UNLABELLED: Over the past three years, several reports have been published on jaw osteonecrosis possibly being associated with the administration of bisphosphonates. Bisphosphonates are highly active inhibitors of osteoclasts. These drugs are used for the treatment of multiple myeloma, bone resorption in the case of metastatic malignant diseases, tumor-associated hypercalcaemia, and in the treatment of osteoporosis. Due to the importance of this presumed side-effect of bisphosphonates for the dentist and the maxillofacial surgeon, we report four cases.

The skeletal effects of radiation are dependent upon many variables, but the pathologic features are consistent. Radiation may cause immediate or delayed cell death, cellular injury with recovery, arrest of cellular division, or abnormal repair with neoplasia. Radiation necrosis and radiation-induced neoplasm still occur despite the use of supervoltage therapy. Complications of radiotherapy are well known and have led to more judicious use of this therapeutic modality. With few exceptions, benign bone tumors are no longer treated with irradiation. Radiation necrosis may be difficult to differe

Chondrosarcoma is among the most common primary bone tumors in adults. In the mandible, chondrosarcoma is a very uncommon malignant cartilage-producing tumor. This case report shows how virtual planning combined with other digital technologies may improve masticatory function rehabilitation in patients with enlarged mandibular chondrosarcoma. The present study reports a case of a 52-year-old male patient who was initially diagnosed with a mandible chondroma, which was successfully excised with no evidence of malignant transformation. Nevertheless, the patient's symptoms recurred after 10 years

BACKGROUND: Odontogenic carcinoma with dentinoid (OCD) is a rare and controversial entity, which has not yet been included in the current World Health Organization classification of odontogenic lesions. Owing to the small number of reported cases, the clinicopathological characteristics, biological behavior, prognosis, and appropriate treatment strategies for OCD remain to be defined. Herein, we present an additional case of OCD with a focus on the differential diagnosis and review of the pertinent literature, in order to enable better recognition by oral clinicians and pathologists and furthe

BACKGROUND: Actinomyces species are commensal oral cavity flora that can cause jaw osteomyelitis. Osteomyelitis of the jaw by Actinomyces is rare, and its presentation can be confused with many different pathologies.
CASE PRESENTATION: This is the case of a 61-year-old female with breast cancer and on chemotherapy as well as non-invasive carcinoma of the tongue who initially presented to the dentist with white spots in the right mandible near the incisors associated with right mandible pain and swelling. Actinomyces-induced osteomyelitis of the mandible was diagnosed. The patient was treated

OBJECTIVE: Extensive mandibulofacial defects can be challenging to reconstruct. We present the case of a complex mandibulofacial defect reconstructed with a mega, chimeric fibula free flap.
METHODS: Ablation of the oral cavity tumor resulted in a large defect involving mandible, floor of mouth, and tongue. Skin of the chin and neck as well as the lower lip were also resected. A fibula free flap was harvested with the skin paddle involving most of the lateral compartment.
RESULTS: The fibula free flap was split into proximal (80 cm2) and distal (120 cm2) skin paddle islands, which were suppli

BACKGROUND: Tracheocutaneous fistula represents one of the most troublesome complications of prolonged tracheostomy. Simple closure of a fistula can be ineffective, particularly in the context of prior surgery and adjuvant radiation. As such, modes of repair have expanded to include locoregional flaps and even free tissue transfers.
OBJECTIVE: This paper describes a case of persistent tracheocutaneous fistula in an irradiated patient who had undergone previous unsuccessful attempts at repair.
METHOD AND RESULTS: The use of regional fasciocutaneous supraclavicular flap with prefabricated conc

Ameloblastoma is an aggressive odontogenic tumor which typically occurs between third and fourth decade of life that often needs resective approach. Immediate reconstruction may show better results. The treatment of multicystic ameloblastoma in the mandible being a rare case that occurred in the late second decade of life, which was surgically removed along with the affected teeth with safety margins, and the region was immediately reconstructed using a vascularized graft, removed from the fibula. Its integration, in combination with osseointegrated dental implants and fixed implant-supported

Cemento-ossifying fibroma is a benign growth, considered to originate from the periodontal ligament. It is often encapsulated and predominantly located in the mandible, and presents as a slow-growing lesion, but may cause deformity if left untreated. The definitive diagnosis of these lesions requires integration of its clinical, radiological and histological features. We present a case of cemento-ossifying fibroma in relation to the lower left one-third of the face, and its management.

Communications between the transverse cervical nerve (C2 and 3) and the cervical branch of the facial nerve have been reported. We describe a case in which the transverse cervical nerve joined the marginal mandibular branch of the facial nerve. It was found during a selective neck dissection and to our knowledge is the first report of such a communication. We discuss the clinical relevance particularly in relation to confusion with the marginal mandibular branch of the facial nerve when operating in the submandibular region.

AIM: This is to report a rare case of ameloblastic fibrodentinoma (AFD), including detailed history, histopathologic as well as radiographic findings. Her condition, a mixed odontogenic tumor, has rarely been reported in publications. We also included the gist of theories put forward regarding the histogenesis of this lesion.
BACKGROUND: AFD is a rare and controversial entity; it boasts a variety of classifications and terminology based on its unique biological nature as well as histopathological features.
CASE DESCRIPTION: The patient was a 13-year-old girl with a chief complaint of central

Reconstruction of the mandible has evolved significantly over the last 40years. Early attempts were often disfiguring and wrought with complications but with the introduction of free tissue transfer of well vascularized bone in the 1970's there was a significant improvement in outcomes. In recent years the harvest, inset, and microvascular anatomosis have been refined to the point that success rates are reported as high as 99% throughout the literature. Focus has now shifted to optimizing functional and aesthetic outcomes after mandible reconstruction. This paper will be a review defect classi

Solitary bone cysts (SBCs) of the jaws are often polymorphic, show scalloped borders when located between the teeth roots, are devoid of an epithelial lining, and are usually empty or contain blood or a straw-colored fluid. The numerous synonyms referring to these lesions reflect their uncertain nature (eg, traumatic bone cyst, simple bone cyst). SBC, also found in other skeletal locations, is often suspected after epidemiologic and radiologic test results and confirmed at surgery. Histology usually shows fibrous connective tissue or only bone. The various etiologic elements responsible for SB

Burkitt's lymphoma is an undifferentiated non-Hodgkin's B-cell lymphoma. Three clinical subtypes are recognized: African (endemic), American (sporadic) and HIV associated. Sporadic Burkitt's lymphoma is a rare malignancy among western populations. This report describes a case of sporadic Burkitt's lymphoma of the jaws with an alarmingly rapid spread associated with acute renal failure. This type of rapid progression bespeaks the need for prompt recognition and life-saving referral by the dental practitioner. The clinical features of Burkitt's lymphoma involving the jaws include severely hyperm

An extensive tumour in a 7-year-old girl, leading to severe disfigurement, proved to be an aggressive fibromatosis on histological examination. Eighteen months after surgery there was no evidence of recurrent disease. This suggests that tumour resection and reconstruction of the mandible had been successful. Contrary to some reports, tumour resection led to curative therapy whereas radiotherapy failed.

A solitary plasmacytoma occurring in the mandible of a 15-year-old Korean male 6 years after renal transplantation is reported. The tumour presented as a hyperplastic gingival overgrowth in the right madibular molar area. Histology and immunohistochemistry revealed plasmacytoma and monoclonality of the kappa chain and IgG. In situ hybridization for Epstein-Barr virus encoded RNA (EBER) showed positive signals in the tumour cells. The tumour regressed after reducing the immunosuppressive agents with concurrent radiotherapy. The patient remains in a stable condition with normal renal functions a
