Treatment of postmenopausal osteoporosis with slow-release sodium fluoride. Final report of a randomized controlled trial.

OBJECTIVE: To test whether slow-release sodium fluoride inhibits spinal fractures and is safe to use. DESIGN: Placebo-controlled randomized trial. INTERVENTIONS: Slow-release sodium fluoride, 25 mg twice daily, in four 14-month cycles (12 months receiving sodium fluoride followed by 2 months not receiving it) compared with placebo. Calcium citrate, 400 mg calcium twice daily, continuously in both groups. PATIENTS: 48 of 54 patients who received sodium fluoride and 51 of 56 patients who received placebo completed at least 1 year of the study. All patients had postmenopausal osteoporosis. RE

Histomorphometric analysis of iliac crest bone biopsies in placebo-treated versus fluoride-treated subjects.

In a 4-year controlled, prospective trial, histomorphometric analysis was used to compare the tissue-level skeletal effects of fluoride therapy in 43 postmenopausal women (75 mg NaF/day) with those of 35 matching placebo subjects; all subjects received 1500 mg/day elemental calcium supplement. In addition to an initial, baseline biopsy, a second biopsy was obtained after 6, 18, 30 or 48 months. Measurements were made on a third biopsy obtained from 8 subjects following at least 72 months of fluoride therapy. The change in cancellous bone volume or trabecular thickness in fluoride-treated subje

Results from a three-year caries clinical trial comparing NaF and SMFP fluoride formulations.

A three-year, double blind, randomised clinical trial was conducted in Polk County, Florida from 1983-1987. The objective was to compare the effect of four dose levels of sodium monofluorophosphate (SMFP) and a single dose level of sodium fluoride (NaF) on DMFS, DMFT, and DFS Interproximal indices. A total of 8,027 children were examined clinically and radiographically at baseline, and 5,474 children completed the three-year study, which included daily supervised brushing at school. No differences existed at baseline between the five study cells on age or gender distribution, or on any of the

Normal age-related changes in fluoride content of vertebral trabecular bone--relation to bone quality.

In several clinical osteoporosis studies, fluoride treatment has been shown to have a positive effect on bone mass but without a concomitant decrease in vertebral fracture rate. In contrast, some studies have shown that increases in spinal BMD are also paralleled by decreased vertebral fracture incidence. We have previously demonstrated, in a pig model, that 6-month treatment with fluoride increased bone mass but decreased bone quality. The aim of the present study was to elucidate whether normal age-related fluoride accumulation in human bone per se influences bone quality. From 73 normal ind

The severity of dental fluorosis in children exposed to water with a high fluoride content for various periods of time.

Inhabitants of the Ikeno district of Japan were accidentally exposed to drinking water containing 7.8 ppm fluoride (F) for 12 years, after which water with 0.2 ppm F was substituted. Dental examinations of local inhabitants revealed that only children aged seven years or less at the introduction or aged 11 months or more at the removal of the high-F water had fluorosis. Regular inspections were made of the 86 children between those age limits. The severity of fluorosis in three tooth types (first permanent molars, upper central incisors, and first premolars) was assessed and related to the per

Effect of a triclosan/copolymer/fluoride dentifrice on plaque formation and gingivitis: a 7-month clinical study.

A total of 108 adult male and female subjects completed a 7-month, double-blind clinical study designed to compare the effect of a dentifrice containing 0.3% triclosan and 2% of a copolymer of methoxyethylene and maleic acid on plaque formation and gingivitis, as compared to a placebo dentifrice. The subjects were stratified into two balanced groups according to baseline plaque and baseline gingivitis scores. They then received an oral prophylaxis and were assigned to the use of either a placebo dentifrice or the triclosan/copolymer dentifrice for the next 7 months. After this time, the dentif

Prevalence of dental caries and enamel defects in children living in areas with different water fluoride concentrations.

The prevalences of dental caries and developmental enamel defects were assessed in 643 randomly selected children aged 11 to 13 years who were lifelong residents of three areas of Naples with high (4 ppm), optimal (1 ppm), and low (0.3 ppm) concentrations of fluoride in their drinking water. The children living in the high fluoride area had significantly lower dental caries scores (DMFT 0.59, DMFS 1.01) than those in the optimal fluoride area (DMFT 1.67, DMFS 2.87) and those in the low fluoride area (DMFT 1.97, DMFS 3.48). The FDI index of developmental defects of dental enamel (DDE) was used

Treatment of the vertebral crush fracture syndrome with enteric-coated sodium fluoride tablets and calcium supplements.

A cohort of 101 patients were treated with enteric-coated sodium fluoride tablets and calcium supplements. Vitamin D was also given in supra-physiologic doses in 70% of the cases. Lumbar bone mineral density (BMD), as measured by dual-photon absorptiometry, increased in a linear fashion up to four years, irrespective of the value of initial BMD and of the underlying condition, be it involutional osteoporosis (the vast majority), glucocorticoid osteoporosis, or even osteogenesis imperfecta. Estrogen replacement therapy (ERT) seemed to promote the fluoride-induced increase in lumbar BMD, as did

The ultrastructure of dentine from rat incisors following exposure to sodium fluoride and potassium pyroantimonate staining.

Weanling rats were given a single intraperitoneal injection of sodium fluoride and control animals normal saline for four consecutive days. The fluoride produced a consistent response in the mineralizing dentine of the incisors in which a hypermineralized band was succeeded by a hypomineralized band. Potassium pyroantimonate staining for calcium ions showed that following injection of fluoride, in contrast to the controls, there were large amounts of calcium pyroantimonate in the pre-dentine and throughout the odontoblasts. This suggests that fluoride temporarily affects the membrane enzyme sy

Interactions of edrophonium, physostigmine and methanesulfonyl fluoride with the snake end-plate acetylcholine receptor-channel complex.

The actions of two reversible anticholinesterase agents, edrophonium and physostigmine, were compared with the irreversible agent methanesulfonyl fluoride (MSF) on miniature end-plate currents (MEPCs) and ACh-induced end-plate current fluctuations recorded from twitch fibers of costocutaneous muscles of garter snakes (Thamnophis sp.). Low concentrations of edrophonium (less than 25 microM) produced a concentration-dependent increase in both the MEPC amplitude and the time constant of MEPC decay. MEPCs recorded at all concentrations studied (25-100 microM) decayed as a single exponential functi

Effect of the fluoride/calcium regimen on vertebral fracture occurrence in postmenopausal osteoporosis. Comparison with conventional therapy.

We assessed the rates of vertebral fracture in patients with postmenopausal osteoporosis. Forty-five patients were not treated (91 person-years of observation); 59 were treated conventionally, with calcium (alone or combined with estrogen) or vitamin D or both (218 years); and 61 were treated with sodium fluoride combined with conventional therapy (251 years). The fracture rate (per thousand person-years) was 834 in untreated patients, 419 in those given calcium with or without vitamin D, 304 in those given fluoride and calcium with or without vitamin D, 181 in those given estrogen and calcium

Effect of chewable fluoride tablets on dental caries in schoolchildren: results after six years of use.

Two groups of children, initially in the first and second grades, chewed, rinsed with, and swallowed an acidulated phosphate-fluoride tablet containing 1 mg of fluoride either once or twice a day in school. A control group of children followed the procedure once a day using a placebo tablet. An evaluation after six years of treatments showed that both the once-a-day and the twice-a-day procedures with fluoride tablets effectively reduced the incidence of dental caries.

Liver membrane adenylate cyclase. Synergistic effects of anions on fluoride, glucagon, and guanyl nucleotide stimulation.

Some effects of salts on the adenylate cyclase of partially purified plasma membranes from rat liver have been studied. Under conditions where cyclic adenosine 3':5'-monophosphate formation was linear with respect to time and protein concentration, the enzyme was stimulated 3- to 6-fold by 10 mM NaF, 10- to 30-fold by 1 muM glucagon, 4- to 5-fold by 0.1 mM 5'-guanylylimidodiphosphate, and in the presence of 3 muM GTP, 2-fold by 10 mug/ml of prostaglandin E1. Various salts were found to stimulate basal activity slightly, but enhanced the response to NaF 3- to 4-fold, to glucagon 1.5- to 2-fold,
