Self-administered acupuncture as an alternative to deliberate self-harm: a feasibility study.

The aim of this mixed methods feasibility study was to demonstrate the acceptability, practicality, and safety of training patients who regularly use deliberate self harm (DSH) to self-administer acupuncture as an alternative coping skill for emotional distress. Ten adult patients with a diagnosis of emotionally unstable personality disorder who regularly self-harmed were recruited to the study following baseline assessment by a psychiatrist. An acupuncturist taught participants to self-acupuncture. During the 6-week intervention participants recorded their emotional distress, coping behaviors

Drug-Checking with Molecularly Imprinted Polymers: Addressing Xylazine-Contaminated Fentanyl.

Xylazine adulterated Fentanyl is considered one of the deadliest drug threats ever faced in the United States. However, due to the complexity of the illegal drug supply, xylazine detection in the frame of drug-checking (harm reduction) remains an unsolved challenge, one which has cost numerous lives across North America. In this work, we present two sensor platforms for detecting xylazine in drug-checking applications. Both platforms are based on molecularly imprinted polymers (MIPs) using vinyl phosphonic acid as the main monomer. By combining the MIPs with thermometric and colorimetric reado…

Effect of varicocele and varicocelectomy on sperm deoxyribonucleic acid fragmentation rates in infertile men with clinical varicocele.

Varicocele is the leading cause of male infertility. It can affect sperm quantity and quality through various non-mutually exclusive pathophysiological mechanisms, mainly oxidative stress. Excessive production of reactive oxygen species may overwhelm the sperm's defenses against oxidative stress and harm the sperm's DNA. Excessive sperm DNA breaks, so-called sperm DNA fragmentation, result from the oxidative stress cascade and are commonly found in the ejaculates of men with varicocele and fertility-related issues. Measuring sperm DNA fragmentation can provide valuable information on the exten

Impact of a reduction in heroin availability on patterns of drug use, risk behaviour and incidence of hepatitis C virus infection in injecting drug users in New South Wales, Australia.

In early 2001, Australia experienced a sudden and dramatic reduction in the availability of heroin. Research examining the impact of the reduction on drug-related harms has yielded a conflicting picture. The current study uses data from a prospective cohort study of anti-HCV negative injecting drug users (IDU) (n=368) to examine patterns of injecting drug use, risk behaviours and HCV incidence before and after the reduction. The proportion of participants mainly injecting heroin declined sharply from 74% to 47% after the onset of the reduction and continued throughout 2001. There was marked sh

"We were focused so long on opioids that stimulants have always been under the rug": Adapting post-overdose outreach for cocaine- and methamphetamine-involved events.

BACKGROUND: Post-overdose outreach programs have expanded in the United States to connect individuals who survive an overdose with harm reduction, treatment, and recovery supports. Despite increased involvement of stimulants (e.g. cocaine and methamphetamine) in overdose morbidity and mortality, these programs have focused almost exclusively on opioids. This qualitative study examines outreach staff perspectives on potential adaptations to better address stimulant-involved overdose following the introduction of a toolkit of stimulant-focused supports. METHODS: Semi-structured interviews were …

Participant perspectives on a safe supply program in Vancouver, Canada: impacts, satisfaction and improvement priorities.

OBJECTIVES: The ongoing toxic drug crisis in Canada, driven largely by fentanyl proliferation, has led to ever-escalating overdose rates. The Safer Alternatives for Emergency Response (SAFER) program was launched in Vancouver, British Columbia (BC) in 2021 to reduce overdose risk by offering prescribed pharmaceutical alternatives, including fentanyl medications. This study aimed to evaluate satisfaction and the program's perceived impact on health and social outcomes among SAFER participants. METHODS: Data were collected from 78 SAFER participants through baseline interviewer-administered que

A pilot evaluation of managed alcohol programs operating in the context of the COVID-19 pandemic.

BACKGROUND: Managed Alcohol Programs (MAPs) are a harm reduction strategy designed for individuals with severe AUD, unstable housing, and previous unsuccessful treatment attempts. MAPs provide access to individualized doses of beverage alcohol alongside other social supports and are effective for stabilizing alcohol consumption and reducing alcohol-related harms. In Canada, MAP models (scattered site outreach or fixed site) were developed in response to the COVID-19 pandemic to reduce harms associated with severe AUD, high-risk drinking, and unstable housing as means of supporting physical iso…

Cost-effectiveness of HIV pre-exposure prophylaxis among female sex workers in Iran.

The Iranian government does not fund pre-exposure prophylaxis (PrEP), and it is not used in Iran due to uncertainties in its cost-effectiveness. In this study, we examined the cost-effectiveness of PrEP among female sex workers (FSWs) in Iran. The cost-effectiveness analysis compared only PrEP, a combination of PrEP with harm reduction (HR) with only HR scenario among FSWs over a 10-year time horizon. The perspective of the healthcare provider was used in the analysis. The primary outcome was disability-adjusted life year (DALY) averted. Analyses included health and cost outcomes, as well as t…

Harm avoidance and incompleteness core motivations in obsessive-compulsive disorder: validation of the Farsi version of the Obsessive-Compulsive Trait Core Dimensions Questionnaire (F-OC-TCDQ) in Iran.

BACKGROUND: Attention to the heterogeneous manifestations of obsessive-compulsive symptoms observed in clinical and nonclinical populations has motivated researchers to use tools to identify homogeneous subgroups of obsessive-compulsive disorder (OCD) patients. The Obsessive-Compulsive Trait Core Dimensions Questionnaire (OC-TCDQ) is used to evaluate two motivational dimensions of OCD (harm avoidance (HA) and incompleteness (INC)), which has been developed in recent years based on the core dimensions model, and to classify homogeneous subgroups. This study aimed to validate the Farsi version o

Externalized nurse-led model for hepatitis C virus microelimination and impact of drug use profile.

BACKGROUND AND OBJECTIVE: Direct-acting antivirals have greatly simplified the treatment of hepatitis C virus (HCV), yet circuits that bring diagnosis and treatment closer to people who inject drugs (PWID) are needed to achieve the elimination targets of the WHO. With this purpose we have established an externalized nurse-driven circuit among former and active PWID in an addiction centre (AC) and a harm reduction centre (HRC). METHODS AND SETTINGS: The nursing staff offered HCV screening, diagnosis and treatment to the AC and HRC users, administered medication after the hepatologist's remote …

Prevalence of Injecting Drug Use and Coverage of Interventions to Prevent HIV and Hepatitis C Virus Infection Among People Who Inject Drugs in Canada.

Objectives. To determine the number of people who inject drugs (PWID) in Canada and the annual coverage of opioid agonist treatment (OAT) and needle-and-syringe provision for PWID.Methods. We estimated the number of PWID in 11 of 13 Canadian provinces and territories in 2011 by using indirect multiplier methods based on provincial and territorial methadone recipient totals and proportion of surveyed PWID receiving methadone. We modeled annual increases for 2011 to 2016 on Quebec and British Columbia longitudinal data. We calculated needle-and-syringe coverage (World Health Organization [WHO] r
