Analysis of technical surgical errors during initial experience of laparoscopic pyloromyotomy by a group of Dutch pediatric surgeons.

BACKGROUND: The adoption of laparoscopic pyloromyotomy (LPM) by pediatric surgeons has been limited due to concerns about long execution times and higher-than-expected morbidity. The aim of the present study was to examine the performance of LPM by pediatric surgeons during the initial stages of their experience. METHODS: Complete videotapes of 50 early LPM performed in one hospital were subjected to Observational Clinical Human Reliability Analysis (OCHRA) by an independent team. RESULTS: This series had a total morbidity of 6% (one intraoperative bleed, one gastric perforation, one incompl

Medicine's greatest gifts to surgery.

Dr. Robert Graves and Dr. Henry Plummer are immortalized by the thyroid diseases bearing their names. Dr. Graves' clinical lectures and Dr. Plummer's "goiter lunches" were considered medical masterpieces; they introduced novel concepts in physical diagnosis that helped surgeons determine when to operate on patients with thyrotoxicosis. Dr. Plummer's observations with perioperative iodine administration helped lower the mortality associated with thyroidectomy--at that time a treacherous operation--to make it one of the safest operations in history. Their fascinating discoveries laid foundations

[Benefits of the 2002-2003 National Program of Extramural Surgery of the ministry of health in ophthalmologic care].

In a recent past article, we reported the general considerations of the national program of extramural surgery in Mexico from the national health secretary. The results obtained concerning ophthalmic care during the last 2 years are presented to provide new information about what has been done in our country, in specialized ophthalmologic care to the economically disadvantaged population who do not have access to other types of medical ophthalmic attention, especially for cataracts and strabismus.

Richard Lower, M.D., physician and surgeon (1631-1691).

Richard Lower (1631-1691) was a follower of William Harvey and conducted extensive studies of the physiology of the cardiovascular and respiratory systems. He employed surgery as a key component of his experimental work. He described and employed cardiac massage and maintained arterial oxygenation through positive-pressure respiration. Making good use of these procedures, he established the role of the lungs in the "admixture of air" to the blood. Lower performed exchange transfusion in dogs and transfused the blood of a sheep into a human. He recognized the role of blood transfusion in replac

[Raoul Palmer and the coelio-surgical adventure from 1940 to 1995].

Raoul Palmer born in Paris in 1904 from Swedish parents with a humanistic tradition and a non-conformist spirit obtained a scientific degree (licence ès-sciences) before starting his medical studies. Attracted by experimental surgery he was at the head of Fiessinger's laboratory before becoming demonstrator of gynecology at the Hôpital Broca in the services of Proust, Brocq, Macquot, Funck-Brentano and Huguier. As a surgeon specialized in sterility since 1935 but conscient as to the difficulty of making a prognosis without being informed of the extent of the lesions, he invented pre-operatory

Reducing the incidence of postoperative nausea and vomiting.

In the first part of this article (Vol 53(7) 1995, p.327), the physiology of vomiting and the factors known to influence postoperative nausea and vomiting (PONV) were summarised. In this second part the therapeutic options available to reduce the incidence of PONV are reviewed.

General surgical adverse events in a UK district general hospital-lessons to learn.

BACKGROUND: An adverse event (AE) is defined as an unintended injury or complication caused by healthcare management rather than the disease process that may prolong admission and lead to disability or death. This study retrospectively assessed all reported general surgery-related AEs in a district general hospital in the south-east of England. METHODS: All general surgical AEs arising from adult inpatient admissions between 2002 and 2007, that had been reported to the risk management team, following completion of the standard 'Adverse Incident Report Form', were retrospectively reviewed. RE…

We then performed propensity score analysis using a 1:3 ratio of general surgery patients to CT patients. Outcomes of interest included 30-d postoperative mortality, 30-d postoperative morbidity, and LOS. Robotic surgery, telerobotic surgery, telepresence, and telementoring. Review of early clinical results.

Although laparoscopic cholecystectomy rapidly became the standard of care for the surgical treatment of cholelithiasis, very few other abdominal or cardiac operations are currently performed using minimally invasive surgical techniques. The inherent limitations of traditional laparoscopic surgery make it difficult to perform these operations. We, and others, have attempted to use robotic technology to (a) provide a stable camera platform, (b) replace two-dimensional with three-dimensional (3-D) imaging, (c) simulate the fluid motions of a surgeon's wrist to overcome the motion limitations of s

Can total thyroidectomy be performed as safely by general surgeons in provincial centers as by surgeons in specialized endocrine surgical units? Making the case for surgical training.

OBJECTIVE: To determine whether surgeons who had received appropriate training in the technique of total thyroidectomy could continue to perform the procedure with minimal morbidity after moving to a provincial surgical practice. DESIGN: Comparison of the complication rates from total thyroidectomy between a specialized endocrine surgical unit and provincial centers. SETTING AND PATIENTS: Six hundred fifty patients undergoing total thyroidectomy by two surgeons over a 5-year period in the endocrine surgical unit at Royal North Shore Hospital, St Leonards, Australia, were compared with 120 pa

Reduction mammoplasty: comparison of results of plastic and general surgeons.

OBJECTIVE: To compare the results in patients who had reduction mammoplasty in a department of plastic surgery with those of patients who had the same operation in a department of general surgery. DESIGN: Retrospective study of casenotes, and questionnaire. SETTING: Department of Plastic Surgery, University Hospital, Linköping, and Department of General Surgery, County Hospital, Motala. SUBJECTS: All 250 patients who had undergone reduction mammoplasty during the five year period 1980-84 (Linköping, n = 173; Motala, n = 77). Distinct challenges faced by surgeons in conducting and publishing surgical research: implications for training, ethics, and academic surgery.

BACKGROUND: Surgical research is beset by a distinct set of challenges that differ meaningfully from those encountered in other medical disciplines. Despite surgeons' historic contributions to medical advancement, the proportion of surgeon-authored publications has declined relative to total medical literature output over the latter half of the twentieth century. This narrative review aims to comprehensively identify, categorise, and discuss the specific barriers surgeons face in conducting and publishing surgical research, with implications for surgical training, research ethics, and academic
