Video-assisted sympathectomy for essential hyperhidrosis: effects on cardiopulmonary function.

BACKGROUND: Essential hyperhidrosis is characterized by overactivity of the sympathetic fibers passing through the upper-dorsal ganglia (second and third thoracic ganglia [D2-D3]), and the treatment of choice is video-assisted thoracoscopy sympathectomy. Alterations in cardiopulmonary function after treatment have been reported. STUDY OBJECTIVE: To evaluate cardiopulmonary function impairment after sympathectomy in patients with essential hyperhidrosis. DESIGN AND SETTING: Prospective controlled trial at a pulmonary function unit of a university hospital. PATIENTS: Twenty patients (2 men an

Utility of Glidescope(®) videolaryngoscopy in surgical procedures involving the larynx.

GlideScope(®) is a recently developed videolaryngoscope that helps to achieve a good view of the laryngeal inlet and the vocal cords. Videolaryngoscopy has been proven effective in patients with unusual anatomical or pathological features, suggesting the possibility of a difficult endotracheal intubation. This device may also be useful for otorhinolaryngologists by facilitating access to the larynx and tongue base, especially in selected cases, where good visualisation of disease-altered structures is vital. According to the current literature, GlideScope(®) has been used for surgical procedur

Video-assisted paraspinal approach for the stabilization of the complex spine.

Access to complex conditions of the thoracic spine and of the posterior mediastinum offers significant challenges, especially if the surgeon wishes to comply with the principles of minimal invasiveness. We report a successful vertical, paraspinal incision along with the subperiosteal removal of the overlying 2-cm rib segments combined with video assistance through single-access video-assisted thoracic surgery to perform a total T6 and partial T7 corpectomy for a metastatic multiple myeloma in a 50-year old man. This approach appears to provide excellent exposure and minimal morbidity when a re

Is minimally invasive parathyroid surgery an option for patients with gestational primary hyperparathyroidism?

BACKGROUND: Gestational primary hyperparathyroidism is associated with serious maternal and neonatal complications, which require prompt surgical treatment. Minimally invasive parathyroidectomy reduces pain, improves cosmesis and may achieve cure rates comparable to traditional open bilateral neck exploration. We report the clinical course of a woman with newly diagnosed gestational primary hyperparathyroidism and discuss the decision making behind the choice of video-assisted minimally invasive parathyroidectomy, amongst the other minimally invasive parathyroidectomy techniques available. [A new surgical technique for thoracic parathyroid glands: video-assisted thoracoscopy with intraoperative Tc-MIBI scintigraphy].

Ectopic mediastinal parathyroid glands are uncommon (1-11% of all cases of hyperparathyroidism). Median sternotomy used to be performed to resect them, with large and painful incisions that prolonged the length of hospital stay. Current imaging techniques (thoracic computed tomography and Tc-mibi scan) can accurately locate the ectopic glands, allowing minimally invasive exploration. We report a new surgical technique: video-assisted thoracoscopic resection associated with intraoperative Tc-mibi scintigraphy for ectopic mediastinal parathyroid glands. This technique allows minimally invasive a

Video-assisted medical thoracoscopy performed with a single-use bronchoscope: Case report of a critically ill patient with pleural empyema.

Current guidelines recommend VATS as the first-line treatment for stage II empyema. Patients with concomitant cardiorespiratory failure are considered ineligible for guideline-recommended management. Alternative treatment options are needed. This case report pertains to an obese man in his early 60 s with respiratory failure and decompensated heart failure. He developed an emphyema, which was refractory to the standard antibiotic therapy, thoracocentesis and other chest drainage. As patient was disqualified from surgical treatment, new approach was implemented. Minimally invasive approach of i

Uniportal subxiphoid thoracoscopic thymectomy using percutaneous suspension technique for giant anterior mediastinal mass resection: a case report.

BACKGROUND: Resection of giant anterior mediastinal masses (AMMs) has traditionally required median sternotomy as the limited operative space and visibility of conventional video-assisted thoracoscopic surgery (VATS) pose significant technical challenges. In this study, we describe a percutaneous suspension technique using a Tian-Ping sternal lifter, to facilitate uniportal VATS for the complete resection of a giant teratoma. CASE PRESENTATION: A 28-year-old Chinese female presented with a three-month history of intermittent chest pain and cough. Computed tomography (CT) revealed a 17.0 × 9.0

Pleural Infection in Adults: Integrating Medical and Surgical Management for Optimal Outcomes.

Pleural infection remains a major cause of morbidity and mortality despite advances in antimicrobial therapy, imaging, and surgical techniques. Management is frequently complicated by delayed diagnosis, heterogeneous disease progression, and uncertainty regarding optimal timing of escalation. This article integrates contemporary medical and surgical perspectives on pleural infection, emphasizing early diagnosis, structured reassessment, and timely multidisciplinary decision-making. We discuss the roles and limitations of antibiotics, chest tube drainage, intrapleural enzyme therapy, and surger

Videolaparoscopic pancreatojejunostomy using a hybrid technique in chronic pancreatitis without significant ductal dilatation.

BACKGROUND: Chronic pancreatitis without significant main duct dilation is difficult to treat, as classic drainage procedures are less effective. Endoscopic therapy is often limited by complex anatomy, multiple strictures, stones, and the need for repeated procedures, frequently providing incomplete pain relief. Surgery offers better long-term pain control, but small-duct disease requires alternative strategies. AIMS: To record the case of a 52-year-old man with alcoholic chronic pancreatitis who had progressive abdominal pain requiring opioids. Imaging showed a mildly dilated (6 mm), tortuou
