Immunohistological analysis of synovium treated with abatacept in rheumatoid arthritis.

The aim of this study was to investigate the histological changes following the treatment with abatacept compared with methotrexate (MTX) by an immunohistological examination of synovial tissue for eleven different molecules to detect the expression patterns of cytokines. We histologically assessed the synovial tissues from 10 methotrexate (MTX)-treated RA patients as controls and 5 abatacept plus MTX-treated RA patients. The synovium samples from both group were assessed by hematoxylin and eosin (HE) staining and analyzed for their expression of tumor necrosis factor-α (TNF-α), interleukin-6

[Case of pulmonary cryptococcosis which developed in a patient receiving abatacept therapy for rheumatoid arthritis].

A 59-year-old woman who had been receiving abatacept therapy for rheumatoid arthritis was noted to have multiple nodules in both lungs on routine chest X-ray films. Chest computed tomography (CT) revealed multiple nodules with small cavities in both lung fields. The Cryptococcus neoformans antigen was detected from her serum and bronchial lavage fluids, indicating a diagnosis of pulmonary cryptococcosis. Abatacept was discontinued, and antifungal treatment with fluconazole was started. The lung nodules gradually shrank, and the cavities disappeared. To the best of our knowledge, we describe th

The CTLA4/CD28 gene region on chromosome 2q33 confers susceptibility to celiac disease in a way possibly distinct from that of type 1 diabetes and other chronic inflammatory disorders.

The effect of the gene region on chromosome 2q33 containing the CD28 and the cytotoxic T-lymphocyte associated (CTLA4) genes has been investigated in several diseases with chronic inflammatory nature. In addition to celiac disease (CD), type I diabetes, Grave's disease, rheumatoid arthritis and multiple sclerosis have all demonstrated associations to the A/G single nucleotide polymorphism (SNP) in exon 1, position +49 of the CTLA4 gene. The purpose of this study was to investigate this gene region in a genetically homogeneous population consisting of 107 Swedish and Norwegian families with CD

Response to mirvetuximab soravtansine and weekly paclitaxel based on treatment sequencing in platinum-resistant ovarian cancer: Does order matter?

OBJECTIVES: With multiple clinical trials now showing survival benefit, we sought to determine whether treatment sequencing of mirvetuximab soravtansine (Mirv) and weekly paclitaxel affects response in platinum-resistant ovarian cancer (PROC). METHODS: This is a single-institution, retrospective review of PROC patients who received both Mirv and weekly paclitaxel. Demographics, clinicopathologic factors, and treatment outcomes were compared between patients based on sequence of treatment. Overall response rate (ORR) was defined as complete or partial response based on imaging and/or ≥ 50% dec

Albumin binding improves the pharmacokinetics and therapeutic efficacy of a 177Lu-labeled HER2 Fab radioconjugate.

Therapeutic antibodies and antibody-drug conjugates are limited by large molecular size, poor tumor penetration, and signaling-adaptive resistance. We introduce an engineering strategy for radiotherapeutic antibody fragments in which an albumin-binding domain (ABD) rebalances molecular size and pharmacokinetics. Using human epidermal growth factor receptor 2 (HER2) as a proof of concept, we developed 177Lu-DOTA-Fab-ABD, which integrates rapid tumor penetration with albumin-mediated extended circulation, improving tumor dosimetry while reducing off-target radiation. In HER2 tumor models, 177Lu-

HER2 Heterogeneous Breast Cancer Models Reveal Novel Therapeutic Targets and Subclonal Dynamics during Evolution to Resistance to HER2-Targeted Therapies.

UNLABELLED: Intratumor heterogeneity for human epidermal growth factor receptor 2 (HER2) in HER2-positive breast cancer is a driver of resistance to HER2-targeted therapies. The advancement of treatments for HER2 heterogeneous (HET) tumors has been hindered by the lack of preclinical models that accurately mimic the human disease. In this study, we describe human HER2 HET breast cancer models composed of ERBB2-amplified (HER2hi) and nonamplified (HER2lo) cell populations derived from the same tumor. Utilizing these models, together with cellular barcoding, we demonstrate subclonal cooperation

A novel CD7-directed antibody-drug conjugate targeting BCL-XL with potent anti-leukemic activity in T-cell acute lymphoblastic leukemia.

Current treatments of T-cell acute lymphoblastic leukemia (T-ALL) are based on intensive chemotherapy regimens which provide overall survival rates of ~85% in children and <50% in adults. Therefore, there is an unmet need for novel therapeutic options in T-ALL. Pre-clinical studies and clinical trials have demonstrated that inhibitors of BCL-XL and/or BCL-2, two anti-apoptotic proteins of the BCL-2 family, are anti-leukemic in T-ALL. However, BCL-XL inhibitors (BCL-XLi) efficacy is undermined by severe, on-target thrombocytopenia. We report here the design of a novel anti-hCD7 mAb-based ADC ca

Trastuzumab Deruxtecan in Metastatic Urothelial Carcinoma with NGS-Detected ERBB2 Amplification: A Four-Patient Real-World Case Series.

Background: Next-generation sequencing (NGS)-detected ERBB2 amplification occurs in a subset of urothelial carcinomas, but its role as a treatment-selection marker for trastuzumab deruxtecan (T-DXd) remains uncertain. Methods: We retrospectively reviewed four patients with metastatic urothelial carcinoma treated with T-DXd in routine practice from 2024. Treatment selection was based on NGS-detected ERBB2 amplification because HER2 immunohistochemistry and in situ hybridization were unavailable. Results: Four men aged 65-76 years received T-DXd: one in the second line and three in the fourth or

Enfortumab vedotin induces immunogenic cell death and shows enhanced preclinical antitumor activity when combined with a PD-1 inhibitor.

Enfortumab vedotin is a Nectin-4-directed antibody-drug conjugate designed to deliver the microtubule-disrupting agent monomethyl auristatin E (MMAE) to tumor cells. Using preclinical models of urothelial cancer (UC), we expand the understanding of the multifaceted mechanism of action for enfortumab vedotin that includes direct cytotoxicity on Nectin-4-positive tumor cells, indirect bystander effect on neighboring Nectin-4-negative tumor cells, and MMAE-mediated induction of immunogenic cell death (ICD) and associated increase in activated immune cells in the tumor microenvironment. Trastuzumab Deruxtecan in HER2-Low or HER2-Mutant Recurrent/Metastatic Secretory Gland Cancers of the Head and Neck: A Case Series.

Trastuzumab deruxtecan (T-DXd) has shown efficacy in HER2-positive (immunohistochemistry [IHC] 3+ or IHC 2+/in situ hybridization [ISH]+) recurrent/metastatic (R/M) secretory gland cancers of the head and neck. However, there are minimal published data regarding the use of T-DXd in HER2-low (IHC 1+ or IHC 2+/ISH-) or HER2-mutant tumors. This case series aimed to describe the clinical characteristics and outcomes of patients with HER2-low or HER2-mutant R/M salivary gland cancer (SGC) or lacrimal gland cancer (LGC) who received T-DXd in any line of therapy. Patients included in this series had

Advanced breast cancer immunotherapy: Surface modification of NK cells for embedding antibody-drug conjugates.

The combination of antibody-drug conjugates (ADCs) and immunotherapy has emerged as a promising strategy for cancer treatment. However, effective integration of chemotherapy and immunotherapy remains a challenge. Here, we introduce a universal surface-engineered NK (USE-NK) platform, which rapidly modifies immune cells into a targeted chemoimmunotherapy system by enabling the spontaneous incorporation of antibodies or ADCs. This platform utilizes a recombinant fusion protein (C3) derived from Protein G and a polymeric lipid-based conjugation strategy to stably attach therapeutic antibodies to

Polatuzumab vedotin, rituximab, cyclophosphamide, doxorubicin, and prednisone (Pola-R-CHP) therapy in diffuse large B-cell lymphoma in patients aged 80 years or older: a real-world study.

Diffuse large B-cell lymphoma (DLBCL) predominantly affects older adults, with an increasing proportion of patients aged ≥ 80 years. Although the POLARIX trial demonstrated improved progression-free survival with polatuzumab vedotin (Pola) combined with R-CHP (Pola-R-CHP), its applicability to very elderly patients remains unclear. A multicenter, retrospective, cohort study was conducted at seven hospitals in Japan, enrolling 172 previously untreated DLBCL patients who initiated Pola-R-CHP between August 1, 2022 and May 1, 2024. Patients were stratified by age (< 80 vs. ≥80 years). Initial dos

ProTide-enabled antibody-drug conjugates: A novel platform for the targeted delivery of phosphorylated drugs.

Herein, we report the first integration of ProTide technology with antibody-drug conjugates (ADCs) to create a novel platform that enables the targeted delivery of active, phosphorylated drugs. Our ADCs contain a specially designed ProTide linker that exploits the ProTide enzymatic activation pathway to release monophosphorylated nucleoside analogues directly into target cells. Using gemcitabine as a model drug conjugated to trastuzumab lysines, we demonstrated good antiproliferative activity in HER2-positive cancer cell lines (SKBR3 and MIA PaCa-2) compared to the unconjugated antibody and ex
