Surface replacing arthroplasty of the proximal interphalangeal joint using the CapFlex-PIP implant: a prospective study with 5-year outcomes.

The purpose of this prospective study was to evaluate the 5-year outcomes in patients after proximal interphalangeal joint arthroplasty using the surface replacing implant, CapFlex-PIP. Ninety-two prosthesis were implanted and 65 patients with 68 implants were available for follow-up. The brief Michigan Hand Outcomes Questionnaire score improved significantly from 45 (SD 15) before surgery to 71 (SD 17) at 5 years. On the numeric rating scale, pain during activities decreased significantly from 6.4 (SD 1.9) to 1.8 (SD 1.9). Range of motion of the joints increased significantly from 45° (SD 21)

Salvaging a Failed Proximal Interphalangeal Joint Implant.

Salvaging a failed proximal interphalangeal (PIP) joint implant arthroplasty remains a considerable technical and rehabilitation challenge. Experienced arthroplasty surgeons have reported 70% survival of revision PIP implants at 10 years with 25% of patients requiring subsequent revision surgery. At this time, there is no consensus surgical approach or implant proven superior for revision implant arthroplasty of the PIP joint. Secondary arthrodesis or amputation may be required to salvage the failed PIP implant arthroplasty with compromised bone stock or soft tissue envelopes that are inadequa

Extension block pinning for the treatment of a dorsal fracture dislocation of the distal interphalangeal joint: case report.

A relatively rare case of dorsal dislocation of the distal interphalangeal (DIP) joint associated with compression fracture of volar base of the distal phalanx is presented. An extension block pin was used to maintain the reduction of the DIP joint during active flexion and extension exercise after surgery. At 49-month follow-up, the clinical results and radiographic findings were satisfactory.

[Pseudoainhum and peripheral neuropathy].

BACKGROUND: Pseudoainhum is a rare disease characterised by gradual fibrous constriction of the fingers and/or toes eventually resulting in their amputation. In this article, we report the first case seen in Morocco, highly unusual in terms of its severity. CASE REPORT: A 46-year-old woman with no toxic habits was hospitalised for spontaneous amputation of the fingers and toes. This condition began when the patient was 12 years old with the appearance of a circular constriction band at the base of the fifth toe, eventually resulting in its loss. The patient gradually lost all her other toes a

Hyperplasia of palmar plates and macrodactyly in a young child.

A case is reported of a young girl who presented with macrodactyly of the right middle finger and tumour masses on the palmar side of both interphalangeal joints. The lesions were fibrocartilaginous and appeared to be hyperplastic palmar plates. The macrodactyly and the hyperplasia of the palmar plates were attributed to trauma.

Triggering, clicking, locking and crepitus of the finger: A comprehensive overview.

Triggering, locking, clicking, and crepitus of the fingers are common symptoms patients present with. Even though crepitus and triggering can occur as part of the same underlying diagnosis, it is important to differentiate between them, as they usually indicate different possible diagnoses. The differential diagnoses that should be considered include trigger finger, metacarpophalangeal joint (MCPJ) arthritis, fractures or dislocations, extensor digitorum communis subluxation or dislocation, locked MCPJ, avascular necrosis of the metacarpal head, and Dupuytren's disease. A thorough clinical exa

Severe mutilating injuries to the hand: guidelines for organizing the chaos.

Major hand injuries have become a rarity in Western countries. The fact that there are well trained teams devoted to their management, should not obscure the fact that the first emergency surgeon has the major role of setting the foundations for a reconstruction. Understanding the goal to be sought: the 'acceptable hand' (one with three fingers, with near normal length, near normal sensation and a functioning thumb), is hoped to be of great help in primary care. Preservation of vital structures such as joints, flexor tendons, and vessels, in the initial debridement, which will help to build th

Biomechanical influence of the vincula tendinum on digital motion after isolated flexor tendon injury: a cadaveric study.

PURPOSE: The vincula are specialized mesotendinous structures attaching to the flexor tendons of the hand. In addition to providing vascular supply to the tendons, the vincula can be mechanically important. The purpose of this study was to quantify the influence of intact vincula on digital flexion after flexor tendon laceration and to assess the ultimate strength and stiffness of the vincula. METHODS: The index, middle, and ring fingers of 12 fresh-frozen cadaveric fingers were dissected free at the level of the metacarpophalangeal joint, preserving at least 10 cm of the flexor and extensor

Pachydermodactyly: An Uncommon Benign Digital Disorder.

Pachydermodactyly is a rare, benign, and non-inflammatory digital fibromatosis, typically presenting in adolescents with painless and progressive swelling of the soft tissues around the proximal interphalangeal (PIP) joints. Most often observed in young males, it is frequently linked to repetitive mechanical behaviors such as finger rubbing, clasping, or habitual trauma. Despite its benign nature and preserved joint function, the condition is often mistaken for inflammatory arthropathies, leading to misdiagnosis and potentially inappropriate management. We describe the case of a 14-year-old gi

A Comparison of Patient Characteristics and Outcomes Between Patients Receiving Flexor Digitorum Superficialis Slip Excision or Isolated A1 Pulley Release for Trigger Finger.

PURPOSE: Resection of the radial or ulnar slip of the flexor digitorum superficialis (FDS) tendon is a known treatment option for persistent trigger finger. Risk factors for undergoing FDS slip excision are unclear. We hypothesized that patients who underwent A1 pulley release with FDS slip excision secondary to persistent triggering would have a higher comorbidity burden compared to those receiving A1 pulley release alone. METHODS: We identified all adult patients who underwent A1 pulley release with FDS slip excision because of persistent triggering either intraoperatively or postoperativel

Multiple loose bodies in rheumatoid arthritis.

Intra-articular loose bodies are usually secondary to osteochondral fractures, osteochondritis dissecans, synovial osteochondromatosis, and degenerative arthritis. The authors report four patients with longstanding rheumatoid arthritis and multiple loose bodies in a variety of joints. All patients have remained active despite their disease, suggesting that the loose body production was secondary to trauma of continued activity superimposed on joints containing pannus and eroded cartilage.

Pachydermodactyly--just a sign of emotional distress.

Pachydermodactyly (PDD) is a benign, asymptomatic soft tissue swelling affecting the skin of the lateral aspects of the proximal interphalangeal joints of the fingers II-IV, mostly in young adolescent males, and could be interpreted as a consequence of tic-like behaviour as an obsessive-compulsive disorder in male adolescents. The differential diagnosis includes numerous diseases; a rapid clinical recognition of PDD would avoid many useless and expensive diagnostic tests. There is no effective medical treatment for PDD, but discontinuation of the tic-like mechanical traumatisation generally le

[Dorso-cubital flap of the thumb. An anatomical study with clinical applications. Apropos of 22 cases].

The arterial supply of the dorsal surface of 25 thumbs was studied by light microscopy. An artery, located on the dorso-ulnar side and connecting the head of the first metacarpal bone with the dorsal arcade of the nail matrix, was constantly found in all dissections. The authors report the results of the anatomical study and the clinical applications of a skin flap which can be raised on the dorso-ulnar surface of the M.P. joint with a distal pedicle. Clinical experience is based on 22 flaps. In 18 cases this flap was used for distal skin loss coverage (palmar or dorsal) of the thumb as an isl

[The first Zancolli procedure in the treatment of cubital and mediocubital claw hand. Apropos of 32 cases].

The authors have reviewed 32 cases of claw hand due to lesions of the ulnar nerve only in 25 cases or associated lesions of the ulnar and median nerve in 7 cases treated by the first Zancolli procedure. This procedure consists of advancement of the pulleys and anterior capsulorraphy of the metacarpo-phalangeal joints. One case had to be operated on twice. In 25 instances there was a complete division of the nerve. One 17 occasions the procedure was done on the ring and little fingers only and on 12 occasions on the other finger. The results were excellent or good in 23 cases. An associated ten

Camptodactyly and knuckle pads coexisting in an adolescent boy: connection or coincidence?

Camptodactyly is a condition characterized by a nontraumatic, fixed flexion contracture at the proximal interphalangeal joint, typically involving the fifth finger. Most occurrences are sporadic, but autosomal dominant transmission and syndromic associations have been described in the literature. We describe the case of an adolescent boy who presented to our clinic with a 2-year history of bilateral, nonsyndromic camptodactyly and knuckle pads.

Extensor tendon dislocation after end-to-side transfer in a rheumatoid patient.

We report a case of extensor tendon dislocation at the metacarpophalangeal joint of the middle finger in which it appeared that an ulnar translation force created by an end-to-side transfer contributed to the dislocation. We recommend alterations in technique or alternative method of extensor tendon reconstruction to avoid this complication if unfavorable factors for end-to-side transfer exist.

Closed injuries to the extension mechanism of the digits.

Injuries to the extensor mechanism create an imbalance in the precise flexion-extension system that regulates finger motion. This imbalance results in visible and functional deformities. Although these deformities are initially supple, they can become fixed and progress to joint degeneration. This article outlines early diagnosis and accurate treatment of the most common extensor tendon injuries. Avoidance of late sequellae is the goal.
