Left ventricular ejection fraction and Global Longitudinal Strain variability between methodology and experience.

INTRODUCTION: Although ejection fraction (EF) is the cornerstone of the assessment of left ventricular (LV) systolic function, its measurement faces a number of challenges related to image quality, assumptions of LV geometry, and expertise. The aim of this study was to test the inter-observer variability of EF and GLS measurement in patients with a broad spectrum of LV function, between physicians and investigators (Inv) with different levels of expertise. METHODS: In 122 patients, EF and GLS were measured by 4 Inv blinded to each other with different level of experience in echocardiography;

Effect of Intermittent High-Mechanical Index Impulses on Left Ventricular Strain.

BACKGROUND: Intermittent high-mechanical index (MI) impulses from a transthoracic ultrasound transducer are recommended for regional wall motion analysis and assessment of myocardial perfusion following intravenous administration of ultrasound enhancing agents (UEAs). High-MI impulses (>1.0) applied in this setting have also been shown to increase microvascular blood flow through a purinergic signaling pathway, but their effects on left ventricular (LV) myocardial function are unknown. Therefore, the aim of this study was to investigate the effect of transthoracic intermittent high-MI impulses

Daily longitudinal strain evaluation for left ventricular function monitoring in a 65-year-old female with reverse syndrome takotsubo and multiple triggering factors.

UNLABELLED: Among patients with takotsubo syndrome (TTS), reverse TTS (rTTS) constitutes 1-23% of all cases reported in the literature. The highest prevalence of rTTS is observed in intracranial hemorrhage, pheochromocytoma and severe infections. A CASE REPORT: The authors describe a case of a 65-year-old female with advanced multiple myeloma in whom rTTS was recognized on admission due to streptococcal sepsis. Other possible triggering factors included: anemia, blood transfusion, transient acute renal failure with electrolyte imbalance. ECG showed ST-segment depression in precordial leads an

Left ventricular hypertrophy and geometric remodeling in hypertension: stimuli, functional consequences and prognostic implications.

OBJECTIVE: To provide a coherent overview of the stimuli to development of abnormal left ventricular geometric patterns, their impact on cardiac function and their relationship to the prognosis of hypertension, studies performed by the authors and other investigators are reviewed. RESULTS: Clinical and experimental studies have shown that an elevated left ventricular mass reflects the additive effects on the heart of higher arterial pressure over time, increased cardiac volume load related to obesity, sodium intake or other stimuli, reduced contractile efficiency of the myocardium and altered

Left atrium as key player and essential biomarker in heart failure.

This article reviews roles of the left atrium as regulator of left ventricular filling, as compensatory reserve in left ventricular dysfunction and as diagnostic marker in patients with cardiovascular disorders. Application of novel imaging tools to assess left atrial function and their integration with conventional clinical methods are discussed. This includes a review of clinical applications of left atrial strain as a method to quantify the reservoir and booster pump components of left atrial function. Emerging methods for assessing left atrial wall stiffness and active work by pressure-str

Prognostic Implications of Right Ventricular Systolic Dysfunction in Cardiac Amyloidosis.

Left ventricular (LV) systolic dysfunction in cardiac amyloidosis (CA) is associated with poor prognosis. This study aimed to investigate the prognostic implications of right ventricular (RV) systolic dysfunction in CA. A total of 93 patients diagnosed with CA who underwent standard and speckle-tracking echocardiography were included. During a median follow-up of 17 (5 to 38) months, 42 patients (45%) died. Nonsurvivors were more likely to present with immunoglobulin light-chain amyloidosis and New York Heart Association class III to IV heart failure symptoms. Regarding the echocardiographic c

Value of the echocardiographic findings of arrhythmogenic right ventricular dysplasia with left ventricular involvement in a child.

A 9-year-old girl was admitted because she had had palpitations for 2 years. Her electrocardiogram revealed multifocal ectopic beats with left bundle branch morphology, sinoatrial block, and first- and second-degree atrioventricular blocks. Cross-sectional echocardiography demonstrated an enlarged left ventricle and filling defects and sacculations of the posterior left ventricular wall, apex, and septum. The diagnosis of arrhythmogenic right ventricular dysplasia (ARVD) was confirmed by endomyocardial biopsy and thallium 201 scintigraphy. We present this case because of the rarity of left ven

Impact of T2DM on right ventricular systolic dysfunction and interventricular interactions in patients with essential hypertension: evaluation using CMR tissue tracking.

BACKGROUND: Previous studies reported that there was right ventricular (RV) systolic dysfunction in patients with hypertension. The aim of this study was to evaluate the impact of type 2 diabetes mellitus (T2DM) on RV systolic dysfunction and interventricular interactions using cardiac magnetic resonance feature tracking (CMR-FT) in patients with essential hypertension. METHODS AND METHODS: Eighty-five hypertensive patients without T2DM [HTN(T2DM -)], 58 patients with T2DM [HTN(T2DM +)] and 49 normal controls were included in this study. The biventricular global radial, circumferential and lo

Diastolic Function Assessment of Left and Right Ventricles by MRI in Systemic Sclerosis Patients.

BACKGROUND: Heart involvement is frequent although often clinically silent in systemic sclerosis (SSc) patients. Early identification of cardiac involvement can be improved by noninvasive methods such as MRI, in addition to transthoracic echocardiography (TTE). PURPOSE: To assess the ability of phase-contrast (PC)-MRI to detect subclinical left (LV) and right (RV) ventricular diastolic dysfunction in SSc patients. STUDY TYPE: Prospective. POPULATION: Thirty-five consecutive SSc patients (49 ± 14 years) and 35 sex- and age-matched healthy controls (48.6 ± 13.5 years) who underwent TTE and MR

Sepsis-induced cardiomyopathy.

Sepsis remains the most common cause of death in critically ill patients worldwide and the most frequent non-cardiac disease diagnosed in the Coronary Care Unit. Myocardial dysfunction caused by sepsis, usually termed 'sepsis-induced cardiomyopathy' (SICM), is common and has long been a subject of interest for intensivists and cardiologists. Despite the lack of a consensus definition, SICM is widely recognized as a reversible condition characterized by acute single or bi-ventricular systolic and/or diastolic dysfunction unrelated to coronary artery disease. While indications for monitoring and

Left Ventricular Remodeling and Its Progression in Asymptomatic Patients with Chronic Aortic Regurgitation: Evaluation by Speckle-Tracking Echocardiography.

BACKGROUND: Progression of chronic aortic regurgitation (CAR) is insidious, and management is challenging. The primary aim of this study was to evaluate left ventricular (LV) remodeling and its progression in asymptomatic patients with CAR and preserved LV ejection fraction by three-dimensional speckle-tracking echocardiography (STE). The secondary aim was to identify the effect of management strategies on LV remodeling in severe CAR. METHODS: One hundred thirty-five patients and 41 control subjects were enrolled. Patients were divided according to regurgitation degree: mild (n = 48), moderat

Right ventricular dysplasia in an asymptomatic young man: an uncommon case with biventricular involvement and no known family history.

A 33-year-old man had cardiomegaly on a routine x-ray examination. He was asymptomatic with no history of infarction, syncope, or palpitations. There was no family history of congenital heart disease or sudden death. Two-dimensional transthoracic echocardiography demonstrated marked enlargement of the right atrium and ventricle with severely depressed right and left ventricular function that was consistent with right ventricular dysplasia. The patient was treated with an angiotensin-converting enzyme inhibitor and did well for 6 months, but then developed symptomatic left-sided congestive hear

Beyond E/e': A Physiology-Based Framework for Emerging Echocardiographic Biomarkers in the Noninvasive Assessment of Left Ventricular Filling Pressure.

OBJECTIVES: Accurate noninvasive estimation of left ventricular (LV) filling pressure is essential for diagnosing and managing heart failure and other cardiovascular diseases. Although the E/e' ratio remains the cornerstone of contemporary echocardiographic assessment, its diagnostic performance is influenced by loading conditions, myocardial disease, cardiac rhythm, and technical limitations. This review presents a physiology-based framework for emerging echocardiographic biomarkers, examining how each parameter reflects distinct pathophysiological processes-including myocardial relaxation, m

Effect of Preoperative Volume Overload on Left Ventricular Function Recovery After Ventricular Septal Defect Repair.

Significant left-to-right shunt across a ventricular septal defect (VSD) may lead to left ventricle (LV) volume overload and dilation. The acute loss of LV preload after repair of VSD may contribute to postoperative LV systolic dysfunction. The primary aim of the study is to assess the effect of presence of preoperative LV dilation on LV systolic function recovery after VSD repair. We evaluated the LV systolic function by measuring LV longitudinal strain and ejection fraction (EF) before surgery (time point 1) and at 5 time points after VSD repair (time point 2: 0 to 2 weeks, time point 3: 2 t

Reduced Myocardial Strain Parameters in Subjects With Pectus Excavatum: Impaired Myocardial Function or Methodological Limitations Due to Chest Deformity?

Pectus excavatum (PE) may cause symptoms and alter cardiopulmonary function. Left ventricular (LV) and right ventricular (RV) function have been reported to be impaired in PE subjects. However, this issue has not been systematically investigated with respect to the degree of chest wall abnormality. We aimed to evaluate the influence of severity of chest shape abnormality on myocardial strain parameters in PE subjects. We studied 30 healthy subjects (55.8 ± 14.0 year/old, 18 males) with PE, assessed by the ratio of chest transverse diameter over the distance between sternum and spine (modified
