent protein kinase II (CaMKII)-promoter-driven adeno-associated viral vector (AAV9)-mediated fyn-shRNA injection in the rat hippocampus. Eight days following AAV administration, rats rece
