Easy visualization of the membranous urethral stump in radical prostatectomy.

PURPOSE: Access to the urethral stump is sometimes difficult during radical retropublic prostatectomy. MATERIALS AND METHODS: A 20F Foley catheter with the 5 ml. balloon inflated with 10 ml. in the bulbous urethra was used to elevate the urethral stump into the operative field. RESULTS: Easy access to the urethral stump was achieved in all patients, which assisted with accurate anastomosis. There was decreased leakage of urine and no anastomotic stricture. No urethral trauma or incontinence was attributable to the technique. CONCLUSIONS: This simple and safe technique allowed easy access to

Lower urinary tract dysfunction in common neurological diseases.

The lower urinary tract has the main function of urine storage and voiding. The integrity of the lower urinary tract nerve supply is necessary for its proper function. Neurological disorders can lead to lower urinary tract dysfunction (LUTD) and cause lower urinary tract symptoms (LUTS). Common causes of neurogenic LUTS or LUTD include spinal cord injury, multiple sclerosis, Parkinson's disease, cerebrovascular accidents, cauda equina syndrome, diabetes mellitus, and multiple system atrophy. The pathophysiology is categorized according to the nature of the onset of neurological disease. Assess

[Urinary infections during pregnancy and methods of prevention].

Urinary tract infections in pregnant women develop owing to the short urethra in women, mechanical pressure of the uterus onto atonic urethra during pregnancy, atony of the urinary bladder caused by the level of progesterone, iatrogenic factors (catheterization), some metabolic disorders, anemia, obstipation etc. These are frequent reasons why pregnant women over the pregnancy period develop infections of urinary tract, where a significant number of them have bacteriuria showing no symptoms of acute infection whatsoever. The research covered unselectively 4,850 urine samples of pregnant women.

Severe Hyponatremia Precipitated by Acute Urinary Retention in a Patient with Psychogenic Polydipsia.

A woman in her late sixties presented with severe hyponatremia and acute kidney injury (AKI) as consequence of psychogenic polydipsia and acute urinary retention due to urinary tract infection. Urinary catheterization promptly drained 5.5 L of urine with resulting polyuria, leading to an initial swift raise in plasma (P) sodium concentration, disregarding the course of fluid resuscitation. After the polyuric phase, normal range P-sodium levels were reestablished by oral water restriction. Treatment with psychoactive drugs, e.g., zuclopentixol, may have contributed to the severity of the condit…

Pubovaginal fascial sling for the treatment of complicated stress urinary incontinence.

We reviewed retrospectively 67 consecutive women with complicated stress incontinence who underwent a pubovaginal fascial sling procedure by a single surgeon. A detailed micturition questionnaire was completed at the last followup, which ranged from 1 to 8 years, with a mean of 3.5 years. Postoperatively, 82% of the women claimed that they were never incontinent and never wore pads, while 9% were incontinent less often than once per 2 weeks and 9% 9% continued to have troublesome incontinence on a daily basis. Only 2 of these women had persistent stress incontinence; the remainder (5) had urge

Bladder dysfunction in peripheral neuropathies.

Normal bladder function depends on the complex interaction of sensory and motor pathways. Bladder dysfunction can develop as a result of several neurological conditions. It can happen in a number of ways, including diabetic cystopathy, detrusor overactivity, bladder outlet obstruction, and urge and stress urinary incontinence. Diabetic neuropathy is the most common cause of peripheral neuropathy-associated bladder dysfunction. Guillain-Barré syndrome (GBS), human immunodeficiency virus (HIV)-associated neuropathy, chronic inflammatory demyelinating polyneuropathy (CIDP), and amyloid neuropathy

Augmentation cystoplasty and the artificial genitourinary sphincter.

In 18 patients with urinary incontinence secondary to decreased bladder compliance and sphincteric incompetence, augmentation cystoplasty and the artificial genitourinary sphincter were combined successfully to produce complete urinary control. Insertion of the sphincter was simultaneous with augmentation cystoplasty in 11 patients. In 6 patients the artificial sphincter was placed before augmentation cystoplasty. One patient underwent placement of a sphincter subsequent to enterocystoplasty and failed Young-Dees reconstruction of the vesical neck. It is noteworthy that in all 6 patients who u

Hematocolpos causing severe urinary retention in an adolescent girl with imperforate hymen: an uncommon presentation.

INTRODUCTION: Hematocolpos is rarely presented as a pelvic mass which mechanically compresses the bladder and the urethra thereby causing urinary retention. CASE REPORT: A 12-year-old girl referred with the history of lower abdominal pain and retention of urine for 24 h. The patient had not started her menses yet. Three weeks before she also complained of discomfort on passing urine, frequency and urgency and was taken to a local outpatient clinic where she was given antibiotics with the diagnosis of urinary tract infection, she had also the history of intermittent urinary catheterization (th…

Kidney transplantation in patients with an abnormal lower urinary tract.

Several factors influence the decision about which type of urinary drainage is best for any given patient. In a patient who has undergone cystectomy, supravesical drainage is required. However, many patients with upper tract urinary diversion have bladders that can be rehabilitated to provide adequate urinary storage. In most patients who have not had a cystectomy, the bladder can be augmented with bowel to achieve a suitable capacity. The urethral resistance component of continence can be achieved using bladder neck reconstruction, periurethral injection, or artificial urinary sphincter. Inte

Ureteral catheter placement for prevention of ureteral injury during laparoscopic hysterectomy.

AIM: Ureteral injury is among the most devastating complications of gynecologic surgery. Estimated incidence of ureteral injury during laparoscopic hysterectomy is 2.6-35 times (0.2-6.0%) that in abdominal hysterectomy. We investigated preoperative ureteral catheter (UC) placement as a way to prevent ureteral injury in laparoscopic hysterectomy. METHODS: Clinical records of 94 women who underwent laparoscopic hysterectomy between February 2006 and January 2007 in Yazaki Hospital, Kanagawa, Japan, were reviewed retrospectively. Thirty-four patients between February and June 2006 underwent the

[TRANSVAGINAL BLADDER NECK CLOSURE FOR PARAPLEGIC ADULTS - REPORT OF TWO CASES].

We report our initial experiences with transvaginal bladder neck closure for two females with a devastated bladder neck. In each, the bladder neck was closed following a circumferential dissection and the suture line covered using an anterior vaginal flap, with urine draining via a suprapubic cystostomy tube. (Case 1) A female in her twenties was presented with urinary incontinence. The patient had been paraplegic since childhood and initially managed with intermittent catheterization. Later, severe incontinence developed, and Foley catheter was placed. At presentation, the urethra and bladder

Periurethral injection of Bulkamid® for neurogenic vs. non-neurogenic female patients with stress urinary incontinence.

INTRODUCTION: Data on bulking agents for stress urinary incontinence (SUI) in neurogenic patients remain limited, especially regarding polyacrylamide hydrogel (PAHG, Bulkamid®), now commonly used. This study evaluates the efficacy, safety, and tolerability of Bulkamid® in neurogenic women with SUI. METHODS: From January 2020 to January 2025, we prospectively analyzed women undergoing periurethral Bulkamid® injections at a single center. Outcomes in neurogenic patients were compared to those without neurological disease. All procedures were performed outpatient under local anesthesia. RESULTS

A Case of Bladder Rupture Initially Misdiagnosed as Ascites Due to Alcoholic Liver Disease.

Bladder rupture is a rare condition, typically associated with trauma but occasionally occurring spontaneously. Non-traumatic bladder ruptur … vative treatment was started, but his renal function deteriorated. On the fifth day, however, after urinary catheterization, 10 L of urine were drained within six hours, resulting in a rapid decrease in serum creatinine levels. Retrospective review of imaging revealed a rupture at the bladder apex, likely caused by chronic bladder overdistension due to alcohol use and di …
