Peroneal tendons subluxation.

Subluxation of the peroneal tendons is uncommon. It occurs especially in skiing, soccer, basketball, rugby, ice skating, judo, sprint, water-skiing, mountaineering, and gymnastics. We present an overview of the injury, with the classification commonly used. Many surgical techniques have been described to manage recurrent subluxation of the peroneal tendons, but only Level IV/Grade C evidence has been produced. Thus, randomized controlled trials are necessary to determinate the best surgical management method. It appears that high-demand individuals should be primarily managed surgically, and r

[Rotator cuff tears: diagnosis and therapy].

Since the life span of our society is increasing and the expectation of high functional demands is growing more and more older people take part in sports activities emphasises the necessity of early diagnosis of rotator cuff lesions and their correct treatment. The goal of any diagnostic means should be a proper treatment of rotator cuff lesions according to the detected pathology. New arthroscopic techniques to surgically reconstruct rotator cuff tears have brought the operative repair techniques another step further. In cases of large and complex cuff tears the surgeon can individually selec

SPLATT combined with tendo achilles lengthening for spastic equinovarus in adults: results and predictors of surgical outcome.

During a 4-year period, split anterior tibial tendon transfer (SPLATT) was performed on 42 adults with cerebrospastic equinovarus deformity. Twenty-one patients (24 feet) had a minimum 1-year follow-up, which included detailed documentation of foot appearance position and function as well as ambulatory status. Thirteen patients were male and 8 were female. Average age of the patients was 41 years. Seventeen patients were independent ambulators with orthoses, one was a maximally assisted ambulator. Three patients with spastic quadriparesis were nonambulatory. All patients had uniform surgical t…

Anterior cruciate ligament functional instability. A static intra-articular and dynamic extra-articular procedure.

The static and functional results in a series of 50 patients evaluated at least two years after surgery were gratifying. None had any episodes of functional instability. Forty-six of the 49 who participated in sports prior to operation returned to all desired sports, including six patients who returned to intercollegiate and national team sports. The overall subjective and objective results were 30, excellent; 17, good; one, fair, and two, failure. Of the two failures, one had excellent static stability but significant pain due to pre-existing chondromalacia patella, and the other had a postsu

Cerebral palsy: a podiatric overview.

Cerebral palsy is a nonprogressive neurologic lesion occurring during gestation or at birth with resultant motor deficit. It can be classified anatomically, according to the cerebral lesion, or functionally. In order to reach a diagnosis, and subsequently determine the correct treatment, a thorough evaluation of the patient must be performed including muscle testing, persistence of pathologic reflexes, and presence of infantile automatisms. The most frequently seen deformities of the lower extremity are equinus, equinovalgus, and equinovarus. The underlying goal of treatment is to stabilize th

Various soft tissue defect reconstructions using anterolateral thigh and vascularized fascia lata composite free flap.

The anterolateral thigh flap (ALT) is versatile for soft-tissue reconstruction of various body defects because of its thick and vascularized fascia component. We present our clinical experience with the functional one-stage reconstruction of complicated soft-tissue defects using ALTs with vascularized fascia lata (FL). Between April 2018 and February 2022, we transferred ALTs with FL components for various soft-tissue defects in 15 patients. The FL component was used for reconstruction of hand & forearm tendon, medial and lateral patellar synovial membrane, plantar aponeurosis, abdominal wall,

Composite palmaris longus-venous flap for simultaneous reconstruction of extensor tendon and dorsal surface defects of the hand--long-term functional result.

Soft tissue loss in the dorsum of the hand resulting from trauma often involves the extensor mechanism due to its thin skin coverage. When such composite skin and extensor tendon defects occur, the wound could be reconstructed with an ipsilateral forearm venous flap with accompanying palmaris longus tendon. Eighteen dorsal finger defects in 12 patients underwent composite venous flap and tendon transfers in the acute stage. We used both Buck-Gram-cko and Tubiana's methods for objective functional assessments. With the former method, the average composite flexion, extension deficit, and total a…

Bilateral snapping biceps femoris tendon: a case report and review of the literature.

Snapping biceps femoris tendon is an uncommon cause of lateral knee pain. We report the case of a 15-year-old athlete with bilateral snapping biceps femoris tendons, painful over his right lateral leg during exercise. He underwent elective exploration of the right knee which revealed an accessory biceps femoris tendon with attachment to the anterolateral tibia. The accessory tendon was released and reinserted onto the fibular head with a Krackow suture. There were no perioperative complications, and he returned to full sporting activities within 3 months. We describe the operative technique us

Early dynamic motion versus postoperative immobilization in patients with extensor indicis proprius transfer to restore thumb extension: a prospective randomized study.

Transfer of the extensor indicis proprius is the gold standard for reconstruction of the extensor pollicis longus tendon to restore thumb extension. Twenty patients were included to a prospective randomized trial to assess whether an early dynamic motion protocol yields a better outcome than immobilization. Evaluation included postoperative range of motion, grip strength, duration of treatment, and time off work. Ten patients of each group had the thumb immobilized in an extension thumb spica cast for 3 weeks after surgery or underwent an early dynamic motion protocol. Follow-up examinations w

Reconstruction with tenodesis in an adult flatfoot model. A biomechanical evaluation of four methods.

Six fresh-frozen adult cadaveric specimens were mounted in an Instron materials testing machine with use of a cemented intramedullary rod. Angular relationships between the first metatarsal and the talus were recorded with a sonic digitizer. A flatfoot deformity was created by dividing the talonavicular joint capsule (superiorly, medially, and inferiorly), the spring ligament, the anteromedial aspect of the subtalar joint capsule, and the plantar fascia. Angular displacement in the sagittal and transverse planes was recorded at no load and at 100, 350, and 700-newton plantar loads. Each specim

Treatment of tendon injuries in children.

Tendon injuries in the child's hand can be repaired primarily with better results than in the adult; this is true at all levels, including the digital sheath. In less than ideal circumstances, secondary conventional tendon grafting also provides predictable satisfactory results and can also salvage a failed primary repair. In children under the age of six years with digital flexor tendon repairs, the limb must be immobilized above the elbow. In the cooperative child over the age of six years with a primary repair of a lacerated flexor tendon, the dorsal splint with rubber band traction as desc

Unravelling the Distinct Phenotype and Mechanosensitive Properties of Different Tendon Cell Populations.

Tendinopathy arises from maladaptive cellular responses, though the drivers remain unclear. Here we isolate and characterise an overlooked tendon cell population residing within interfascicular matrix (IFM), demonstrating its importance as a highly mechanosensitive cell in tendon. We describe the first successful isolation and long-term culture of enriched populations of primary IFM and fascicular matrix (FM) cells, enabling direct comparison of their phenotypes and mechanosensitivity. IFM cells exhibited a potent response to stiff substrates, displaying cytoskeletal remodelling, rapid driftin

Parsonage-Turner syndrome and a localised swelling around the extensor tendons of the hand: a clinical sign indicating increased risk of rupture of the tendon.

Two patients presented with intense pain in the shoulder followed by weakness and paralysis of muscles in the shoulder and arm, together with a peculiar swelling on the hand. The swelling seems to predispose to rupture of the extensor tendon. If this tumour is a regular finding in Parsonage-Turner syndrome, it may also help in making a definitive diagnosis.
