BACKGROUND AND OBJECTIVES: Gout shows an increasing incidence and has become a major health problem in the Western world. Despite effective treatment options, neither diagnosis nor therapeutic efforts can be regarded sufficient to date. It was of interest to investigate current handling of gout among Austrian rheumatologists.
MATERIAL AND METHODS: In 2012, a survey of Austrian rheumatologists concerning diagnosis and management of hyperuricemia and gout was performed. With the consent of the Austrian Society of Rheumatology, 574 society members (343 via electronic mail, 23 via mail) were sent…

Serum uric acid levels are maintained by urate synthesis and excretion. URAT1 (coded by SLC22CA12) was recently proposed to be the major absorptive urate transporter protein in the kidney regulating blood urate levels. Because genetic background is known to affect serum urate levels, we hypothesized that genetic variations in SLC22A12 may predispose humans to hyperuricemia and gout. We investigated rs893006 polymorphism (GG, GT and TT) in SLC22A12 in a total of 326 Japanese subjects. Differences in clinical characteristics among the genotype groups were tested by the analysis of variance (ANOV…

The global population is experiencing a metabolic explosion, with the prevalence of hyperuricemia (HU) as a metabolic disorder and a causal factor for noncommunicable diseases (NCD) increasing rapidly worldwide over the last 2 decades. This rise is attributed to diets high in purine, alcohol, red meat, high-fructose foods, and lifestyle changes. The connection between HU and various NCDs is now stronger than ever. As HU progresses, systemic inflammation arises, leading to endothelial dysfunction and end-organ damage-molecular changes that were previously unrecognized. HU is increasingly seen a…

Febuxostat is a potent non-purine selective xanthine oxidase inhibitor approved by the FDA in 2009 for management of hyperuricemia in people with gout. Areas covered: The authors summarize the pre-clinical studies and pivotal randomized controlled trials of febuxostat when used as a treatment for hyperuricaemia in gout Expert opinion: Febuxostat has clinical efficacy in serum urate lowering, and long-term use leads to improved outcomes such as gout flare frequency and tophus burden. Recently published post-approval placebo-controlled trials have demonstrated urate-lowering efficacy and safety …

According to the characteristics of gout's clinical symptoms, the common gout animal models were analyzed, and the anastomosis and application prospect of the existing gout animal models with the clinical symptoms were discussed. At present, there are three models of gout: hyperuricemia model, gouty arthritis model and gouty nephropathy model. There are many methods of gout modeling, but there is a lack of animal model replication that could reflect the characteristics of gout's clinical symptoms and reflect the etiology of gout, and there is still a gap between the clinical symptoms and the c…

RATIONALE: Gouty panniculitis, characterised by the deposition of monosodium urate crystals in subcutaneous tissue, is a rare clinical manifestation of gout.
PATIENT CONCERNS: The case of a 67-year-old man is reported, who presented an erythematous nodule on the upper part of the right buttock suspicious for an abscess. This was in the context of chemotherapy for non-Hodgkin's lymphoma.
DIAGNOSES: Histopathologic examination demonstrated gouty panniculitis.
INTERVENTIONS: Because infection was suspected, an incision was performed. The lesion was found to be densely calcified and friable, wi

UNLABELLED: BACKGROUND; Familial juvenile hyperuricemic nephropathy (FJHN) is a genetic disorder with the autosomal dominant mode of hereditability; characterized with hyperuricemia, gout and progressive renal disease. Characterization of the disease together with clinical and biochemical findings in patients of Czech population is described.
METHODS AND RESULTS: The bloodlines of three Czech families with FJHN were set up on the basis of their family history. The specimens of blood and urine were taken from 57 family members for biochemical investigations and isolations of genomic DNA. Blood…

Vitamin A, a fat-soluble essential vitamin, is implicated in a large range of physiological processes. Up to now, the associations between v … revalent MetS, general obesity, high low-density lipoprotein (LDL), high total cholesterol (TC) and hyperuricemia increased with vitamin A in a dose-dependent way. Logistic regression analysis showed that serum vitamin A Z scores were positively associated with MetS and central obesity, elevated blood pressure (BP) and elevated triglyceride (TG). Sex stratifica …

This case highlights that pediatric Burkitt lymphoma may present without classic systemic manifestations, instead presenting as acute renal failure secondary to peritoneal lymphomatosis. Prompt recognition of rapid progression, hyperuricemia, and obstructive uropathy, especially with initially normal blood counts, is essential to minimize diagnostic delay and prevent increased morbidity and mortality.

Obesity and hyperuricemia frequently coexist, yet researchers have long lacked a causal framework regarding the two disorders. In this issue of Cell Host & Microbe, Tian et al. address this gap by mapping a uric acid-driven liver-gut axis that remodels gut microbiota, thereby promoting dietary lipid absorption and further obesity.

Spontaneous tumour lysis syndrome is characterized by hyperuricemia, hyperkalemia, hyperphosphatemia, metabolic acidosis and hypocalcemia, that occur even prior to the treatment of a neoplasm. This rare occurrence was encountered in a patient with non-Hodgkin's lymphoma (NHL), of follicular cell type. Conservative but intensive treatment led to complete resolution. Subsequent chemotherapy was well tolerated.
