A 68-year-old right-handed woman was admitted to Tokyo Metropolitan Geriatric Hospital because of slowly progressive dysarthria and writing disability over 2-year period. On admission, severe dysarthria was observed, but no dysphagia. The dysarthria mostly resembled a type of pseudobulbar palsy, although it was associated with effortful speech production. An oro-facial apraxia was also found. She could name objects, and could understand spoken words correctly. The quality assurance guidelines for clinical practice in speech/language pathology are effective for assuring the provision of quality clinical services to patients, as well as increasing revenues for hospitals and home care agencies by decreasing or eliminating denials for speech/language pathology claims. The guidelines can be used to evaluate the service and performance of speech therapists employed either directly or by independent contract.

Disproportionate involvement of language has been claimed to be a distinguishing feature of Alzheimer's disease (AD) with onset before age 65. We tested this hypothesis in a group of 133 patients with possible AD by NINCDS criteria. Sixty-one had onset of symptoms prior to age 65; the remaining 72, at 65 or later.
