The use of a three-dimensional imaging model to optimize the operative separation of pygopagus conjoined twins.

INTRODUCTION: The surgical management of conjoined twins has evolved to the point that elective operative separation can safely be performed in the majority. Cross-sectional imaging is invaluable to accurately delineate the site of union and configuration of shared organ systems prior to surgery. The use of three-dimensional modeling also dramatically improves the planning process and assists in intraoperative decision-making. We present a case of pygopagus conjoined twins who underwent successful operative separation at 19 months of age with the aid of such technology. METHODS: A three-dimen

Hopf Bifurcation in an Incommensurate Caputo Fractional-Order Computer Virus Epidemic Model with Multiple Time Delays.

Complex nonlinear dynamical systems, often associated with high-entropy time series, have been widely employed to describe and predict intricate dynamic phenomena in real-world systems. Motivated by the need to better understand such complex dynamics in network-based epidemic processes, this paper investigates bifurcation dynamics in a fractional-order extension of the classical Susceptible-Latent-Breaking-Out model for computer virus propagation. The proposed framework incorporates two distinct transmission-related time delays and employs Caputo fractional derivatives of incommensurate orders

The Case for Local AI Development: Lessons From Computer‑Aided Detection of Tuberculosis and Silicosis in Southern Africa's Ex‑Miners.

The co‑epidemic of silicosis and tuberculosis (TB) in South Africa's mining industry affects a large number of migrant workers and is compounded by limited access to chest X‑ray (CXR) screening. Although artificial intelligence (AI)‑based computer‑aided detection (CAD) systems for TB have demonstrated impressive accuracy against microbiological standards, validation among silica‑exposed populations has been limited. Moreover, well‑documented biases hinder CAD utility in diverse patient populations, potentially exacerbating existing healthcare inequities. In this article, we describe the challe

Automatic video analysis and classification of sleep-related hypermotor seizures and disorders of arousal.

OBJECTIVE: Sleep-related hypermotor epilepsy (SHE) is a focal epilepsy with seizures occurring mostly during sleep. SHE seizures present different motor characteristics ranging from dystonic posturing to hyperkinetic motor patterns, sometimes associated with affective symptoms and complex behaviors. Disorders of arousal (DOA) are sleep disorders with paroxysmal episodes that may present analogies with SHE seizures. Accurate interpretation of the different SHE patterns and their differentiation from DOA manifestations can be difficult and expensive, and can require highly skilled personnel not

The Performance of Computer-Aided Detection Digital Chest X-ray Reading Technologies for Triage of Active Tuberculosis Among Persons With a History of Previous Tuberculosis.

BACKGROUND: Digital chest X-ray (dCXR) computer-aided detection (CAD) technology uses lung shape and texture analysis to determine the probability of tuberculosis (TB). However, many patients with previously treated TB have sequelae, which also distort lung shape and texture. We evaluated the diagnostic performance of 2 CAD systems for triage of active TB in patients with previously treated TB. METHODS: We conducted a retrospective analysis of data from a cross-sectional active TB case finding study. Participants ≥15 years, with ≥1 current TB symptom and complete data on history of previous T

Monitoring polio supplementary immunization activities using an automated short text messaging system in Karachi, Pakistan.

PROBLEM: Polio remains endemic in many areas of Pakistan, including large urban centres such as Karachi. APPROACH: During each of seven supplementary immunization activities against polio in Karachi, mobile phone numbers of the caregivers of a random sample of eligible children were obtained. A computer-based system was developed to send two questions--as short message service (SMS) texts--automatically to each number after the immunization activity: "Did the vaccinator visit your house?" and "Did the enrolled child in your household receive oral polio vaccine?" Persistent non-responders were

Development and evaluation of detection systems for staphylococcal exfoliative toxin A responsible for scalded-skin syndrome.

Staphylococcal scalded-skin syndrome is usually diagnosed clinically by its characteristic exfoliating rash. Isolation of Staphylococcus aureus from the patient further supports the diagnosis. Several detection systems have been developed to determine whether the isolated strain produces exfoliative toxin, but none are routinely available in hospital laboratories. In a novel approach, we used computer models to predict the structure of the exfoliative toxins based on other serine proteases and to identify surface epitopes for the production of antibodies that specifically bound the exfoliative
