Drastic Reduction in Time to Controlled Viral Load in People With Human Immunodeficiency Virus in France, 2009-2019: A Longitudinal Cohort Study.

BACKGROUND: Aspirational targets to end AIDS by 2030 include having 95% of people with human immunodeficiency virus (HIV; PWH) diagnosed, 95% treated, and 95% with controlled viral load (VL). Our objective was to describe, using a large French prospective cohort, the median transition times through the cascade of care between 2009 and 2019. METHODS: We analyzed patients whose first HIV diagnosis was made between 1 January 2009 and 31 December 2019. Using the Kaplan-Meier method, we estimated the time to linkage to care (from HIV diagnosis to first biological assessment), to treatment (date of

A standardized care pathway increases optimal dialysis starts.

OBJECTIVE: To determine whether an intensive value-based care educational program that includes a standardized end-stage renal disease (ESRD) transition pathway would improve the number of optimal starts within Kidney Contracting Entities (KCEs). STUDY DESIGN: Retrospective cohort study. METHODS: We recorded optimal starts, defined as the initiation of dialysis without a central venous catheter, and the initial modality type (hemodialysis vs peritoneal dialysis [PD]) in adult Medicare patients in a Comprehensive Kidney Care Contracting program. The setting was 4 KCEs within a single physicia

Adulting in the Current Era: Piloting an Individualized In-Person Congenital Heart Disease Transition Education Program.

Adolescents with congenital heart disease (CHD) have varying degrees of readiness to transition to adulthood, including CHD knowledge, disease management skills, and engagement in the transition process. The objective of our study was to determine if educational and skill building visits in an individualized in-person transition program were feasible and improved these aspects of transition readiness. We conducted a retrospective cohort study analysis of an in-person transition program from 2019 to 2021. Inclusion criteria included patients aged 14-21 years old with age-appropriate neurodevelo

Steps of care for adolescents with anorexia nervosa-A Delphi study.

OBJECTIVE: Anorexia nervosa (AN) commonly develops during adolescence. Existing literature offers some treatment guidelines, but clear clinical criteria for initial recommendations and steps of care are needed. The aim of the present study was to develop expert consensus for a stepped-care algorithm for treatment of adolescents with AN. METHOD: The Delphi approach was used to identify clinical parameters that guide initial treatment recommendations and recommendations for transitions between levels of care. The Delphi approach provides a useful expert consensus when empirical data are limited

Linkage to hepatitis C care after incarceration in jail: a prospective, single arm clinical trial.

BACKGROUND: Hepatitis C virus (HCV) is a major public health problem in correctional settings. HCV treatment is often not possible in U.S. jails due to short lengths of stay. Linkage to care is crucial in these settings, but competing priorities complicate community healthcare engagement and retention after incarceration. METHODS: We conducted a single arm clinical trial of a combined transitional care coordination (TCC) and patient navigation intervention and assessed the linkage rate and factors associated with linkage to HCV care after incarceration. RESULTS: During the intervention, 84 p…

Assessing the Temporality Between Transitions onto Opioid Agonist Therapy and Engagement with Antiretroviral Therapy in a Cohort of HIV-Positive People Who Use Opioids Daily.

A robust evidence-base describes the beneficial association between opioid agonist therapy (OAT) and HIV-related outcomes among people living with HIV and opioid use disorder. While some evidence suggests the stabilizing effect of OAT on antiretroviral therapy (ART) treatment engagement, less is understood about the potential for an inverse relationship. We sought to examine the relationship between transitions in ART engagement and transitions onto OAT. We used data from a prospective cohort of people living with HIV who use drugs in Vancouver, Canada-a setting with no-cost access to ART and

Fifty years of bone tumors.

There have been enormous advances in the treatment of bone tumors over the past half-century. The most notable of these has been the transition from amputation as the standard of care to limb salvage surgery. This transition is the result of advances in imaging techniques, accurate diagnosis, systemic therapies (including chemotherapy), and prosthetic design for the reconstruction of musculoskeletal defects. Advances have also been made in the management of benign and metastatic bone tumors.

Providing better opportunities for older children in the child welfare system.

A growing number of children over age 10 reside in and emancipate from foster care every year. Older children face many of the same challenges as younger children, but they also have unique developmental needs. This article discusses older children in the child welfare system and finds: Approximately 47% of children in foster care are over age 11, and in 2001, 20% of children leaving foster care were over age 16. Older children need permanency, stability, and a "forever family." Maintaining connections with siblings and other kin can be a crucial resource for older children as they transition

Transition Regret and Detransition: Meanings and Uncertainties.

Gender transition is undertaken to improve the well-being of people suffering from gender dysphoria. However, some have argued that the evidence supporting medical interventions for gender transition (e.g., hormonal therapies and surgery) is weak and inconclusive, and an increasing number of people have come forward recently to share their experiences of transition regret and detransition. In this essay, I discuss emerging clinical and research issues related to transition regret and detransition with the aim of arming clinicians with the latest information so they can support patients navigat

Outcome of transition phase patients with juvenile idiopathic arthritis.

OBJECTIVES: Across diagnosis groups, successful transition of adolescent and young adults from children's hospitals to adult care is often associated with decreased treatment adherence and treatment results. The aim of this study was to characterize disease activity and anti-rheumatic medications following transfer of care of juvenile idiopathic arthritis (JIA) patients to the adult clinic. METHOD: All consecutive JIA patients aged 16-20 years who visited the specific transition clinic in the rheumatology outpatient clinic of Helsinki University Hospital between November 2012 and May 2013 and

Dynamic modeling in medical technology assessment. Fitting hearing aids in The Netherlands.

OBJECTIVES: The main objective of this article is to demonstrate the usefulness of dynamic modeling for an economic assessment of technology in health care. Specifically, this approach is applied to assess the impact of the use of hearing aids in Dutch health care. METHODS: The population is divided into different health classes between which, over time, transitions occur. Transition probabilities are derived from exogenous data. The transitions are associated with economic and societal costs and benefits. People who are satisfied with their hearing aids experience benefits. These benefits ar

Transmural collaboration in lifestyle care: views of healthcare professionals and individuals with spinal cord injury, cauda equina syndrome, or spina bifida.

PURPOSE: Individuals with spinal cord injury, cauda equina syndrome or spina bifida often face challenges in maintaining a healthy lifestyle. A coordinated lifestyle care pathway beginning in the rehabilitation center and continuing into primary care may support the conditions needed for sustainable lifestyle behavior. This study aimed to identify determinants to inform the development and implementation of a lifestyle care pathway. METHODS: We conducted semi-structured interviews with 19 healthcare professionals and 24 individuals with spinal cord disorders. Separate topic guides were develo

A Novel Reentry Hotline Led by Community Health Workers to Address the Health Needs of People Returning From Incarceration: Transitions Clinic Network, California, June 2020‒June 2023.

Community members returning from incarceration experience serious health disparities, which intensified during the COVID-19 pandemic. In response, community health workers with lived experience of incarceration in the Transitions Clinic Network established a California reentry health care hotline to support time-sensitive linkages to postrelease medical care. From June 2020 to June 2023, the hotline supported 1276 people transitioning from incarceration. Lessons from this peer-led public health intervention are relevant for states utilizing Medicaid waivers to improve health equity for justice

Propensity-matched study of enhanced primary care on contact with the criminal justice system among individuals recently released from prison to New Haven.

BACKGROUND: Health systems can be integral to addressing population health, including persons with incarceration exposure. Few studies have comprehensively integrated state-wide data to assess how the primary care system can impact criminal justice outcomes. We examined whether enhanced primary care can decrease future contact with the criminal justice system among individuals just released from prison. METHODS: We linked administrative data (2013-2016) of Connecticut Department of Correction, Department of Mental Health and Addiction Services, Department of Social Service, Court Support Serv

'Hearts' and 'minds': Illustrating identity tensions of people living and working through marketising policy change of allied health disability services in Australia.

Service-based caring sectors like disability are increasingly being operated via market logic, including shifts towards personalised funding. These shifts must be brought to life in/through people already located in relation to ideas and values that underpin historical policies. Our manuscript examines how identities are re/shaped in relation to marketised policy change and explores how identity change unfolds (or not) during periods of transition: situated within the transition to the National Disability Insurance Scheme executed in Australia as a major disability funding reform. Our qualitat

Adolescent and Adult Transitions From Major Depressive Disorder to Bipolar Disorder.

IMPORTANCE: Bipolar disorder (BD) often first appears in adolescence after onset of major depressive disorder (MDD), but diagnosis and treatment are commonly delayed. This delay is a concern because untreated BD is associated with adverse long-term outcomes, a more recurrent disease course and difficult-to-treat illness, and suicide attempts and deaths. OBJECTIVE: To examine the association of age at MDD onset with early transition to BD and the subsequent use of psychiatric inpatient services as a severity indicator. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study analyze

Deep Brain Stimulator Surgery Does Not Require Postoperative Intensive Care Unit Admission.

BACKGROUND: Historically, patients in an academic tertiary care center were admitted to the intensive care unit (ICU) after Deep brain stimulation (DBS) placement. DBS patients progressed quickly through the ICU and did not require traditional ICU management. We identified an opportunity to shift DBS postoperative care from the ICU to the floor. METHODS: Key stakeholders were engaged to create a protocol to transition postoperative DBS patients from ICU to medical-surgical care. Forty-one DBS patients were admitted postoperatively to the ICU in the 6 months before implementation of the new pro

Talking with adolescent and young adult cancer survivors about health after cancer: A review and communication guide for clinicians.

Adolescent and young adult (AYA) cancer survivors represent a vulnerable population in cancer care and survivorship. AYA survivors are a heterogeneous group that includes people between the ages of 15 and 39 years who were treated for cancer during their childhood or AYA years, at which time they had variable agency and may have received cancer care in pediatric or adult settings. AYA survivors experience one or multiple health care transitions, moving from active oncology to posttreatment survivorship and/or from pediatric to adult care. Clinician communication that centers the needs and pref
