Unilateral Pulmonary Artery Agenesis in an Elderly Female.

Unilateral pulmonary artery agenesis (UPAA) is a rare congenital anomaly due to a malformation of the sixth aortic arch of the affected side during embryogenesis. It can occur in isolation or may be associated with other congenital cardiovascular malformations. The diagnosis is usually set at adolescence, however it can remain asymptomatic and late diagnosis is possible. Recurrent pulmonary infections, decreased exercise tolerance, and shortness of breath on exertion are the most common symptoms.

Repair and follow-up of Tetralogy of Fallot with pulmonary stenosis.

Tetralogy of Fallot is the most common cyanotic congenital heart defect. Advances in surgical technique and postoperative care have improved survival which is now very good. Patients now face long-term morbidities such as reduced exercise tolerance and arrthymias. Cardiologists caring for these patients are confronted with decisions regarding best care practices. This article will review the evidence available on repair and postoperative follow-up for patients with Tetralogy of Fallot with pulmonary stenosis.

[Successful preoperative respiratory rehabilitation in patients with aortic valve stenosis associated with severe respiratory dysfunction].

We describe 2 cases of aortic valve stenosis with severe pulmonary dysfunction. Preoperative respiratory rehabilitation programmed by the rehabilitation doctors was cautiously undertaken to improve their exercise tolerance and respiratory reserve. These 2 patients underwent aortic valve replacement eventually. Postoperative course in each patient was uneventful without respiratory complication. Preoperative respiratory rehabilitation can be performed in the high risk patient with severe pulmonary dysfunction as long as careful risk management is guaranteed.

Dramatic response to nebulized morphine in an asthmatic patient with severe chronic cough.

We report a 56 year old man with a 16 year history of bronchial asthma who suffered severe coughing spasms that prevented employment, greatly reduced exercise tolerance and caused profound social embarrassment. His cough failed to respond to all standard asthma therapy and nebulized lignocaine. The patient was commenced on nebulized morphine sulphate with dramatic reduction in cough and improved quality of life. This benefit has been maintained at 2.5 years of follow-up.

Impact of eccentric cycling in coronary rehabilitation program: a pragmatic randomized controlled trial versus conventional rehabilitation.

BACKGROUND: This randomized controlled trial examined the feasibility of adding eccentric exercise to a conventional cardiac rehabilitation … (CON: 12.6%, MIX: 16.14%) and ankle MVC (CON: 15.5%, MIX: 11.30%), with no significant differences. Exercise tolerance did not differ significantly between the groups, but perceived effort was significantly lower in the MIX group (P<0.0001) compared to the CON group. CONCLUSIONS: Integrating eccentric exercise into cardiac rehabilitation is safe and well-tolerated. …

Association between Yo-yo intermittent recovery test, physiological markers of endurance performance, and buffering capacity in professional soccer players.

The Yo-Yo Intermittent Recovery Test Level 1 (YYIR1) is widely recognized as a reliable tool for assessing intermittent endurance and aerobic capacity in team sports athletes. This study aims to verify the association between YYIR1 and physiological markers of endurance performance in professional soccer players, as well as the relationship between metabolic buffering capacity, metabolic acidosis tolerance, and YYIR1 total distance. Twenty-six professional soccer athletes (26.3±4.98 years, 76.4±8.96 kg, 178±6.52 cm, 54.1±4.85 mL·kg-1·min-1 V̇O2max) performed the YYIR1 and underwent a cardiopul

Chronic Thromboembolic Pulmonary Disease With Exercise Pulmonary Hypertension: A Noninvasive Model to Predict Exercise Hemodynamics.

BACKGROUND: Chronic thromboembolic pulmonary disease corresponds to exercise impairment after a pulmonary embolism due to persistent chronic thrombi and exercise pulmonary hypertension (PH). Diagnosis requires exercise right heart catheterization (RHC), whereas data on noninvasive diagnosis are scarce. RESEARCH QUESTION: Is there an association between noninvasive parameters and exercise PH among symptomatic patients with chronic thromboembolism? STUDY DESIGN AND METHODS: Data come from a prospective cohort of 92 patients with chronic thrombi and absence/only mild resting PH undergoing cardi

Cardiopulmonary Exercise Testing in Adult Congenital Heart Disease.

Recently, the number of patients with congenital heart diseases reaching adulthood has been progressively increasing in developed countries, and new issues are emerging: the evaluation of their capacity to cope with physical activity and whether this knowledge can be used to optimize medical management. A symptom-limited cardiopulmonary exercise test has proven to be an essential tool, because it can objectively evaluate the functional cardiovascular capacity of these patients, identify the pathological mechanisms of the defect (circulatory failure, shunts, and/or pulmonary hypertension), and

The Effects of "Physical Activity on Prescription" in Persons With Transient Ischemic Attack: A Randomized Controlled Study.

BACKGROUND AND PURPOSE: Transient ischemic attack (TIA) is a strong predictor of stroke, and physical inactivity is one risk factor for TIA/stroke. "Physical activity on Prescription" (PaP) can increase physical activity, but the effect of PaP after TIA has not been described. Our goal was to objectively measure the effect of PaP on physical activity and physical capacity, as well as self-rated health at 3 and 6 months after TIA. METHODS: The primary outcome was moderate to vigorous intensities of physical activity objectively assessed by accelerometry. Secondary outcomes were steps per day a

Ability to work in anaerobic condition is associated with physical performance on the six-minute walk test in older patients receiving cardiac rehabilitation.

OBJECTIVE: During maximal incremental exercise, the ability to work in the anaerobic condition, expressed by the respiratory exchange ratio, is associated with physical performance. Further, peak respiratory exchange ratio is regarded as the best non-invasive measure of a patient's actual exercise effort. This study examined whether ability to work in the anaerobic condition is also associated with physical performance in submaximal constant work rate exercise. SUBJECTS AND METHODS: A total of 75 older patients (51 men, 24 women), mean age 71.1 years (standard deviation 6.7 years), who had re

Systemic lupus erythematosus and exercise.

Systemic lupus erythematosus (SLE) is a rheumatic disease characterized by a variety of symptoms, especially fatigue, pain and reduced quality of life. Physical exercise is a useful tool for improving cardiovascular fitness, reducing metabolic abnormalities and fatigue and improving quality of life. However, very few studies have focused on the relationship between SLE and physical exercise. This paper reviews the main SLE symptoms that can be alleviated by exercising, as well as the results of studies seeking to analyse the exercise capacity and physical training possibilities of SLE patients

Eccentric training for the treatment of tendinopathies.

Tendinopathy can result from overuse and is experienced in the affected tendon as pain with activity, focal tenderness to palpation, and decreased ability to tolerate tension, which results in decreased functional strength. While tendinopathy often occurs in those who are active, it can occur in those who are inactive. Research has shown that an eccentric exercise program can be effective in the treatment of tendinopathies. The earliest studied was the Achilles tendon, and subsequent studies have shown benefits using eccentric exercises on other body regions including the patellar tendon, prox
