Successful replantation of the lower leg after 42-hour ischemia: case report.

A case of right lower-leg replantation after 42-hr ischemia is presented. Revascularization of the other foot with circulatory decompensation after 36-hr ischemia was carried out simultaneously. The replanted lower leg survived. Following its shortening in replantation by 12 cm, right lower-leg lengthening by 8 cm was carried out 1.5 years after replantation with the aid of a distraction apparatus. The locomotor function in both lower extremities recovered.

Synovial joint formation requires local Ext1 expression and heparan sulfate production in developing mouse embryo limbs and spine.

Heparan sulfate proteoglycans (HSPGs) regulate a number of major developmental processes, but their roles in synovial joint formation remain unknown. Here we created conditional mouse embryo mutants lacking Ext1 in developing joints by mating Ext1(f/f) and Gdf5-Cre mice. Ext1 encodes a subunit of the Ext1/Ext2 Golgi-associated protein complex responsible for heparan sulfate (HS) synthesis. The proximal limb joints did form in the Gdf5-Cre;Ext1(f/f) mutants, but contained an uneven articulating superficial zone that expressed very low lubricin levels. The underlying cartilaginous epiphysis was

Who Benefits From Allowing the Physis to Grow in Slipped Capital Femoral Epiphysis?

BACKGROUND: The globally acknowledged treatment for mild to moderate slipped capital femoral epiphysis (SCFE) is single screw in situ fixation, also used for prophylactic contralateral fixation. The Free-Gliding Screw (FG; Pega Medical) is a 2-part free-extending screw system designed to allow the growth of the proximal femur. We aimed to analyze the relationship between skeletal maturity and potential growth of the proximal physis and remodeling of the femoral neck using this implant. MATERIALS AND METHODS: Females below 12 years and males below 14 years undergoing in situ fixation for stabl

[Comparative study on the effect of microscopic revascularization and apexification in the treatment of pulp necrosis of permanent teeth].

PURPOSE: To compare the curative effect of microscopic revascularization and apexification in the treatment of pulp necrosis of permanent teeth. METHODS: Seventy-five cases of pulp necrosis in young permanent teeth were divided into two groups according to different treatment methods. Group A (n=30) underwent revascularization under microscope, while group B (n=45) underwent apexification. The treatment effect and pain improvement of the two groups were compared. The changes of the wall thickness and root canal length of the affected teeth before and after treatment were observed, and the bon

Open elbow arthrolysis for post-traumatic elbow contracture.

BACKGROUND: Post-traumatic contracture is a common complication after elbow trauma. If conservative therapy fails to restore adequate elbow motion, arthrolysis is indicated. The purposes of this study were to evaluate the clinical outcome of open arthrolysis for post-traumatic elbow contracture and to determine factors influencing the outcome. METHODS: Twenty-seven patients with post-traumatic elbow contracture were followed-up after open arthrolysis for at least 12 months. Before surgery, the mean limitation in extension was 30 and the mean maximum flexion was 83 degrees. A posterior surgica

Arteriovenous vascular loops in free flap reconstruction of the extremities.

BACKGROUND: High-energy trauma or peripheral arteriopathy may render recipient vessels unsuitable for free tissue transfer. Vein grafts can be used to lengthen the vascular pedicles to reach healthy distant vessels but with an increased complication rate. METHODS: From 1995 to 2006, 56 arteriovenous loops were constructed in the extremities of 55 patients as recipient vessels for 50 free flaps and two ectopic replantations. There were 40 lower extremity reconstructions, seven reconstructions of traumatic penile amputations, three upper extremity reconstructions, and two ectopic replantations

A novel device for resistance-free biomechanical testing of the metaphysis of long bones.

BACKGROUND: Biomechanical testing is an essential component of bone research. In order to test the metaphyseal region of long bones, a typical location for the nowadays increasing field of osteoporotic bone changes, three-point bending and breaking test devices are suitable and widely used. The aim of our study was to increase the effectiveness of this method by using a newly developed ball-mounted platform design. This new design eliminates the negative effects of friction, present in previous studies, caused by the lengthening of the distal tibia along its diaphyseal axis while sliding over

The relationship between form and function of the carnivore mandible.

Dietary morphology diversified extensively in Carnivoraformes (living Carnivora and their stem relatives) during the Cenozoic (the last 66 million years) as they evolved to capture, handle, and process new animal and plant diets. We used 3D geometric morphometrics, mechanical advantage, and finite element analysis to test the evolutionary relationship between mandibular form and biomechanical function as subclades independently made the transition from mesocarnivorous diets (50%-70% animal matter) to hypercarnivorous (>70% animal matter) and osteophagous ones (substantial bone processing). We

Comparative Histoanatomy of the Epiglottis and Pre-epiglottic Space of the Chimpanzee Larynx.

OBJECTIVES: Understanding of the evolution of the larynx clarifies the physiology and functional histoanatomy of the present-day human organ. Comparative histoanatomy of the epiglottis and pre-epiglottic space of the chimpanzee larynx (a hominid, phyletic closest relative of humans) was investigated. METHODS: Two normal adult chimpanzee larynges obtained from natural deaths were investigated. The whole organ serial section technique was employed. RESULTS: The histoanatomical structures of the chimpanzees' epiglottis and pre-epiglottic space were considerably similar to those of human adults.

Current Concepts in Radial Club Hand.

Radial club hand is a complex congenital abnormality of the radial or pre-axial border of the upper extremity. It has a wide range of phenotypes from hypoplasia of the thumb to complete absence of the radius and the first ray. Centralization with tendon transfer is a popular method for maintaining the correct position of radial club hand. On the other hand, various corrections were devised, e.g. radialization after distraction to emphasize the fact that the head of the ulna is positioned under the radial carpal bones and is no longer placed in a slot in the center of the carpus, microvascular

Anterior Decompensation Using Segmental Osteotomy for Patients With Mandibular Asymmetry.

To obtain sufficient correction of facial asymmetry by orthognathic surgery, precise and sufficient dental decompensation during preoperative orthodontic treatment is needed. Facial asymmetry often includes complicated 3-dimensional dental compensation in the anterior alveolar region, which can limit the treatment options and lengthen the treatment time. As an alternative, anterior decompensation using segmental osteotomy (ADSO) with the patient under local anesthesia could be a reasonable approach, as it represents an effective, selective, and relatively safe method for correcting lower anter

Transdiscal C6-C7 contralateral C7 nerve root transfer in the surgical repair of brachial plexus avulsion injuries.

BACKGROUND: Repair of complete brachial plexus avulsion injuries may require contralateral C7 nerve root transfer. The available techniques might allow direct neuroraphy in about 50 % of cases but the others require interposing nerve grafts or humeral shaft shortening. We aimed to see if transdiscal C6-C7 contralateral C7 nerve root transfer is technically feasible and if it allows direct coaptation with the contralateral nerve roots in 100 % of cases. METHODS: In ten fresh-frozen adult cadavers, the C7 nerve root was sectioned just before it connects with other brachial plexus branches and r

Mammals as prey: estimating ingestible size.

Most mammals have deformable bodies, making it difficult to measure the size of living or freshly killed ones accurately. Because small rodents are common prey of many snakes, and because nearly all snakes swallow their prey whole, we explored four methods for determining the ingestible size (the smallest cross-sectional area that the largest part of the rodent can be made into without breaking bones or dislocating joints) of 100 intact rodents, including 50 Musmusculus and 50 Rattus norvegicus. Cross-sectional areas derived from maximal height and width of specimens at rest or the same specim

Tooth-borne distraction of the lower anterior subapical segment for correction of class II malocclusion, subsequent to genioplasty.

INTRODUCTION: Alveolar distraction is mainly used to increase height and width of the alveolar crest. This technique, however, is not typically used for lengthening the perimeter of the dental arch or improving teeth axes. We applied alveolar distraction in a tooth-borne manner in the second stage of our original method and obtained favorable results. We therefore present an outline of this method. CASE REPORT: Genioplasty was first performed to create an infrastructure for sequential advancement of the subapical alveolar segment. After bone union, anterior subapical alveolar osteotomy was pe

Fibular collateral ligament-biceps femoris calcific bursitis causing flexion contracture in the knee, external rotation in the leg, and equinus deformity in the ankle.

OBJECTIVE: The fibular collateral ligament (FCL)-biceps femoris (BF) bursa is not a commonly known location for calcific bursitis. A case of FCL-BF calcific bursitis is presented. CLINICAL FEATURES: An 18-year-old man had a blunt trauma on the lateral side of his right knee approximately 1 year before his examination. The pain and physical distortion developed in the trauma region over time. Physical examination revealed a 30 degrees flexion contracture and slight valgus deformity in the right knee. There was 30 degrees of external rotation in the leg and a 15 degrees equinus deformity in the

Intestinal failure.

The recognition of intestinal failure (IF) as a distinct clinical entity over the past 20 years has primarily come about through the emergence of increasingly successful treatment based on use of intravenous nutrition delivered both in hospital and at home. Intestinal failure has many and varied causes, and may be complete or partial, acute and short lived, or chronic and permanent. If the latter, lifetime dependence on parenteral nutrition will usually be indicated and where possible patients are trained in the techniques of home parenteral nutrition (HPN). The success of HPN is itself primar

Class switched bovine ultralong CDR H3 amplicons versus canonical in immune and non-immune tissues.

Within the bovine immune response, a distinct type of immunoglobulin (Ig), known as ultralong complementarity-determining region 3 (CDR3) antibodies, coexists alongside canonical Igs. These ultralong heavy chains (HC) feature a remarkable structure with a lengthened CDR3 of the heavy chain (CDR H3), allowing them to adopt either a smaller and more extended paratope than canonical antibodies. However, very little is known about the distribution of ultralong CDR H3s in cattle tissues and which isotypes they are ultimately associated with. This study analyzed and quantified the amplicons of the f

Skeletal, dental and facial aesthetic changes following anterior maxillary segmental distraction by tooth-borne device in patients with cleft lip and palate.

Maxillary skeletal deficiency secondary to cleft lip and palate (CLP) remains a significant challenge. The aim of this study was to present the comprehensive skeletal, dental and facial aesthetic outcomes of anterior maxillary segmental distraction osteogenesis (AMSDO) for treatment of maxillary hypoplasia in patients with CLP. Twelve patients with maxillary hypoplasia treated with AMSDO by a customized tooth-borne distractor were included. Three-dimensional changes of anterior maxillary segment, upper incisor and pharyngeal space were measured by three-dimensional reconstruction derived from

Pediatric Pes Planus: A State-of-the-Art Review.

Flatfoot (pes planus) is common in infants and children and often resolves by adolescence. Thus, flatfoot is described as physiologic because it is usually flexible, painless, and of no functional consequence. In rare instances, flatfoot can become painful or rigid, which may be a sign of underlying foot pathology, including arthritis or tarsal coalition. Despite its prevalence, there is no standard definition for pediatric flatfoot. Furthermore, there are no large, prospective studies that compare the natural history of idiopathic, flexible flat feet throughout development in response to vari
