Management of labyrinthine fistulae in chronic ear surgery.

The appropriate management of labyrinthine fistulae has been debated in the literature for years. After several recent cases of labyrinthine fistulae at our institution, a review of the published data regarding hearing outcome with fistula management was undertaken. Results of this critical review were presented at departmental grand rounds. The grand rounds presentation, data and discussion are presented to better illuminate the topic of labyrinthine fistula management.

Unilateral endolymphatic hydrops: what about the contralateral ear?

INTRODUCTION: The early recognition of incipient Meniere's disease (MD) in the asymptomatic ear of a patient with known unilateral disease has profound implications for patient management and follow-up, but the criteria for a right and precocious diagnosis is still controversial. MATERIALS AND METHODS: We evaluated forty-nine patients with MD, selected according to Committee on Hearing and Equilibrium guidelines. All patients underwent laminar tonal audiometry, stapedial reflex study, Glycerol dehydration test, Auditory Brainstem Response (ABR) vestibular examination. MRI was performed in 14 …

Jacobson's nerve schwannoma: a rare middle ear mass.

The case of a 55-year-old woman with a middle ear mass is presented. The preoperative diagnostic workup, including an audiogram and imaging studies, and the histopathologic findings of the tumor are reviewed. The tumor, a schwannoma, arose from Jacobson's nerve in the middle ear. The surgical anatomy of Jacobson's nerve and the surgical approach to this tumor and to other tumors of the middle ear space are discussed. Tumors of the tympanic cavity are rare, with the exception of cholesteatoma; otherwise, the most common among them are paraganglioma and facial nerve neuroma. This report represen…

Evaluation of tympanostomy tubes in middle ear affections.

One hundred cases of Chronic Tubotympanitis with or without effusion were treated by myringolomy and Grommet insertion in the tympanic membrane. 60% cases presented with effusion and 40% with tubal catarrh. Hearing loss of conductive type 15-45dB was the main feature in all the cases. The effusion (serous, mucoid and mixed type) was drained in 84% of the cases. The recovery in the state of middle ear was evident in 80% cases showing return to normal tympanogram of type 'A' from type 'C' and type 'B' variety suggesting equalisation of middle ear pressure with the atmospheric pressure after the …

CT and MR imaging after middle ear surgery.

This article describes the current value of imaging in patients after stapes surgery and surgery after chronic otitis media including cholesteatoma. Possibilities and limits of computed tomography (CT) and MRI are described and most important investigation parameters are mentioned. After otosclerosis surgery, CT is the method of first choice in detection of reasons for vertigo and/or recurrent hearing loss in the later postoperative phase. CT may show the position and condition of prosthesis, scarring around the prosthesis and otospongiotic foci. Sometimes, it gives indirect hints for perilymp…

Middle ear problems after a Gow-Gates injection.

BACKGROUND: Knowledge of the potential adverse reactions to mandibular block anesthesia is important in the practice of dentistry. This article presents a complication in the middle ear not previously reported. CASE DESCRIPTION: Unusual middle ear symptoms immediately followed the administration of 1.8 milliliters of 3 percent mepivacaine for a Gow-Gates mandibular block injection. Over the course of 10 days, the patient had complaints of inner ear pressure, inability to equilibrate ear pressure, decreased hearing, pain and severe headache before returning to normal without further complaints…

In-the-ear hearing aids within auricular prostheses.

We report a child with bilateral congenital microtia in whom cosmetic and auditory rehabilitation has been effected using in-the-ear hearing aids within prostheses overlying the rudimentary external auditory meati after canaloplasty. This novel method of auditory rehabilitation has not been reported before and is suitable in selected cases. The prostheses themselves were successfully secured using a prosthetic contact adhesive we have developed that offers excellent retention, little if any skin reaction, and high patient acceptability.

Cytotoxicity due to corrosion of ear piercing studs.

It is well known that allergic and/or inflammatory reactions can be elicited from the use of gold-coated studs, particularly the type used for piercing ears, since they are left in contact with body fluids until the puncture heals. Inasmuch as gold is known as a non-toxic element, other elements of the substrate material may be responsible for some allergies. Therefore, characteristics of the coating, such as defects that expose the substrate to the human skin or body fluids, play an important role in the development of skin sensitization. In this study, the cytotoxicity of commercial studs us…

Sudden hearing loss and autoimmune inner ear disease.

This case report describes the audiologic and medical diagnostic evaluations, results, and treatment options in a patient with a classic presentation of immune-mediated sensorineural hearing loss, commonly called autoimmune inner ear disease (AIED). It reviews findings of the basic battery, immittance audiometry, transient otoacoustic emissions, and auditory brainstem response measures and medical findings over more than 2 years. AIED generally causes asymmetric bilateral sensorineural hearing loss with atypical configuration. Although hearing loss is generally fluctuant, the overall pattern i…

Successful partial ear replantation after prolonged ischaemia time.

We present the case of a 34-year-old male patient who had successful replantation of upper pole of pinna 33 h after amputation. As no vein was anastomosed, systemic heparinisation and subcutaneous injection of heparin to the replanted ear were used to encourage outflow. Complications included arterial spasm and bleeding. Management of similar cases as planned urgent cases rather than emergency cases is discussed.

Effect of laser uvulopalatoplasty on middle ear function.

OBJECTIVES: Palatal musculature is known to be responsible for the active opening of the eustachian (auditory) tube. Because laser-assisted uvulopalatoplasty (LAUP) may involve partial division of these muscles, the effect of this procedure on middle ear pressures (MEPs) and middle ear volumes (MEVs) was investigated. STUDY DESIGN: A controlled interventional trial with one limb receiving treatment and the other none. Preoperative and postoperative measurements were carried out in both groups. METHODS: A control group of 15 normal volunteers and a study group of 22 patients undergoing LAUP a…

Treatment, outcomes, and prognostic factors of ear cancer.

Cancer of the ear is rare and a consensus has not been reached as to the most appropriate treatment. In this retrospective study, we examined the treatment modalities, prognostic factors, and outcomes of patients treated for ear cancer at the National Taiwan University Hospital during a 15-year period. The disease-free survival rates of patients with three different disease grades were compared using the log-rank test. The effects of prognostic factors on survival were examined with Cox's proportional hazard model. Of the 61 ear cancer patients treated from January 1982 through October 1996, 4…

Double cartilage block ossiculoplasty in chronic ear surgery.

OBJECTIVES/HYPOTHESIS: Multiple techniques of ossicular reconstruction have been advocated for hearing rehabilitation in the setting of chronic otitis media No single method can adequately address the clinical spectrum of disease severity. In the situation of a severely diseased ear requiring a canal wall down (CWD) mastoidectomy in the presence of an intact stapes superstructure, the authors have employed a double cartilage block (DCB) ossiculoplasty. The technique and short-term results are reviewed. STUDY DESIGN: Retrospective chart review in a tertiary referral otologic practice. METHODS…

[Middle ear ossicular replacement prostheses: cause of failure].

There are various prostheses used for ossiculoplasty in chronic middle ear disease. Plastipores are one of the most commonly used prostheses. We performed ossiculoplasty with plastipores in 237 ear operations in a five years period. The results of the operations were summarized and causes of prosthesis failure were discussed. In 55 (23.2%) patients, tympanic membrane perforation recurred. Cholesteatoma recurrence was encountered in 16 (6.8%) cases. The extrusion of prosthesis was observed in 10 (4.2%) cases. The underlying ear disease was cleaned in 156 (65.8%) patients. An air-bone gap to wit…

Single-vessel arteriovenous revascularization of the amputated ear.

Successful single-vessel arteriovenous replantation of a completely amputated human ear is described. This result was followed by an experimental study using a rabbit model, to confirm that an amputated ear could survive replantation with only a single arteriovenous anastomosis. Fifteen animals were placed in one of two experimental groups: Group 1-arteriovenous replantation, no treatment (n = 6); and Group 2-arteriovenous replantation with medicinal leeching (n = 9). All ears demonstrated initial reperfusion of the replantation immediately following microanastomosis. Laser Doppler flow readin…

[Mastocytes in chronic inflammation of middle ear mucosa].

INTRODUCTION: Pathohistological studies have shown that in secretory otitis media an increased number of mast cells have been found in mucosa of the middle ear and an increased level of histamine in the cavity of the middle ear. The aim of this study was to analyse the distribution and functional state of mast cells in middle ear mucosa biopsies in patients with chronic otitis media. MATERIAL AND METHODS: The pathohistological analysis was performed on 118 biopsy specimens of mucosa of the middle ear. The samples were taken intraoperatively from 39 patients during the surgical treatment of ch…

[Emergency diseases of the ear (comparative statistical data)].

Statistical data are detailed concerning compositional, therapeutic and diagnostic aspects of urgent diseases of the ear (inflammation and traumas of the external ear, acute and aggravated chronic otitis media, acute neurosensory hypoacusis, etc.). 580 patients were urgently hospitalized to ENT clinic (Moscow General Hospital N 1) at the Russian State Medical University in 1995 for urgent diseases of the external, middle and internal ear. This makes up 28% of a total of hospitalized ENT patients. The most effective policy of treatment for each ear disease form is analyzed.

Multiple myeloma presenting with external ear canal mass.

The manifestations of multiple myeloma are protean and related to bony osteolytic lesions, and to medullar and renal insufficiency. We report a patient who presented with otalgia as the inaugural symptom of multiple myeloma. Local irradiation combined with systemic chemotherapy led to the disappearance of the temporal bone mass and the accompanying symptoms. To date, 24 months after the diagnosis, the patient is still in remission. The literature on otological involvement in multiple myeloma is reviewed. Symptoms are non-specific and include hearing loss, tinnitus, dizziness, facial paralysis,…
