Eccentric cycling is superior to standard rehabilitation for Post-ICU recovery in COVID-19 survivors.

BACKGROUND: The COVID-19 pandemic left numerous patients with post-intensive care syndrome (PICS) that resulted in prolonged physical and health impairment. Compared to standard rehabilitation (STD REHAB), eccentric cycling (ECC) has lower cardiopulmonary demands while inducing greater functional performance improvements after training, which could be ideal for individuals with PICS. AIM: To compare the effects of eight weeks of ECC versus STD REHAB on functional performance and quality of life of patients after hospitalization. METHODS: Randomised crossover design study (clinicaltrial.gov:

Regulation of acute stress experience through functional near-infrared spectroscopy neurofeedback of the prefrontal cortex.

BACKGROUND: Acute stress plays a key role in mental health, and there is a pressing need for accessible, brain-based interventions that strengthen stress regulation by directly targeting the underlying brain mechanisms. We therefore developed a real-time functional near-infrared spectroscopy (fNIRS)-informed neurofeedback training targeting a prefrontal region involved in cognitive emotion regulation (left lateral prefrontal cortex, PFC) and evaluated its potential to enhance stress regulation using the socially evaluated cold pressor test, an acute stress paradigm that combines physical and s

Performance in the Duke-Elder ophthalmology undergraduate prize examination and future careers in ophthalmology.

AIMS: Cognitive factors (eg, academic achievement) have had a significant role in selecting postgraduate surgical trainees in the past. This project sought to determine the role of a national undergraduate ophthalmology prize examination (Duke-Elder examination) in the selection of postgraduate ophthalmology trainees. This would also serve as a quality assurance exercise for the assessment, in which the ultimate aim is to encourage trainees into ophthalmology. METHODS: A retrospective analysis of the top 20 ranked candidates in the Duke-Elder examination from 1989 to 2005 (except 1995) was ca

Auditory training improves neural timing in the human brainstem.

The auditory brainstem response reflects neural encoding of the acoustic characteristic of a speech syllable with remarkable precision. Some children with learning impairments demonstrate abnormalities in this preconscious measure of neural encoding especially in background noise. This study investigated whether auditory training targeted to remediate perceptually-based learning problems would alter the neural brainstem encoding of the acoustic sound structure of speech in such children. Nine subjects, clinically diagnosed with a language-based learning problem (e.g., dyslexia), worked with au

Brain functional connectivity after Stroop task induced cognitive fatigue.

We aim to investigate the changes in brain functional connectivity induced by cognitive fatigue from a whole-brain perspective and multiple angles to uncover the underlying mechanisms in healthy individuals, thereby enhancing high-quality cognitive activities and the effectiveness of cognitive rehabilitation training. This study involved 48 healthy adults aged 20-35 and was conducted in two phases: cognitive fatigue validation and whole-brain connectivity feature analysis. We used fMRI to compare Amplitude of Low-Frequency Fluctuations (ALFF), Regional Homogeneity (ReHo), Functional Connectivi

Profiles of Extra-Task Workload Among Clinical Nurses and Associations With Narrative Cognitive Ability: A Latent Profile Analysis.

BACKGROUND: In addition to providing direct care, clinical nurses often undertake teaching, research, quality improvement and care coordination responsibilities. However, whether these extra-task demands form distinct workload patterns, and how such patterns relate to narrative cognitive ability, remains unclear. AIM: To identify distinct extra-task workload patterns among clinical nurses and compare narrative cognitive ability across the identified profiles. METHODS: A cross-sectional survey was conducted among 1081 clinical nurses at a tertiary general hospital in Henan Province, China, be

The Use of an Errorless Learning Application to Support Re-Learning of (Instrumental) Activities for People Living with Korsakoff Syndrome.

Korsakoff syndrome (KS) is a severe neuropsychiatric syndrome derived from acute thiamine deficiency and concomitant alcohol use disorders. KS patients need lifelong assistance because of the severity of their cognitive problems. In clinical practice and research, errorless learning has proven to be an effective cognitive rehabilitation method for patients with KS. Our study focused on optimizing errorless learning by introducing new software technology to support the training process of errorless learning. Although the benefits of errorless learning for patients with Korsakoff's syndrome have

Augmenting one-session treatment of children's specific phobias with attention training to positive stimuli.

This study examined the efficacy of combining two promising approaches to treating children's specific phobias, namely attention training and one 3-h session of exposure therapy ('one-session treatment', OST). Attention training towards positive stimuli (ATP) and OST (ATP+OST) was expected to have more positive effects on implicit and explicit cognitive mechanisms and clinical outcome measures than an attention training control (ATC) condition plus OST (ATC+OST). Thirty-seven children (6-17 years) with a specific phobia were randomly assigned to ATP+OST or ATC+OST. In ATP+OST, children complet

ADAS-Cog Trajectories Differ from Expected Decline in Dementia Following Repeated Non-Invasive Interventions over 3 Years.

Background and Objectives: Non-pharmaceutical interventions such as cognitive training, transcranial electrical stimulation (tES), and repetitive transcranial magnetic stimulation (rTMS) have shown promise in improving cognitive outcomes in Alzheimer's disease (AD) and dementia. However, the long-term effects of repeated non-invasive interventions remain unknown. This study investigated whether repeated non-invasive interventions administered over a span of 1 to 3 years were associated with slower cognitive decline compared to typical AD progression, and whether longer no-treatment intervals b…

Psychosocial therapy for posttraumatic stress disorder.

Immediately after experiencing a traumatic event, many people have symptoms of posttraumatic stress disorder (PTSD). If trauma victims restrict their routine and systematically avoid reminders of the incident, symptoms of PTSD are more likely to become chronic. Several clinical studies have shown that programs of cognitive-behavioral therapy (CBT) can be effective in the management of patients with PTSD. Prolonged exposure (PE) therapy-a specific form of exposure therapy-can provide benefits, as can stress inoculation training (SIT) and cognitive therapy (CT). PE is not enhanced by the additio

Telehealth delivery of group-format cognitive rehabilitation to older veterans with TBI: a mixed-methods pilot study.

Traumatic brain injury (TBI) is common among Veterans and may interact with aging, increasing risk for negative cognitive, emotional, and functional outcomes. However, no accessible (i.e., in-home) group interventions for TBI targeted to older adults exist. Goal Oriented Attentional Self-Regulation (GOALS) is a manualized, group cognitive rehabilitation training that improves executive function and emotional regulation among Veterans with TBI and healthy older adults. Our objectives were to adapt GOALS for delivery to older Veterans via in-home video telehealth (IVT) and evaluate feasibility a

The impact of modifying obsessive-compulsive beliefs about perfectionism.

BACKGROUND AND OBJECTIVES: Cognitive-behavioural models of obsessive-compulsive disorder (OCD) suggest that maladaptive beliefs about perfectionism play a key role in the development and maintenance of OCD. Cognitive-bias modification for interpretation bias (CBM-I) is an experimental procedure that can test this proposed causal relation. METHODS: As such, the current study investigated whether multiple CBM-I sessions administered in different contexts can modify perfectionism biases. Undergraduate students high in OCD-related perfectionism beliefs were randomly allocated to either an experim

Inhibitory control training and transcranial direct current stimulation of the pre-supplementary motor area: behavioral gains and transfer effects.

Inhibitory control is a critical part of executive function and an important cognitive process in daily life. It is currently unclear how to optimally improve inhibitory control ability through behavior training and other interventions. Here, we explored the factors that influence inhibition control training in two experiments, focusing on the gains and transfer effects of training. Experiments 1 and 2 investigated the effects of anodal transcranial direct current stimulation (tDCS) over the pre-supplementary motor area and an increase in training duration on the training effect for inhibitory
