Delayed spinal cord injury following microchip placement in a dog.

A three-year-old female, entire Yorkshire Terrier dog was examined because it had progressive non-weight-bearing left forelimb lameness and tetraparesis of two weeks duration. Clinical signs were first observed following mating. Examination confirmed non-weight-bearing left forelimb lameness and tetraparesis. Left forelimb muscle atrophy was also noticed. Survey radiography revealed a metallic foreign body consistent with a microchip in close proximity to the left articular facets between the fifth and sixth cervical vertebrae. Computed tomography identified the exact location of the foreign b

A method to retrieve a displaced dental implant from the maxillary sinus.

Dental implants can be displaced into the maxillary sinus. Retrieval by endoscopic or Caldwell-Luc techniques have been previously reported. A modified Caldwell-Luc technique is presented here, where a small round lateral osseous window is created. A plastic surgical aspirator tip is cut and modified to fit tightly into the access window. The sinus is filled with saline, and the patient is placed in a lateral recumbent position in the dental chair with the involved sinus on the underside. The saline acts as a vehicle to bring out the implant. The festooned aspirator is then introduced into the

An unusual cause of ileal perforation: report of a case and literature review.

An ileal perforation resulting from a migrated biliary stent is a rare complication of endoscopic stent placement for benign or malignant biliary tract disease. We describe the case of a 59-year-old woman with a history of abdominal surgery in which a migrated biliary stent resulted in an ileal perforation. Patients with comorbid abdominal pathologies, including colonic diverticuli, parastomal hernia, or abdominal hernia, may be at increased risk of perforation from migrated stents.

Travel of a mis-swallowed long spoon to the jejunum.

Foreign-body ingestion is a relatively common presentation at emergency departments, but long metallic spoon swallowing is an infrequent occurrence. Unlike most cases of foreign-body ingestion, there have been no reported cases of long foreign bodies reaching the jejunum. We report a rare case of a coffee spoon that was swallowed accidentally and passed through the pylorus and duodenal loop and reached the jejunum, with no complications.

Posterior migration of fusion cages in degenerative lumbar disease treated with transforaminal lumbar interbody fusion: a report of three patients.

STUDY DESIGN: A case report of 3 patients with posterior migration of bullet-shaped fusion cages after transforaminal lumbar interbody fusion (TLIF). One patient required emergency revision surgery; the other 2 patients are being observed during conservative treatment. OBJECTIVE: To review cases of posterior migration of fusion cages and report ensuing clinical courses. SUMMARY OF BACKGROUND DATA: TLIF is a commonly used procedure; however, there are few reports describing cage migration after the procedure. In most cases, when posterior cage migration follows posterior lumbar interbody fusi

The influence of cage positioning and cage type on cage migration and fusion rates in patients with monosegmental posterior lumbar interbody fusion and posterior fixation.

In posterior lumbar interbody fusion, cage migrations and lower fusion rates compared to autologous bone graft used in the anterior lumbar interbody fusion procedure are documented. Anatomical and biomechanical data have shown that the cage positioning and cage type seem to play an important role. Therefore, the aim of the present study was to evaluate the impact of cage positioning and cage type on cage migration and fusion. We created a grid system for the endplates to analyze different cage positions. To analyze the influence of the cage type, we compared "closed" box titanium cages with "o

Toxic endothelial cell destruction syndrome after intraocular lens repositioning with intracameral epinephrine.

A 66-year-old Chinese man developed corneal endothelial decompensation after intraocular lens (IOL) repositioning using intracameral epinephrine was performed. The presentation was compatible with toxic endothelial cell destruction syndrome, which is caused by prolonged, direct exposure of the corneal endothelium to relatively high concentrations of intracameral epinephrine. Despite its effective and immediate mydriatic effect, intracameral epinephrine is not recommended for intraoperative mydriasis in procedures such as IOL repositioning or secondary IOL implantation in which minimal irrigati

Stent placement for benign colonic stenosis: case report, review of the literature, and animal pilot data.

BACKGROUND: Permanent metal stent placement for malignant intestinal obstruction has been proven to be efficient. Temporary stents for benign conditions of the colon and rectum are less studied. This is a case study, review of the literature, and observation from an animal model on placement of stents in the colorectum for benign disease. MATERIALS AND METHODS: A 55-year-old man presented with recurrent obstructions from a benign stricture of the distal sigmoid colon. After failed balloon dilations, a polyester coated stent was placed. The purpose of the stent was to improve symptoms and avoi

Massage-induced delayed venous stent migration.

Catheter-directed therapies in chronic deep vein thromboses can help improve leg function by mechanically addressing residual obstruction in lower extremity or pelvic veins, although the reported use of stents in leg veins is relatively unusual. The author reports a case of this type with long-term patency and clinical success, culminating in asymptomatic delayed venous migration of a stent to the right atrium after 3 years. Open heart surgery was required to remove the embedded stent fragments. The attributed mechanism was deep tissue massage of the thigh.

Retrograde jejunoduodenogastric intussusception due to a replacement percutaneous gastrostomy tube presenting as upper gastrointestinal bleeding.

Percutaneous endoscopic gastrostomy (PEG) tube complications can be serious or life threatening. Retrograde intussusception is a very rare complication of PEG tubes with only 9 cases reported in the literature. We describe a case of retrograde intussusception, associated with the use of a Foley catheter as a replacement gastrostomy tube, presenting with upper gastrointestinal bleeding. To our knowledge, this is the first reported case of PEG-related retrograde intussusception successfully managed in a non-surgical manner. Retrograde intussusception likely occurred due to migration of the repla

Device dislocation, probably due to paroxysmal coughing early after percutaneous closure of secundum type atrial septal defect.

In a 56-year-old woman with a secundum-type atrial septal defect, a closure device was successfully implanted with primarily adequate positioning of the occluder. Two weeks after discharge, the patient developed paroxysmal coughing that probably led to a device dislocation. Months later, a considerable left-to-right shunt was detected, and the device was found to be ingrown in a defective position and had to be removed surgically. In conclusion, the case demonstrates that recurrent coughing may interfere with closure devices and may lead to dangerous dislocation.

Embolization coils migrating and being passed per rectum after embolization of a splenic artery pseudoaneurysm, "the migrating coil": a case report.

Acute or chronic blood loss from pseudoaneurysms of the splanchnic artery in chronic pancreatitis poses diagnostic and management challenges. Arteriographic examination offers both diagnostic and therapeutic options, with success rates of 76%-100% for both modalities. In cases of failure of embolization, repeat embolization is also an option. Surgical intervention is advocated for rebleeding and failure of embolization. Evidence-based guidelines regarding the optimal treatment modality for this condition are lacking. There has been a reported case of dislodgement of coil into the stomach throu
