Evaluation of the drug-drug interaction between low-dose methotrexate and short-term use of non-steroidal anti-inflammatory drugs.

INTRODUCTION: There is ongoing debate about the clinical significance of the Drug-Drug Interaction (DDI) between low-dose methotrexate (LD-MTX) and the short-term use of non-steroidal anti-inflammatory drugs (NSAIDs), and clear guidance for DDI management is lacking. This review addresses this gap by providing a comprehensive overview of the current data on the DDI between LD-MTX and NSAIDs, to offer guidance on DDI management during short-term analgesic use. AREAS COVERED: To explore the pharmacokinetic and pharmacodynamic aspects of the DDI between LD-MTX (≤30 mg weekly) and NSAIDs for shor

Pharmacokinetic drug-drug interaction between first-line anti-TB and anti-diabetic drugs in patients with tuberculosis and diabetes mellitus.

BACKGROUND: The association between Tuberculosis (TB) and Diabetes Mellitus (DM) have always been an area of interest for decades. Many studies have reported the impact of DM on the development and prognosis of TB and vice versa. There is a major research gap in the area of drug-drug interactions between anti-TB drugs and anti-diabetic drugs. Hence, this study investigated the drug-drug interactions between first-line anti-TB and anti-diabetic drugs in patients with TB and DM. METHODOLOGY: A prospective study was undertaken in three groups of adult patients, namely, TB, DM and DMTB from May 2…

Analysis of Drug-Drug Interaction Labeling Language and Clinical Recommendations for Newly Approved Drugs Evaluated With Digoxin, Midazolam, and S-Warfarin.

PURPOSE: To best promote drug tolerability and efficacy in the clinic, data from drug-drug interaction (DDI) evaluations and subsequent translation of the results to DDI prevention and/or management strategies must be incorporated into the US Food and Drug Administration (FDA) product labeling in a consistent manner because differences in language might result in varied interpretations. This analysis aimed to assess the consistency in DDI labeling language in New Drug Applications (NDAs). METHODS: NDAs of recently approved drugs (2012-2020) that increase the exposure of digoxin, midazolam, an

Prediction and in vitro evaluation of selected protease inhibitor antiviral drugs as inhibitors of carboxylesterase 1: a potential source of drug-drug interactions.

PURPOSE: To predict and determine whether the protease inhibitors (PIs) nelfinavir, amprenavir, atazanavir, ritonavir, and saquinavir could serve as metabolic inhibitors of the human CES1 (hCES1) using both molecular modeling techniques and in vitro inhibition assays. METHODS: Initially, a molecular modeling approach was utilized to predict whether the selected PIs could serve as hCES1 inhibitors. The inhibitory effects of these PIs on hCES1 activity were then further evaluated utilizing previously established in vitro assay. RESULTS: Pharmacophore and 2D-QSAR modeling predicted that nelfina

Effect of cationic drugs on the transporting activity of human and rat OCT/Oct 1-3 in vitro and implications for drug-drug interactions.

The inhibitory effects of cationic drugs (beta-adrenoreceptor antagonists, calcium (Ca)-channel blocker, I(f) channel inhibitor, antiarrhythmic drugs, and antibacterial drugs) that inhibit 1-methyl-4-phenylpyridinium (MPP) and/or metformin uptake into hOCT1-3/rOct1-3-expressing cells and human/rat hepatocytes were investigated in this study. The drug-drug interaction (DDI) potential of these drugs for the hOCT/rOct-mediated hepatic/renal uptake process was also assessed. The IC(50) values of cardiovascular drugs, including an I(f) channel inhibitor with a new mechanism of action, were greater

SOC-DGL: Social Interaction Behavior Inspired Dual Graph Learning Framework for Drug-Target Interaction Identification.

The identification of drug-target interactions (DTI) is critical for drug discovery and repositioning, as it reveals potential therapeutic uses of existing drugs, accelerating development and reducing costs. However, most existing models focus only on direct similarity in homogeneous graphs, failing to exploit the rich similarity in heterogeneous graphs. To address this gap, inspired by real-world social interaction behaviors, we propose SOC-DGL, which comprises two specialized modules: the Affinity-Driven Graph Learning (ADGL) module, learning global similarity through an affinity-enhanced dr

Apigenin, a novel candidate involving herb-drug interaction (HDI), interacts with organic anion transporter 1 (OAT1).

BACKGROUND: Apigenin is a flavonoid compound, widely distributed in natural plants. Various studies have suggested that apigenin has inhibitory effects towards several drug transporters, such as the organic anion transporting (OAT) polypeptides, 1B1 and 1B3 (OATP1B1 and OATP1B3). However, the mechanism by which apigenin interacts with OAT1 has not been well studied. METHODS: MDCK cells stably-expressing OAT1 were used to examine the inhibitory effects of apigenin on OAT1. UPLC-MS/MS was used to evaluate the in vitro and in vivo effects of apigenin on the uptake of acyclovir by OAT1. Cytotoxic

N(1)-methylnicotinamide as an endogenous probe for drug interactions by renal cation transporters: studies on the metformin-trimethoprim interaction.

PURPOSE: N(1)-methylnicotinamide (NMN) was proposed as an in vivo probe for drug interactions involving renal cation transporters, which, for example, transport the oral antidiabetic drug metformin, based on a study with the inhibitor pyrimethamine. The role of NMN for predicting other interactions with involvement of renal cation transporters (organic cation transporter 2, OCT2; multidrug and toxin extrusion proteins 1 and 2-K, MATE1 and MATE2-K) is unclear. METHODS: We determined inhibition of metformin or NMN transport by trimethoprim using cell lines expressing OCT2, MATE1, or MATE2-K. Mo…

Pharmacokinetic interactions between tacrolimus and Wuzhi capsule in liver transplant recipients: Genetic polymorphisms affect the drug interaction.

Wuzhi capsule (WZC), a commonly used Chinese patent medicine to treat various types of liver dysfunction in China, increases the exposure of tacrolimus (TAC) in liver transplant recipients. However, this interaction has inter-individual variability, and the underlying mechanism remains unclear. Current research indicates that CYP3A4/5 and drug transporters influence the disposal of both drugs. This study aims to evaluate the association between TAC dose-adjusted trough concentration (C/D) and specific genetic polymorphisms of CYP3A4/5, drug transporters and pregnane x receptor (PXR), and plasm

Management of COVID-19 in patients with seizures: Mechanisms of action of potential COVID-19 drug treatments and consideration for potential drug-drug interactions with anti-seizure medications.

In regard to the global pandemic of COVID-19, it seems that persons with epilepsy (PWE) are not more vulnerable to get infected by SARS-CoV-2, nor are they more susceptible to a critical course of the disease. However, management of acute seizures in patients with COVID-19 as well as management of PWE and COVID-19 needs to consider potential drug-drug interactions between antiseizure drugs and candidate drugs currently assessed as therapeutic options for COVID-19. Repurposing of several licensed and investigational drugs is discussed for therapeutic management of COVID-19. While for none of th…

SSE-DDI: Selective Substructure Encoding with Bond-Centered Molecular Representations for Drug-Drug Interaction Prediction.

Drug-drug interactions (DDIs) can alter therapeutic efficacy or cause severe adverse reactions, posing major risks in polypharmacy. Accurate DDI prediction is therefore important for medication safety and early stage drug screening. Although molecular graph learning has been widely used for DDI prediction, many methods still rely on atom-centered propagation and global graph aggregation, which may obscure interaction-relevant local structural patterns. To address this limitation, we propose SSE-DDI, a molecular structure-driven framework centered on selective substructure encoding for relation…

Loss of blood pressure control after rifampin initiation: a clinically significant antihypertensive drug-drug interaction.

A man in his early 60s with previously well-controlled hypertension developed sudden deterioration in blood pressure control 2 weeks after starting isoniazid-rifampin therapy for latent tuberculosis infection. His blood pressure had previously been stable on an angiotensin receptor blocker and a calcium channel blocker. He presented with a headache and newly uncontrolled hypertension. Rather than representing progression of essential hypertension, the deterioration was temporally associated with rifampicin-induced cytochrome P450 enzyme induction. Rifampin is a potent inducer of CYP3A4 and can

A deep representation learning algorithm on drug-target interaction to screen novel drug candidates for Alzheimer's disease.

Alzheimer's disease (AD) is a serious neurodegenerative brain disorder with complex pathophysiology. While currently available drugs can provide symptomatic benefits, they often fail to cure the disease. Thus, there is an urgent need to explore new therapeutic agents. In this study, we developed DTIP (Drug-Target Interaction Prediction), a machine learning-based approach to search novel drugs for AD by utilizing the information of drug-target interaction (DTI). By training a Skip-gram model on drug-target sequences derived from known DTI information, the algorithm learned the drug-target relat

Severe Impact of Omeprazole Timing on pH-Sensitive Dasatinib Absorption: Unveiling Substantial Drug-Drug Interaction.

The absorption and bioavailability of most tyrosine kinase inhibitors are affected by gastrointestinal pH as they are weak basic lipophilic drugs. Hence, concomitant use of acid reducing agents (ARAs) is frequently restricted. Particularly comedication of crystalline dasatinib (Sprycel) and proton-pump inhibitors (PPIs) should be avoided. Drug-drug interaction (DDI) studies with PPIs report approximately 40%-80% bioavailability reduction of dasatinib. Limitations in the design of these studies do not allow for assessing the near maximum DDI as timing of PPI dosing was either not reported or 22
