Serum magnesium estimation was done in 19 children who had heart failure of varied etiology. Five of nine toxic patients and three of 10 nontoxic ones had magnesium deficiency (serum magnesium less than 1.5 mEq. per liter). Mean serum magnesium level was significantly lowered (P less than 0.01) in 19 children and it was further lowered in nine toxic patients (P less than 0.001) as well as in eight hypomagnesemic patients (P less than 0.001) than in healthy control subjects. Mean serum digoxin level in toxic patients was significantly higher than in nontoxic ones (P less than 0.05). In three ca…

BACKGROUND: Magnesium is essential for myocardial function and rhythm. Although typically well-regulated, patients with heart failure are prone to magnesium deficiency. Most clinical laboratories define normal serum magnesium as 1.6-2.6 mg/dL, but this range may include chronic latent magnesium deficiency. The Magnesium Global Network (MaGNet) recommends 2.1-2.3 as optimal. We examined outcomes in heart failure patients across and outside these ranges.
METHODS: Using ambulatory serum magnesium levels, we categorized 627,349 Veterans with heart failure (2001-2023) into <1.6 (6%), 1.6-1.7 (11%)…

Hypermagnesemia is an often-overlooked problem in acute medical care situations. Magnesium is used in several areas of medicine and for various reasons. It also is found in many seemingly innocuous cathartics and gastrointestinal medications. Magnesium replacement in enteral or parenteral feeding and magnesium deficiency is addressed often. Despite the necessity to consider magnesium in several situations, many physicians are relatively unfamiliar with the use of magnesium in their patients. Magnesium is often used in patients with renal insufficiency, which further increases the risk of hyper…

Samples of ventricular and lumbar cerebrospinal fluid (CSF) were taken at intervals from six lactating dairy cows, made hypomagnesaemic by feeding a magnesium-deficient diet. Initially the ventricular CSF had a higher magnesium concentration than lumbar CSF but as the cows became hypomagnesaemic the magnesium concentration in ventricular CSF decreased more rapidly than that in lumbar CSF. Tetany occurred when the concentration of magnesium in ventricular CSF fell below about 0.5 mmol litre-1. Intravenous infusion of magnesium into two hypomagnesaemic cows increased the concentration of magnesi

OBJECTIVE: Magnesium level is known to decline during pregnancy. A suggested role for magnesium deficiency in conditions like pre-eclampsia and pre-term birth has prompted studies with conflicting evidence. The primary objective of this study was to determine the prevalence of hypomagnesemia in pregnancy, while the secondary objectives attempted to define maternal and fetal outcome due to hypomagnesemia.
SUBJECTS AND METHODS: A pilot study was performed to determine the mean serum magnesium level for the population of female patients attending the University of Benin Teaching Hospital. The re…

While studying the effect of low-magnesium diet on the rat liver considerable proliferation and enlargement of cells, identified as the Ito cells, has been observed. These events were accompanied by the accumulation of lipid substances in the cells. It is the authors' opinion that the lipid accumulation in the Ito cells is due to the deficiency in the lipid metabolising and transporting enzymes. Disturbances in the production of enzymatic proteins may result from the considerable and longlasting magnesium deficiency. The contribution of the fat-storing cells to the collagen biosynthesis is als…

The contribution of magnesium to cell differentiation is not clear. Some studies indicate that low extracellular magnesium promotes cell dif … ial cells and pre-adipocytes seems to be independent of extracellular Mg concentration. Conversely, magnesium deficiency retards, while high extracellular magnesium accelerates phorbol ester-induced U937 cell differentiation, probably by interfering with calcium homeostasis or with the activity of kinases. We conclude that the extracellular magnesium concentration aff …

In addition to the well-known complexities inherent in the recognition of hypomagnesemia and/or magnesium depletion, the management of magnesium deficiency oftentimes is not straightforward and requires the understanding of several important principles. Among these is that the avoidance of ineffective undertreatment of patients with symptomatic severe depletion may require parenteral administration of quite large amounts of magnesium for several days. This point is evident in the included case example describing a patient in whom the small amounts of magnesium given initially were entirely ine…

Effect of mineral restriction was studied to clarify which mineral in the diet is most indispensable in preventing paraquat (PQ) toxicosis. ODS rats were chosen as the experimental animal owing to the inability to synthesize vitamin C similarly to humans. Rats were fed with either mineral-adequate or restricted diets dosed with 125 ppm PQ. The mineral-adequate diet was based on the American Institute of Nutrition-76, and the restricted diet was one-half the amounts. Measurements were made on the onset day of PQ toxicosis, body weight changes during the feeding experiment, and changes of two ac

Sodium (Na+) and calcium (Ca++) ions are both involved in the activation of cardiac muscle cells. Na+ ions initially depolarise the cell, and the cell-inward flux of Ca++ ions maintains the plateau phase of the action potential. Ca++ ions trigger the release of more Ca++ ions from the sarcoplasmic reticulum. The free Ca++ ions inside the cell subsequently activate the formation of actomyosin complexes and initiate the contraction. Potassium (K+) and magnesium (Mg++) ions modulate the actions of Na+ and Ca++. Hyperkalaemia lowers the membrane potential, and thereby reduces excitability and cond

The magnesium ion, which is a bivalent cation and is necessary for life, is a co-factor in more than 300 enzyme reactions. It has an action on uterine myometrium causing it to relax by stimulating B2 adrenergic receptors and cyclical AMP. It competes with the calcium ion which brings about inhibition of myosine kinase, and therefore a drop in phosphorylated myosine. Deficiency of magnesium is one explanation for uterine overactivity; because it is difficult to estimate how important it is in certain premature labours because this is caused often by many factors, and the levels of plasma magnes

Prostacyclin, synthesized from arachidonic acid, is a strong vasodilator and the most powerful inhibitor known for platelet aggregation. Magnesium deficiency as a risk factor for cardiovascular diseases was related to imbalance of thromboxane and prostacyclin in the vasculature. In this study, we examined the effect of a low level of magnesium on prostacyclin generation in cultured human umbilical vein endothelial cells by measuring arachidonic acid release, 6-ketoprostaglandin F1alpha (6-keto-PGF1alpha) production, calcium ((45)Ca2+) influx, and activity of phospholipase A2 (PLA2) and cycloox…

BACKGROUND: Laboratory investigations may be added to existing requests either automatically on the basis of algorithms (reflex testing) or by laboratory professionals (reflective testing). The clinical utility of reflex and reflective testing is not fully established. We studied efficiency (number of tests that needs to be added to make a diagnosis) and effectiveness (number of diagnoses) of reflex and reflective testing in selected biochemical scenarios.
METHODS: Using fixed rules, we prospectively measured efficiency and effectiveness of reflex and reflective testing in the following scena

(1) Dietary deficiencies of Zn, Mg and K lead to a rapid drop in serum concentrations, with no change (Zn) or a slow decline (Mg and K) in the concentrations in skeletal muscle. (2) These deficiencies all lead to inhibition of growth and protein synthesis in muscle. (3) The inhibition of protein synthesis is faster in onset than the loss of Zn, Mg and K from muscle and therefore unlikely to result from cellular mineral deficiency. (4) The deficiencies are likely to be detected by the early drop in serum concentration, but the mechanism is unknown. (5) Possible mediators of growth inhibition ar

The "spontaneous" tetanic attacks in magnesium (Mg) deficiency resemble those occurring in pyridoxine (Pyr) deficiency. A common mechanism may underlie both seizure patterns. Nine groups (n = 10) of male Mongolian gerbils that were Mg-deprived, Pyr-deprived, or both were used to explore this possibility. The animals were tested for spontaneous seizing in an open field at four-day intervals over 40 days. By the end of the period, 70% of the gerbils that were both Mg- and Pyr-deprived had seized, and 90% had died. One fatality, but no seizures, occurred among Pyr-deprived groups given 500 to 2,0

Recent findings on changes in the mineral content of tissues in bovine hypocalcaemia and hypomagnesaemia show the necessity of using different mineral solutions for the treatment and prevention of these conditions. Infusion of 2% CaCl2-6H2O with 10% MgCl2-6H2O and 5% glucose one after the other or in combination produced severe functional changes in cardiac activity and respiration. Recommended concentrations for treatment or prophylaxis of hypomagnesaemia were 10 g magnesium chloride and 2 g calcium chloride, or alternatively 12 g magnesium adipate and 5 g calcium gluconate, in 100 ml of dist
