BACKGROUND: We performed an individual patient data analysis of COMBINE AF to assess differences in 14 clinically relevant outcomes between patients with paroxysmal (PAF) vs. non-PAF.
METHODS: Cox-proportional-hazards models stratified by trial and adjusted for CHA2DS2-VASc elements were constructed. Sensitivity analyses were performed across subgroups.
RESULTS: Among 71,466 patients with AF, 16,609 (23 %) had PAF. The overall follow-up period was 157,225 patient-years (median 2.2 years). Compared with non-PAF patients, PAF patients were more likely to be women (43 % vs 35 %), have prior cor

OBJECTIVE: Left atrial appendage (LAA) thrombus has heretofore been considered a contraindication to percutaneous LAA closure (LAAC). Data regarding its management are very limited. The aim of this study was to analyse the medical and invasive treatment of patients referred for LAAC in the presence of LAA thrombus.
METHODS: This multicentre observational registry included 126 consecutive patients referred for LAAC with LAA thrombus on preprocedural imaging. Treatment strategies included intensification of antithrombotic therapy (IAT) or direct LAAC. The primary and secondary endpoints were a

OBJECTIVES: This study hypothesized that cerebral protection prevents strokes in patients undergoing transfemoral transcatheter aortic valve replacement (TAVR) in clinical practice.
BACKGROUND: Preventing strokes is an important aim in TAVR procedures. Embolic protection devices may protect against cardiac embolism during TAVR, but their use and outcomes in clinical practice remain controversial.
METHODS: Isolated transfemoral TAVR procedures performed in Germany with or without cerebral protection devices were extracted from a comprehensive nationwide billing dataset.
RESULTS: A total of 4

BACKGROUND: Strokes caused by normal variants of the cerebral circulation can be difficult to diagnose, hence a high index of suspicion is needed. This case series discusses the clinical and radiological aspects of one such stroke caused by occlusion of the artery of Percheron (AOP).
MATERIALS AND METHODS: Computerized discharge summaries, outpatient records and imaging from picture archiving and communication system (PACS, GE), of patients with AOP infarction over a period of 12-years (2002-2014) were identified and their clinical and radiological features analyzed.
RESULTS: Of 3589 strokes

BACKGROUND: It is unclear if brief episodes of paroxysmal atrial fibrillation (PAF) detected by prolonged cardiac monitoring are an occult of cause of cryptogenic strokes (CS). We compared the incidence of PAF in patients with CS and patients with stroke of known cause (SKC) using prolonged ambulatory cardiac monitoring.
METHODS: We prospectively enrolled patients within 3 months of ischemic stroke to undergo noninvasive cardiac monitoring for 3 weeks. Primary end point was PAF detection independently confirmed by 2 blinded cardiologists.
RESULTS: The study consisted of 132 patients, 66 had

OBJECTIVES: This study evaluated morphologic features of fossa ovalis membrane aneurysm and associated cardiac abnormalities that may predispose to systemic embolism.
BACKGROUND: An increasing number of fossa ovalis membrane aneurysms are diagnosed by echocardiography. Higher frequencies of such aneurysms have been reported in patients with embolic stroke.
METHODS: The hearts of 20 adults with fossa ovalis membrane aneurysms (mean [ +/- SD] age 62 +/- 19 years, range 24 to 87; 12 women [60%], 8 men [40%]) were examined. The areas of excised atrial septum, fossa ovalis membrane and fossa oval…

Brucella endocarditis (BE) is a rare but life-threatening complication of brucellosis and remains the principal cause of disease-related mortality. Prosthetic-valve involvement is exceptionally uncommon and is usually managed with combined medical and surgical therapy because of the high risk of abscess formation and embolic complications. We report a 51-year-old man with a bioprosthetic aortic valve who presented with recurrent fever, weight loss, and acute neurological deficits following previously treated, culture-confirmed brucellosis related to consumption of unpasteurized goat milk. BACKGROUND: Kommerell diverticulum frequently requires thoracic endovascular aortic repair (TEVAR) with carotid-subclavian bypass (CSB). Although CSB is durable, delayed embolic stroke secondary to graft failure is rare. We report a rare case of large vessel occlusion caused by a stump-related embolism from an occluded CSB.
CASE DESCRIPTION: A 34-year-old man with a history of deep vein thrombosis under warfarin therapy developed right hemiplegia and aphasia 85 days after TEVAR and left CSB for Kommerell diverticulum. Upon admission, physical examination revealed a marked inter-arm blood pres…

Marantic endocarditis (ME) is characterised by the formation of sterile valvular vegetations, carrying a high risk of systemic embolism. It is frequently associated with advanced neoplasms, particularly adenocarcinomas of the lung. Osimertinib, often used in lung adenocarcinomas, is linked with thrombocytopenia. We report the case of a 57-year-old female with stage IV lung adenocarcinoma on osimertinib, who presented with acute neurological deficits due to an M2 segment occlusion of the left middle cerebral artery, which was treated with mechanical thrombectomy. The post-procedure course was m

INTRODUCTION: Left ventricular (LV) thrombus is a complication of acute myocardial infarction and is associated with systemic thromboembolism. We describe a trans-aortic endoscopic approach to the removal of an LV thrombus in a patient undergoing concurrent coronary artery bypass grafting and aortic valve replacement.
CASE PRESENTATION: A 47 year old male presented following an embolic middle cerebral artery stroke and underwent transthoracic echocardiography demonstrating a mobile LV thrombus. Additional investigation revealed a moderately stenosed bicispid aortic valve, two vessel coronary

BACKGROUND: Severe coronavirus-induced disease 2019 (COVID-19) leads to acute respiratory distress syndrome with an increased risk of venous thrombo-embolic events. To a much lesser extent, arterial thrombo-embolic events have also been reported in this setting.
CASE SUMMARY: Here, we describe four different cases of COVID-19 infection with ischaemic arterial events, such as a myocardial infarction with high thrombus load, ischaemic stroke on spontaneous thrombosis of the aortic valve, floating thrombus with mesenteric, splenic and renal infarction, and acute limb ischaemia.
DISCUSSION: Card

Atrial fibrillation is associated with substantial morbidity and mortality. Pooled data from trials comparing antithrombotic treatment with placebo have shown that warfarin reduces the risk of stroke by 62%, and that aspirin alone reduces the risk by 22%. Overall, in high-risk patients, warfarin is superior to aspirin in preventing strokes, with a relative risk reduction of 36%. Ximelagatran, an oral direct thrombin inhibitor, was found to be as efficient as vitamin K antagonist drugs in the prevention of embolic events, but has been recently withdrawn because of abnormal liver function tests.

BACKGROUND: Patent foramen ovale (PFO) is often asymptomatic but can cause paradoxical embolism. Although cryptogenic stroke is common, this case is unique due to splenic infarction and a visible thrombus in transit.
CASE SUMMARY: A 30-year-old woman presented with acute dyspnea, hemoptysis, and left upper quadrant abdominal pain. Investigations revealed deep vein thrombosis complicated by pulmonary embolism and paradoxical embolization through PFO, leading to splenic infarction-a rare sequence of events.
CONCLUSIONS: Screening for PFO should be considered in patients with venous thrombosis

BACKGROUND: Non-traumatic convexity subarachnoid hemorrhage (cSAH) is a rarely reported condition with a wide spectrum of etiologies. Cerebral ischemia secondary to extracranial or intracranial atherosclerotic disease has been identified as a relatively uncommon cause of cSAH.
CASE REPORT: We report a case of cSAH caused by cardioembolic stroke. A 69-year old female patient developed suddenly left-sided face and body weakness and numbness and visual neglect on the left. She was newly detected with paroxysmal atrial fibrillation on the ground of thyrotoxicosis. Brain magnetic resonance imaging…
