A new risk classification for patients undergoing cardiac surgery has been used for the last two years by the anaesthesiologists of the Montreal Heart Institute. Anaemia is one of the commonest symptoms reported to medical practitioners by their patients. A combined morphological and functional approach to classification is preferred, but the crucial point is the need to establish the cause of the symptom before selecting any form of therapy. Treatment before making an accurate diagnosis, especially the use of haematinics other than those specifically needed, may be dangerous and cannot be too strongly deprecated.

Renal injuries require immediate or early surgical intervention for threatened exsanguination, damage to the pedicle vessels and disruption of the renal pelvis. Elective delayed operation prevents late complications in those patients in whom there is transparenchymal connection between the pelvocaliceal system and the perirenal space following trauma. There appears to be an increased incidence of nephrectomy if the latter are explored early. Expectant treatment is appropriate for renal lacerations without extravasation, if there is no threat of exsanguination and if the pedicle is intact.

On the grounds of the analysis of 1300 case records and own observations with the use of angioscopy and angiography the authors have come to the conclusion that acute cholecystitis in elderly individuals results from the blood supply disorder in the gallbladder wall, caused by the thrombosis of the vascular branches of the cystic artery. They have suggested the classification of acute cholecystitis, mirroring the stage character of the disease and the nature of pathological changes in the gallbladder.

Of 185 patients with myopathy, 22 showed abnormal muscle mitochondria. In 12 of the 22 patients, all of whom had ocular myopathy or the ophthalmoplegia-plus syndrome, muscle biopsies contained 5%-25% "ragged red" fibers.
