Is hyperbilirubinemia a contraindication for neonatal circumcision? A survey of practice patterns of pediatric urologists and a review of the literature.

INTRODUCTION AND OBJECTIVE: 50-80% of term newborns develop jaundice, or hyperbilirubinemia (HB), in their first week. The vast majority have benign etiologies, including physiologic jaundice of the newborn and breast milk/breastfeeding jaundice, which do not affect the synthetic capacity of the liver, thus conferring a low risk of peri-procedural bleeding. Though uncommon, HB in the setting of sepsis, biliary obstruction, or metabolic disease, may increase procedural bleeding risk. Circumcision of neonates with HB has not been well studied. We sought to characterize practice patterns among So

Pancreatic Calculus Causing Biliary Obstruction: Endoscopic Therapy for a Rare Initial Presentation of Chronic Pancreatitis.

INTRODUCTION: Biliary obstruction in chronic calcific pancreatitis (CCP) is often caused by inflammatory or fibrotic strictures of the bile duct, carcinoma of head of pancreas or less commonly by compression from pseudocysts. Pancreatic calculi causing ampullary obstruction and leading to obstructive jaundice is extremely rare. METHODS: The medical records of all patients with CCP or biliary obstruction who underwent endoscopic retrograde cholangiopancreatography (ERCP) over 4 years between 2010-2014 at Kasturba Medical College, Manipal were analyzed. RESULTS: Five patients of CCP with impac

A persistent gallstone-an unusual cause of colonic obstruction in sigmoid adenocarcinoma.

An 88-year-old woman presented with acute post-prandial abdominal pain, nausea, and rigors, with biochemical evidence of obstructive jaundice. Computed tomography demonstrated choledocholithiasis with acute cholecystitis and an incidental segment of thickened descending colon. Following endoscopic sphincterotomy with stone extraction and subsequent laparoscopic cholecystectomy, she was discharged home. She re-presented 6 days later with constipation and lower abdominal pain. Repeat imaging revealed large bowel obstruction with marked cecal dilatation due to migration of a gallstone impacted at

Lemmel Syndrome Presenting with Transient Cholangitis and Bacteremia: A Case Report.

Lemmel syndrome is a rare disease characterized by mechanical obstruction of the bile duct secondary to a periampullary duodenal diverticulum. We present the case of a 74-year-old male patient admitted with generalized abdominal pain and fever. His initial blood work revealed derangement of liver function tests (LFTs) and leukocytosis. Imaging, including ultrasound and contrast-enhanced computed tomography (CECT) of the abdomen and pelvis, showed multiple gallstones without biliary dilation and ruled out any hepatobiliary obstruction and micro-abscesses. However, an upper gastrointestinal endo

[Iatrogenic biliovenous fistula].

A 44-year-old man had suffered from cholecystolithiasis with recurrent jaundice for many years, and a cholecystectomy was carried out. Because of a radiomanometric obstruction of the common bile duct, the intra- and extrahepatic bile ducts were also scanned. One day after the operation the jaundice increased (serum bilirubin max. 742 mumol/l). The cause of the icterus was found in a small biliovenous fistula, and diverse surgical and angiological attempts to resect or close the fistula failed. In the third postoperative week renal failure developed, followed by a disturbance of coagulation and

Biliary tract surgery.

Postoperative wound infection rates after biliary tract surgery vary tremendously from 2% after uncomplicated cholecystectomy to 20% in series including many jaundiced patients. Almost all such infections arise from organisms growing in the diseased biliary tract, since infection rate of 1% are achieved when the bile is sterile. A history of cholangitis identifies only one third of the patients with infected bile, but four easily recognized clinical factors point to positive bile cultures in 60% to 75% of patients: (1) age over 70, (2) obstructive jaundice, (3) common duct stones without jaund

Pizotyline-induced cholestatic jaundice.

We treated a case of drug-induced cholestatic jaundice in which the causative agent was pizotyline (Pizotifen), a phenothiazine-related drug. The patient's symptoms were compatible with either hepatitis or biliary obstruction. Diagnostic laboratory studies were performed to exclude both of these entities. The history of drug ingestion plus the clinical and histologic features established pizotyline as the causative agent.

Guidelines for biliary stents and drains.

Hepatobiliary and pancreatic cancers can cause malignant biliary constriction of the bile ducts, a lethal complication that leads to obstruction of the bile ducts, cholangitis, sepsis, and death. Patients may present with symptoms such as painless jaundice, dark or amber urine, light colored stools, and weight loss. These patients often have locally advanced disease at presentation, and surgical intervention is often not possible. Biliary stents or percutaneous transhepatic drains are often the treatment intervention to relieve biliary obstruction. This is a perspective educational paper provi

Pancreatic metastasis and extrahepatic biliary obstruction in squamous cell lung carcinoma.

Primary lung cancer frequently metastasizes to distant organs. The pancreas is a relatively infrequent site of metastasis. As a result, extrahepatic bile duct obstruction (EHBDO) resulting from pancreatic metastasis from lung cancer is extremely rare and occurs mostly with small-cell lung cancer (SCLC). We report an unusual case in which a patient presented with jaundice and, after extensive workup, was diagnosed with squamous cell carcinoma of the lung with metastasis to the pancreas causing EHBDO. He underwent endoscopic retrograde cholangiopancreatography (ERCP) and the obstruction was reli

Transjugular insertion of biliary stent in patients with malignant biliary obstruction complicated by ascites with/without coagulopathy: a prospective study of 12 patients.

PURPOSE: In patients with malignant biliary obstruction complicated by massive ascites, when endoscopy fails, safe routes for biliary decompression are needed as an alternative to percutaneous approach. We aimed to evaluate the safety and effectiveness of transjugular insertion of biliary stent (TIBS) in patients with malignant biliary obstruction complicated by massive ascites with or without coagulopathy. METHODS: From March 2012 to December 2017, a total of 12 consecutive patients with malignant biliary obstructions treated with TIBS were enrolled in this study. Five patients had jaundice

Primary Sclerosing Cholangitis in a Child: A Rare Case Report.

Primary sclerosing cholangitis (PSC) is a chronic inflammation of the bile ducts that leads to progressive narrowing, ultimately resulting in liver cirrhosis. A 10-year-old boy presented with jaundice, abdominal pain, nausea, weight loss, and yellow papules and nodules on the eyelids, elbows, and axillae. A physical examination revealed jaundice, pallor, and hepatosplenomegaly. The laboratory results revealed anemia, cholestasis, and elevated liver enzymes, along with hypoalbuminemia. The tests for viral hepatitis and autoimmune markers were negative. Abdominal ultrasonography revealed increas

[A Case of Signet-Ring Cell Carcinoma of the Ampulla of Vater in a Young Male].

A male in his twentieth was referred to our hospital for jaundice. Computed tomography(CT)showed dilation of the intrahepatic and extrahepatic bile ducts and showed a lesion at the ampulla of Vater, which caused obstructive jaundice. Upper gastrointestinal endoscopy revealed a tumor of protruded-predominant type with raised margins at the ampulla of Vater, and biopsy from the lesion indicated malignancy. With no apparent distant metastasis, radical resection was assumed to be possible, thus we performed subtotal stomach preserved pancreatoduodenectomy. Before the operation, endoscopic retrogra

Hepatobiliary rhabdomyosarcoma mimicking choledochal cyst: Lessons learned.

INTRODUCTION: The differential diagnosis of hepatic cystic lesions is a challenging process especially in case of hepatic rhabdomyosarcoma (HRMS) presenting as hepatic cyst. PRESENTATION OF CASE: We introduce our experience with a case of HRMS in a 3-year-old female patient who was misdiagnosed to have type IV-A choledochal cyst and definitive correct diagnosis was reached after the pathological and immunohistochemical examination of the surgically resected lesion. This case presentation is followed by important practical messages to hepatobiliary surgeons regarding HRMS. DISCUSSION: HRMS is

Congenital choledochal malformation: not just a problem for children.

Choledochal cysts remain relatively uncommon in Western Europe and the US, although they are appreciably more common in Asia. Their aetiology remains obscure although abnormalities of the pancreaticobiliary junction may be the primary pathology in some, with biliary dilatation following reflux of activated pancreatic secretions. Most anomalies will present in childhood with obstructive jaundice or abdominal pain; a proportion may only present for the first time during adulthood, and some of these will show malignant transformation. The classical triad of pain, jaundice and a palpable mass is,

Biliary strictures complicating pancreaticoduodenectomy.

BACKGROUND: Biliary-enteric anastomotic strictures may complicate pancreaticoduodenectomy. Anastomotic ischaemia and reflux of gastric and enteric contents with secondary bacterobilia and cholangitis may contribute. METHODS: Four patients (3 females, 1 male) with a mean age of 50 yr (range 26-73 yr) presented 1-12 yr following pancreaticoduodenectomy with features suggestive of biliary-enteric anastomotic stricture formation. These included recurrent cholangitis, obstructive jaundice, and liver abscess. Diagnosis was confirmed by percutaneous or endoscopic cholangiography. Endoscopic and radi
