Chronic low back pain secondary to intervertebral disc degeneration (IVDD) is the leading cause of disability across all musculoskeletal disorders worldwide. For decades, gut-disc crosstalk research has been framed almost exclusively by a systemic inflammatory paradigm, in which circulating pro-inflammatory cytokines (e.g., TNF-α, IL-1β) act as obligate mediators linking gut microbial dysbiosis to intervertebral disc degeneration. However, multi-cohort two-sample Mendelian randomization (MR) analyses have not identified significant mediation by the circulating inflammatory factors tested to da

Brain-derived neurotrophic factor (BDNF) is an essential mediator of exercise-induced neuroplasticity. The majority of BDNF in circulation (∼99%) is stored bound to platelets. Platelet activation by exercise stimulates the release of their contents, representing a dynamic, bioavailable pool of BDNF. Hypoxia also independently activates platelets; therefore the combination of these stressors provides a unique model to examine the role of platelets in liberating BDNF and to quantify its flux across the brain during exercise. Twelve healthy adults (six women; 28 ± 4 years) performed exercise to e

BACKGROUND: Hyperphosphatemia confers adverse cardiovascular outcomes, and commonly occurs in late-stage CKD. Fibroblast growth factor 7 (FGF7) is a phosphaturic peptide which decreases renal phosphate transport in vitro and in vivo. Serum FGF7 concentrations are reduced in hyperphosphatemic patients with hypophosphatasia and are elevated in some hypophosphatemic patients with tumor-induced osteomalacia. No data, however, are available on whether circulating FGF7 concentrations increase to compensate for phosphate retention in CKD patients.
METHODS: This was a cross-sectional study performed

BACKGROUND: Glaucoma is the leading cause of irreversible blindness worldwide, causing continuous and progressively worsening damage to visual function, which leads to vision loss. Optic nerve protection is an important treatment for glaucoma with controlled intraocular pressure (GPCI), but to date, there is no universally accepted effective optic nerve protection agent. Acupuncture can protect the optic nerve by increasing blood flow to the eye. However, fear of pain or the limitations of treatment place and time lead to poor patient compliance. Press-needle therapy is a characteristic of tra

BACKGROUND: Coronary artery calcification (CAC) is a common cardiovascular complication in patients with chronic kidney disease (CKD). Fibroblast growth factor 23 (FGF23), a key regulator of phosphate metabolism, has been implicated in vascular calcification, but studies evaluating its association with CAC in CKD have yielded inconsistent findings. This meta-analysis aimed to evaluate the association between circulating FGF23 levels and CAC in patients with CKD.
METHODS: PubMed, Embase, Web of Science, Wanfang, and CNKI were systematically searched for observational studies evaluating the ass

OBJECTIVE: To explore whether circulating plasma proteins have potential causal effects on gout risk by integrating genome-wide association study (GWAS) data with protein quantitative trait loci (pQTL) information, aiming to identify candidate proteins and pathways for therapeutic targeting.
INTRODUCTION: Despite advances in gout pathophysiology, personalized therapies and validated drug targets remain limited. We conducted an exploratory Mendelian randomization (MR) analysis to assess whether circulating plasma proteins show evidence consistent with potential causal effects on gout risk, int

BACKGROUND: Childhood obesity is increasingly recognized as a trigger for early cardiovascular risk, partly mediated by subclinical inflammation and endothelial dysfunction. This study aimed to evaluate the expression of pro-inflammatory cytokines (TNF-α, IL-17A, and IL-2) in whole blood and investigate their associations with anthropometric measures, blood pressure, microvascular endothelial function, and circulating apoptotic endothelial microparticles (EMPs) in children with healthy weight versus those with overweight/obesity.
METHODS: A cross-sectional study was conducted with 113 childre

INTRODUCTION: Uterine artery embolization (UAE) has been gaining increasing popularity as an effective and minimally invasive treatment for uterine fibroids. However, there has been growing concern over the risk of unintended embolization of the utero-ovarian circulation, leading to reduction of ovarian blood supply with subsequent impairment of ovarian reserve. The purpose of this study was to investigate the impact of UAE on circulating anti-Müllerian hormone (AMH) and other markers of ovarian reserve.

BACKGROUND: Besides its capacity to regulate organ and tissue growth, the insulin-like growth factor I exerts biologic actions that resemble those of insulin. Tissue access of the factor depends on the distribution of the circulating bound factor between its binding protein 3 that remains within the intravascular space and its binding protein 1 that is able to cross the endothelium.
OBJECTIVE: To investigate whether the distribution of the circulating factor between its binding proteins is altered in patients with essential hypertension and whether this is related to changes in organ damage a

Periodontal disease (PD) and coronary artery disease (CAD) are two highly prevalent chronic systemic disorders with overlapping population risk profiles. Accumulating observational, cellular, and genetic studies have suggested potential positive correlations between PD severity, periodontal pathogen load, and CAD incidence, coronary lesion severity, and major adverse cardiac events, whereas Mendelian randomization analyses may not support a direct causal link between the two illnesses. Several synergistic pathogenic pathways may mediate their association: periodontal pathogens may translocate

B. parapertussis is one of the etiologic agents of whooping cough and its incidence has increased following the COVID-19 pandemic. Since current pertussis vaccines fail to induce protection against B. parapertussis, the course of infection critically relies on the innate immune response. However, in the absence of opsonic antibodies, B. parapertussis fails to activate resting neutrophils, thereby preventing bactericidal responses. We here investigated whether neutrophil priming with gamma-interferon (IFN-γ), the main proinflammatory cytokine present during infection, changes this outcome. We o

Breast milk derived exosomes (MDEs) are small extracellular vesicles which have been capturing attention due to their role in fetal-maternal communication, mostly for their beneficial effects related to neurodevelopment during the infant's early postnatal life. Ongoing studies highlight how environmental factors, maternal nutrition and lifestyle, affect the composition of MDEs (signaling molecules, immune factors, essential nutrients, etc.), which contribute to infant immune system maturation, gastrointestinal function and brain development. Scientific evidence indicates that milk-derived exos

Avobenzone (AVO) and ethylhexyl salicylate (EHS) are widely used organic UV filters with distinct photochemical properties and reported biological effects. Experimental and predictive evidence suggests that some lipophilic UV filters may reach systemic circulation and potentially cross the blood-brain barrier (BBB), raising concerns about possible central nervous system effects, although direct evidence for AVO and EHS remains limited. This study evaluated the effects of subcytotoxic concentrations (0.01-1 µM) of AVO and EHS on differentiated SH-SY5Y human neuroblastoma cells, focusing on earl

Parkinson's disease (PD) is characterized by progressive degeneration of dopaminergic neurons in the inflamed substantia nigra, resulting in striatal dopamine (DA) depletion and severe motor dysfunction. Here, we report a chemically programmed, inflammation-responsive prodrug nanoparticle (NP) system-β-glucans-DA(OAc)2 NPs-that enables precise oral gut-to-brain delivery of DA into deep brain regions. In this design, the acetylated DA prodrug DA(OAc)2 is conjugated to β-glucans via a reactive oxygen species (ROS)-cleavable thioketal linker, thereby preventing DA autooxidation, enhancing stabili

Portal hypertension leads to the development of a complex and heterogeneous network of portosystemic collateral pathways, which represent adaptive responses to increased portal pressure. Although traditionally considered compensatory mechanisms, growing evidence indicates that Spontaneous Portosystemic Shunts (SPSS) are ineffective in normalizing portal pressure and may contribute to adverse clinical outcomes. In patients with cirrhosis, particularly those with preserved liver function, the presence and extent of SPSS are independently associated with hepatic encephalopathy, variceal bleeding,

Alzheimer's disease (AD) is a progressive neurodegenerative disorder characterized by cognitive decline and significant neuropathological changes. Early and accurate diagnosis remains a major challenge, highlighting the need for reliable, minimally invasive biomarkers. MicroRNAs (miRNAs), small non-coding RNAs that regulate gene expression, have emerged as promising candidates. Their expression is altered in the brains of AD patients, reflecting disease-specific pathological processes, and they are detectable in peripheral biofluids. However, discrepancies in miRNA profiles between the brain a
