Acromioclavicular and Coracoclavicular Ligament Reconstruction for Acromioclavicular Joint Instability.

Numerous operative techniques have been described for acromioclavicular and coracoclavicular ligament reconstruction for the treatment of acromioclavicular joint instability. Injuries of this nature are commonly sustained by contact-sport athletes in high-impact collisions or falls. Traumatic injury to the acromioclavicular joint and ligamentous structures can range in severity, as can the degree of subsequent joint instability. Injuries classified between Type I and Type III are generally treated nonoperatively, whereas Type-IV injuries may be treated similarly to the treatment described in t

Lower Extremity Injuries Due to Chainsaw During Four-Year Periods.

Background Chainsaws cause injuries mostly on the upper extremities, then on the face and lower extremities. In the literature, there are many studies about hand and face injuries; however, articles about lower extremity injuries are limited. The aim of the study is to define injury patterns, treatments, and results of the cases that we have encountered in our hospital and to evaluate precautions after reviewing the literature. Methods Patients admitted to our hospital's Emergency Department with chainsaw-related lower extremity injuries between 2016 and 2021 are evaluated. Patients' demograph

MMP inhibition as a potential method to augment the healing of skeletal muscle and tendon extracellular matrix.

The extracellular matrix (ECM) of skeletal muscle and tendon is composed of different types of collagen molecules that play important roles in the transmission of forces throughout the body, and in the repair and regeneration of injured tissues. Fibroblasts are the primary cells in muscle and tendon that maintain, repair, and modify the ECM in response to mechanical loading, injury, and inactivity. Matrix metalloproteinases (MMPs) are enzymes that digest collagen and other structural molecules, which are synthesized and excreted by fibroblasts. MMPs are required for baseline ECM homeostasis, b

Effects of superficialis tendon repairs on lacerated profundus tendons within or proximal to the A2 pulley: an in vivo study in chickens.

PURPOSE: In a flexor tendon injury model in chickens we undertook a study to evaluate effects of the flexor digitorum superficialis (FDS) tendon repairs on excursions, work of flexion, and adhesions of the repaired flexor digitorum profundus (FDP) tendon after their injuries within or proximal to the equivalent of the A2 pulley and early tendon motion. METHODS: Thirty-five leghorn chickens were divided into 3 groups. In group 1 the FDS and FDP tendons of the long toes on both sides were transected in the area covered by the pulley. In group 2 the tendons were transected proximal to the pulley

[Reconstruction of the heel in a two-year-old boy after lawn mower injury].

INTRODUCTION: Lawn mower injuries in children usually involve the lower extremities and can lead to serious amputation injuries. Treatment should look not only at the acute reconstruction, but also on maintaining the ability to grow. We report the case of a two-year-old boy with amputation of the heel. CASE REPORT: The boy was run over by a lawn mower. He suffered a complete loss of heel soft tissue, 30 % of the os calcis and the Achilles tendon. The one-stage reconstruction was performed by transplantation of an iliac crest graft, fascia lata to reconstruct the Achilles tendon and a microvas

Stress-related injuries around the lesser trochanter in long-distance runners.

OBJECTIVE: Imaging abnormalities around the lesser trochanter are occasionally found in long-distance runners, yet little research has been conducted concerning this area of the hip. In addition, the relation between iliopsoas insertional abnormalities at the lesser trochanter and femoral neck stress injuries has not been examined, to our knowledge. We report MRI findings at the lesser trochanter in nine long-distance runners with hip or groin pain and a consistent constellation of the following findings: abnormalities associated with the iliopsoas tendon and its insertion, including marrow ed

Musculoskeletal ultrasound abnormalities in professional rugby union players in the United States.

This cross-sectional observational study aimed to characterize the prevalence of musculoskeletal sonographic abnormalities among players (N = 23) in a Major League Rugby team during preseason medical evaluations. Bilateral ultrasound examinations were performed on the acromioclavicular (AC) joints, quadriceps, patellar, and Achilles tendons. Players most commonly reported prior injuries to the ankle/foot, leg, and shoulder. Ultrasound findings revealed a high prevalence of abnormalities across all examined sites: 65.2% in the AC joints, 70.6% in the quadriceps tendons, 100% in the patellar ten

Flexor tenolysis.

Tenolysis is a well-established salvage procedure, which can be applied when non-gliding adhesions form along the surface of a tendon after injury or repair and prevent gliding of the tendon in the performance of its intended function.(8,15,17,29,30,39,40,44,49) Tendon adhesions will occur whenever the surface of a tendon is damaged either through the injury itself, be it laceration or crush, or by surgical manipulation.(18) At any point on the surface of a tendon where violation occurs, an adhesion will form in the healing period.(20,26) When these adhesions cannot be mobilised by an adequate

Flexor Carpi Radialis Tendon Rupture Associated With a Distal Radius Fracture: A Case Report Highlighting Preoperative Three-Dimensional CT Findings.

Flexor carpi radialis (FCR) tendon rupture associated with a distal radius fracture is a rare entity. We report a case of a 48-year-old man who sustained a Colles-type distal radius fracture complicated by an FCR tendon rupture at the time of injury. The rupture was first identified intraoperatively during open reduction and internal fixation. Radiographs demonstrated a prominent volar radial third fragment along the anatomical course of the FCR tendon. Retrospective three-dimensional (3D) reconstruction of preoperative CT images using soft-tissue settings clearly revealed discontinuity of the

Gap junction protein expression and cellularity: comparison of immature and adult equine digital tendons.

Injury to the energy-storing superficial digital flexor tendon is common in equine athletes and is age-related. Tenocytes in the superficial digital flexor tendon of adult horses appear to have limited ability to respond adaptively to exercise or prevent the accumulation of strain-induced microdamage. It has been suggested that conditioning exercise should be introduced during the growth period, when tenocytes may be more responsive to increased quantities or intensities of mechanical strain. Tenocytes are linked into networks by gap junctions that allow coordination of synthetic activity and

Patellar Tendon Repair With Bioinductive Implant Augmentation.

Patellar tendon ruptures are relatively uncommon injuries that often occur in the setting of chronic patellar tendinopathy, particularly in those participating in high-level pivoting sports and jumping athletics secondary to overuse. Tear patterns vary, with the most common being avulsions from the inferior pole of the patella. For complete tears resulting in extensor mechanism compromise, surgical treatment typically entails acute repair. In this technique, we describe the use of a bioinductive bovine collagen implant to augment a patellar tendon repair in the setting of chronic patellar tend

Medial Ankle Instability: The Deltoid Dilemma.

Diagnosis and treatment of medial ankle instability (MAI) are still controversial and poorly discussed in literature. The purpose of this review is to highlight different clinical presentations of MAI and develop a guide for its management. The deltoid ligament complex is injured more commonly than expected, because deltoid ligament injuries may either be isolated or occur in combination with other lesions, such as lateral ankle ligament injury, posterior tibial tendon insufficiency, osteochondral lesion, and others. The presence of a pes planovalgus deformity in a patient without posterior ti

Boxer's knuckle--dorsal capsular rupture of the metacarpophalangeal joint of a finger.

An injury to the dorsal aspect of the metacarpophalangeal joint of a finger usually damages the sagittal fibers of the extensor tendon mechanism. Rupture of the underlying joint capsule, a far more serious injury, has received scant attention in the medical literature. The problem is often unrecognized because the extensor tendon may remain in its normal midline position. Eight dorsal capsular ruptures are described in six patients who had closed direct trauma to either the knuckle of their index or long fingers. Most of the injuries occurred in professional boxers.

Triceps brachii rupture: case report.

A 44-year-old man has been seen by the present authors, apparently the third reported case of triceps brachii rupture. He had had bilateral nephrectomies one year earlier and since then has been medicated with Dilantin for grand mal seizures which followed hypovolemia during dialysis. A grand mal seizure immediately preceded the patient's right triceps brachii rupture and other multiple orthopaedic injuries. Following repair of the tendon defect the patient regained an active range of motion. The possible relationship of tendon rupture and avulsion to primary and secondary hyperparathyroidism

Complications of flexor tendon repair.

This article reviews and highlights complications of flexor tendon repairs. Although the outcomes of flexor tendon repairs have improved over the years, fair or poor functional outcomes are seen, especially in patients whose trauma involves multiple structures of the hand and in zone 5 with multiple tendon lacerations. Rupture of the flexor tendon after repair is no longer a major problem if current repair principles are carefully adhered to. Different degrees of adhesion formations and interphalangeal (IP) joint stiffness still occur in a few patients. Early active postoperative mobilization

Preclinical tendon and ligament models: Beyond the 3Rs (replacement, reduction, and refinement) to 5W1H (why, who, what, where, when, how).

Several tendon and ligament animal models were presented at the 2022 Orthopaedic Research Society Tendon Section Conference held at the University of Pennsylvania, May 5 to 7, 2022. A key objective of the breakout sessions at this meeting was to develop guidelines for the field, including for preclinical tendon and ligament animal models. This review summarizes the perspectives of experts for eight surgical small and large animal models of rotator cuff tear, flexor tendon transection, anterior cruciate ligament tear, and Achilles tendon injury using the framework: "Why, Who, What, Where, When,
