Simultaneous fractures of the distal radius and scaphoid.

Simultaneous fractures of the distal radius and the scaphoid are uncommon. A study of seven male patients who had sustained this fracture combination revealed the most frequent cause of the injury was a fall from a height. The distal radial fracture, usually unstable and intra-articular, was the principle fracture in this complex injury. In contrast, most of the scaphoid fractures were stable. Four of the radial fractures were treated by closed reduction and cast immobilization. In the remaining three, an external fixator was used to maintain the reduction. Four scaphoid fractures were immobil

Feasibility of 4-point fixation using the preauricular approach in a zygomaticomaxillary complex fracture.

OBJECTIVE: A zygomaticomaxillary complex fracture is a facial bone fracture that commonly occurs as a centrally and laterally protruding zygomatic area. The exact reduction of the displaced fracture is the most important task in the treatment of a zygomatic fracture, from the aesthetic point of view. In some types of zygomaticomaxillary complex fracture, however, it is somewhat difficult to maintain the reduction after the surgery using 3-point fixation. In addition, surgery using 3-point fixation may cause malunion or nonunion. Thus, 4-point fixation using the coronal approach is alternativel

The Pediatric Open Supracondylar Fracture: Associated Injuries and Surgical Management.

BACKGROUND: While supracondylar (SC) fractures are relatively common in children, the incidence of open injuries is believed to be only 1%. Two prior studies on open SC fractures in children reported an increased incidence of vascular injuries. The purposes of our study were to clarify the incidence, associated conditions, and current treatment for open SC fractures. METHODS: The Pennsylvania Trauma Outcome Study database was queried. Subjects age 25 to 156 months old admitted to trauma centers between January 2000 and December 2015 with a SC fracture were included. Controls were those with c

Primary dynamic interlocking nail in femoral shaft fracture: A case series.

INTRODUCTION AND IMPORTANCE: Femoral shaft fracture is one of the most frequent injuries encountered by an orthopedic surgeon. Surgical treatment is commonly needed. Intramedullary nailing remains the gold-standard in surgical treatment of femoral shaft fracture. One of the constant dilemmas in intramedullary nailing is whether to use a static or dynamic locking screw for treating femoral shaft fractures. CASE PRESENTATION: We reported three cases of simple femoral shaft fracture and surgically fixed with primary dynamic interlocking nail. Closed reduction with reamed nail was performed in 2

[Percutaneous pinning for proximal humerus fractures].

Proximal humerus fractures are relatively frequent. The are several possibilities for fixation of proximal humerus fractures: close reduction and fixation with percutaneous pinning or intramedullary rod, open reduction and fixation with tension band or a plate. Close reduction and percutaneous pinning have the advantage to be an easy technique, with good results and it is considered to be ideal in young patients with two-part fractures. The number and the directions of the pins depend upon the number of fragments displaced, reducibility, bone quality, patient age, other pathology associated. S

Optimal timing of femur fracture stabilization in polytrauma patients: A practice management guideline from the Eastern Association for the Surgery of Trauma.

BACKGROUND: Femur fractures are common among trauma patients and are typically seen in patients with multiple injuries resulting from high-energy mechanisms. Internal fixation with intramedullary nailing is the ideal method of treatment; however, there is no consensus regarding the optimal timing for internal fixation. We critically evaluated the literature regarding the benefit of early (<24 hours) versus late (>24 hours) open reduction and internal fixation of open or closed femur fractures on mortality, infection, and venous thromboembolism (VTE) in trauma patients. METHODS: A subcommittee

Nonunion of forearm shaft fractures in children after intramedullary nailing.

The elastic stable intramedullary nailing (ESIN) is the current treatment of choice for unstable forearm shaft fractures in children. There is no large study on paediatric nonunion of forearm shaft fractures in children after intramedullary nailing. There are only sporadic reports on nonunions after ESIN in children. The aim of this study was to define predisposing factors of nonunions in paediatric forearm fractures. All children who had been treated for forearm fractures by ESIN in our hospital from 1990 to 2006, and all children treated elsewhere surgically and being followed up at our inst

Comparison of 2 Types of Local Anesthetic Techniques in the Reduction of Distal Radius Fracture: A Prospective Cohort Study.

BACKGROUND: The treatment of distal radius fractures may require manipulation of the fracture assisted by finger traction, causing pain both at the fracture site and at the fingers. The usual type of anesthesia used does not anesthetize the fingers. METHODS: We conducted a prospective cohort study with two groups, hematoma block (HB) and hematoma with associated median nerve block (MHB). Characteristic variables of the patients were collected. The main variable for the analysis was pain, measured using the Visual Analogical Scale (VAS). It was measured prior to the injection (VAS1), during fr

Supra-Temporalis Approach for Treating Zygomaticomaxillary Complex Fracture.

PURPOSE: There are multiple approaches described to access the zygomaticomaxillary complex (ZMC) fractures: lateral eyebrow, upper blepharoplasty, coronal, subciliary, subtarsal, infraorbital, transconjunctival, and transoral. All these approaches have their advantages, disadvantages, and indications according to location of fracture, degree of displacement, and surgeon's experience with a specific technique. However, there is not a good approach for treating the zygomatic arch or body fracture. In this paper, the authors described a supra-temporalis approach to treat the zygomatic arch or bod

Conversion of Neer Type II Closed Distal Clavicle Fracture to an Open Fracture Following Surgery Delay: A Case Report.

INTRODUCTION: Type II distal clavicle fractures are associated with significant displacement and high rates of nonunion. Due to the risk of nonunion, these fractures are most often managed operatively soon after the injury. CASE REPORT: We present an 84-year-old male who sustained a Type II closed distal clavicle fracture that converted to an open fracture 1 month following initial injury. The patient underwent surgical repair with open reduction internal fixation and coracoclavicular ligament reconstruction. Following surgery, the patient experienced hardware failure with nonunion. He did no

[Intramedullary nailing of unstable proximal humeral fractures].

Unstable subcapital fractures and dislocation fractures of the humerus can usually be set by closed reduction. Stabilization of these fractures by intramedullary pins is a method which avoids damage to the soft tissues around the shoulder. Since 1985, elastic steel pins with special features have been in use. The last 20 mm of the tip of the pins is angulated at 15 degrees, and the distal end is curved in a special way for the last 20 mm. According to our experience, the results after using this method in the treatment of unstable fractures of the surgical neck (ASIF type A II/2) and dislocati

Deep Bone Abscess After Pin Tract Infection of an Operatively Treated Supracondylar Humeral Fracture.

Deep bone abscesses are a rare complication following a pin tract infection. This article shows the importance of early detection, surgical management, and microbiology-guided antibiotic therapy to treat the abscess. We aim to highlight the key presenting factors, the radiological signs, and intraoperative features of a deep bone abscess, to facilitate detection in these rare circumstances. Closed or open reduction and internal fixation with Kirschner wires (K-wires) has become the standard of care for displaced supracondylar fractures. Although pin tract infection (PTI) is the most common com

Extension block and direct pinning methods for mallet fracture: A comparative study.

BACKGROUND: Mallet fracture can easily occur during sports activities or in daily life; however, the principles and methods of treatment for such fractures remain debated. Among the surgical treatments, various methods of closed reduction have been proposed. We treated patients with the extension block method (EBM) and the direct pinning method (DPM), and then compared the results. We assessed differences in range of motion and measurements of finger movement after surgery. METHODS: A total of 41 patients who underwent surgery from August 2013 to September 2015 were evaluated retrospectively.

Outcomes of Open Reduction and Internal Fixation in Intra-articular Distal Femur Fractures with Incarcerated Patella: A Case Series of a Rare Injury Pattern.

INTRODUCTION: Distal femur fractures associated with patellar incarceration are rare, complex injuries that pose significant diagnostic and surgical challenges. The patella, an essential component of the knee extensor mechanism, may become entrapped within distal femoral fracture fragments following high-energy trauma, creating a mechanical block to reduction. Failure to recognize this uncommon injury pattern can result in repeated unsuccessful attempts at closed reduction, leading to chondral injury, knee stiffness, and suboptimal functional outcomes. Early diagnosis using computed tomography

Treatment strategies of distal radius fractures.

There has been a surge in the operative management of distal radius fractures. Closed reduction, external fixation, and open reduction with internal fixation each have advantages and disadvantages. The purpose of this review is not to provide the clinician with an algorithm for treatment of distal radius fractures. These fractures span an extensive spectrum of severity across age groups and demographics. Fortunately, the surgeon holds a vast array of options to provide care for patients with distal radius fractures. The choice of fixation or conservative care resides in the personality of the

[Fractures of the condylar process].

Fractures of the mandibular condyle are frequently seen. There is no consensus on the most appropriate treatment modality. Closed reduction with help of arch bars and guiding elastics is in most cases sufficient. Open reduction and fixation with a plate osteosynthesis or a specially designed lag screw is occasionally performed. Intracapsular fractures of the condylar head are only operated in case of severely reduced mobility, or ankylosis. Fractures of the mandibular condyle normally heal uneventfully. Complications mostly occur in bilateral condylar neck fractures, or intracapsular fractures

Teriparatide as nonoperative treatment for femoral shaft atrophic nonunion: A case report.

BACKGROUND: Femoral shaft fractures are a common type of fracture among adults and have high union rates. However, clinical decisions are difficult to make because of the different types of nonunions. Atrophic nonunion usually requires revision surgery combined with bone grafting. Furthermore, no study of teriparatide administration for femoral atrophic nonunion have been previously reported. CASE SUMMARY: A 60-year-old woman had a right femoral shaft fracture due to a traffic accident, and she immediately underwent closed reduction and internal fixation surgery with an intramedullary nailing

Development of an Effective Treatment Algorithm for the Stubbed Great Toe.

While toe fractures are the most common fractures of the foot in the pediatric population, the majority of these injuries do not require aggressive treatment. However, the mechanism of stubbing the great toe with bleeding at the base of the nail, a laceration proximal to the nail fold, or signs of a displaced fracture through the physis of the distal phalanx signal a likely open fracture involving the distal phalanx of the great toe. Unrecognized open fractures can lead to mistreatment and consequently osteomyelitis or growth disturbances. We report a case of a patient who required open reduct
