The purpose of this study was to identify and describe the outcome of motor vehicle crashes for adolescents and young adults with positive toxicology screens. A retrospective design was used to collect data for 134 subjects ranging in age from 15 to 25 at an urban Level II trauma center. Outcomes related to sex, age, injury-severity score, length of stay, and hospital cost were analyzed using multiple regression, and the relationship was significant (p < .0001). Spearman's correlation analysis resulted in a significant relationship between survival outcome, injury severity, discharge, and hosp

OBJECTIVE: To describe the frequency, characteristics, and circumstances of pediatric sledding-associated injuries and to assess the validity of published risk factors and prevention measures for these injuries.
METHODS: A retrospective, descriptive study of patients admitted to the 25 accredited trauma centers in Pennsylvania.
RESULTS: Two hundred twenty-six patients were admitted for sledding-related injuries. Sixty-nine percent were male. The mean ICU LOS was 1.2 days (SD = 3), and the mean hospital LOS was 7 days (SD = 7.2). Ninety-eight percent were discharged alive. Forty-seven percent

Diffuse axonal injury (DAI) is a severe form of traumatic brain injury that is seen almost exclusively following motor vehicle accidents. It is associated with immediate coma lasting from six hours to prolonged coma. The author reviews the mechanism of injury, significant neurological findings, neuropathology, and diagnostic criteria. Priorities for nursing care in the emergency department, critical care, and inpatient and rehabilitation units are outlined.

INTRODUCTION: Patients with rib fractures from traumatic injuries may suffer increased morbidity, an increased hospital stay, an increased length of time in the intensive care unit (ICU), pulmonary complications resulting in the need for mechanical ventilation, and increased mortality. Some studies have focused on developing specific scoring systems to triage and to help identify patients most at risk for the most severe complications. One such protocol is the RibScore. At our institution, we use and modified the Pain, Inspiratory Effort, Cough score (mPIC score) to help stratify patients most

BACKGROUND: Several types of pressure sources, including underwater bubble devices, mechanical ventilators, and the Infant Flow Driver, are used for providing continuous positive airway pressure (CPAP) to preterm infants with respiratory distress. It is unclear whether the use of bubble CPAP versus other pressure sources is associated with lower rates of CPAP treatment failure, or mortality and other morbidity. OBJECTIVES: To assess the benefits and harms of bubble CPAP versus other pressure sources (mechanical ventilators or Infant Flow Driver) for reducing treatment failure and associated m

A cross-sectional survey of the prevalence of eye injuries among primary school children in two noncontiguous local government areas of Enugu State of Nigeria was undertaken. One of the local government areas was urban, while the other one was rural. Children who were <15 years in two randomly selected primary schools in the urban area and three randomly selected schools in the rural area were interviewed and examined with Snellen chart, pen torch, head loupe, and direct ophthalmoscope. The findings were recorded using a semi-structured questionnaire and the World Health Organization Programme

Trauma can cause visible, often profound physical injuries for patients. The emotional and social drain that families and health care providers experience can also be life-altering. REBUILD is a program designed by a level 1 trauma center that incorporates former patients and their families in a support group to aid previous and new patients, their families, and care providers by sharing mutual experiences and learning from each other. Health care providers have found that participating in the program has been personally gratifying and professionally beneficial by preventing burn-out.

Bioterrorism is an old tactic used in warfare. The use of fatal diseases or man-made poisons to subdue others has not been limited to wartime. Trauma personnel should be aware of commonly used agents, the signs and symptoms of their exposure, and recommended treatment. Children can be victims of terrorists' acts and need special attention because of their unique response to exposure, varying dosages of medications, and lack of active immunity.

Serious errors can occur with many medical devices (e.g., infusion pumps, ventilators, anesthesia machines), and even highly trained professionals can make critical errors. The FDA now requires that manufacturers consider the user when designing medical equipment and has issued a guidance document describing human factor (HF) problems and the HF design process. The Association for Advancement of Medical Instrumentation has also published guidelines, and the American Medical Association's National Patient Safety Foundation has selected reduction of medical errors as their mission. It is importa

In spite of advances in the care of traumatically injured patients, management of adult respiratory distress syndrome has continued to challenge clinicians since it was first described. Recently the alveolar macrophage, which elaborates proinflammatory mediators, has been implicated in the pathogenesis of acute lung injury and eventually in a clinical picture of adult respiratory distress syndrome. This case history presents the use of experimental methods of treatment, including chemical mediator inhibitors, continuous arteriovenous hemofiltration, and pressure-controlled inverse ratio ventil

Nasal fractures are very common in facial trauma but should not be considered minor injuries until they have been thoroughly assessed. Trauma care providers need to have an understanding of common mechanisms of injury, normal anatomy, and assessment techniques to make the diagnosis. Management of the injury will depend on whether the fracture is nondisplaced, displaced, open, or associated with a complication that necessitates a more aggressive approach.

Spinal cord injuries are not as common as many other types of injuries. The victims are often young, the injury debilitating, and the effects devastating and incalculable. The acute management of patients with spinal cord injury can significantly affect the patient's eventual neurologic and functional outcome and ultimately their quality of life. Early interventions are aimed at reestablishing physiologic homeostasis, lessening the amount of secondary injury, and preserving neurologic function.

The care of a patient who became impaled on a large aluminum pipe is presented. A review of the literature reveals that most patients with a type I injury either do not survive or present with an unpredictable pattern of injury. Preoperative care requires rapid stabilization, assessment, and interventions based on the pattern of injury. Perioperative management may involve multiple surgeons performing simultaneous surgical procedures.

PURPOSE: To evaluate the need for obtaining postdischarge chest radiographs for trauma patients who were treated with a thoracostomy tube.
METHODS: A retrospective medical record review was conducted for all patients treated with a thoracostomy tube while admitted to the trauma service at Saint Louis University Hospital over a 12-month period. Patients who died during their hospital stay were excluded.
RESULTS: During the 12-month study period, 155 trauma patients who were treated with a thoracostomy tube were discharged from the hospital. The indications for the thoracostomy tube were pneum

Do perforating lateral osteotomies cause less ecchymosis and edema compared with the popular continuous method? Many studies have demonstrated that perforated osteotomies cause less trauma and periosteal disruption. Numerous investigators have subjectively perceived less postoperative ecchymosis and edema, but no clinical study has compared the perforated methods versus the continuous technique in the same patient. This prospective, randomized, partially blinded study was designed to test the hypothesis that the perforating method causes less postoperative ecchymosis and edema compared with th
