The purpose of this study was to examine electrophysiologic abnormalities, including motor-evoked potentials, in a patient with post-traumatic syringomyelia before and after syringopleural shunting. A patient with C5 quadriplegia presented with pain, ascending sensory loss, and new weakness in the left upper extremity two yr after spinal cord injury. MRI revealed a syrinx extending from C2 to T12. We measured central motor conduction times (CMCTs) to the biceps, median F-wave latencies, needle electromyography and motor nerve conduction studies. Six days before surgery, CMCTs were 9.0 ms on th

Diabetes mellitus is associated with several non articular rheumatic conditions and is a cause of Charcot's arthropathy. We report three cases of long-standing insulin-dependent diabetics who developed an inflammatory monoarthritis of the ankle. There was no evidence of a peripheral neuropathy or sepsis. They were all seronegative for rheumatoid factor. In two the synovitis persisted; in the third there was a gradual resolution. This type of inflammatory synovitis has not been previously described in diabetes. It developed after a mean duration of diabetes of 34.3 years (range 18-52 years). We

Reports of isolated long thoracic nerve palsy are rare in Japan. We reported a case of isolated long thoracic nerve palsy, resulted from recurrent injury to the nerve. Muscle CT and electrodiagnostic study were useful for confirming diagnosis of this cases. This patient was a student aged 20 years, with nothing of importance in his family or past history. After he started practicing archery, winging of left scapula was gradually developed. Physical examination revealed weakness and atrophy of left serratus anterior muscle. There was no wasting and weakness of other should girdle muscles. Hemat

Short-latency somatosensory evoked potentials elicited by stimulation of the posterior tibial nerve at the ankle were studied in 75 preterm infants. Normative data for the latency of the first cortical component (P1) were obtained for infants from 27 weeks gestation to term, and showed a linear decrease with increasing gestational age. As the pathway of this response traverses areas of the brain likely to be affected by ischaemic and haemorrhagic lesions, abnormalities in the response might indicate later motor disorder.

Nerve conduction velocity is the parameter most often attended to when performing nerve conduction studies, but amplitude and duration of the nerve potentials should also provide important information about the status of the nerve examined. This study compared the results of amplitude and duration measurement to nerve conduction velocity to determine whether or not the various parameters correlated. Using standard techniques, median nerve motor and sensory conduction studies were performed on 390 unselected patients. The results were analyzed and correlation statistics calculated between the p

The pudendo-pudendal reflex (bulbocavernosus reflex) was studied in male and female rats. Electrical stimulation of pudendal nerve afferents induced reflex volleys in ipsi- and contralateral pudendal nerves. The early components of the reflex were unaffected by acute spinal transection. The reflex was shown to be polysynaptic and bilaterally organized. No qualitative differences were noted between males and females, but a significantly longer central delay was observed in female rats, compared to males.

Acute brachial neuropathy is an uncommon etiology of shoulder pain and disability. It can, however, present in association with athletic activity and therefore must be included in the differential diagnosis of athletes with such symptomatology. Findings that should alert the examiner to the possible presence of acute brachial neuropathy include 1) onset with noncontact as well as contact sports, 2) rather acute onset of pain without specific inciting trauma, 3) persistent, often severe pain that continues despite rest, 4) patchy brachial plexus and/or peripheral nerve involvement, and, 5) domi

Mechanisms of ventilatory acclimatization to chronic hypoxia remain unclear. To determine whether the sensitivity of peripheral chemoreceptors to hypoxia increases during acclimatization, we measured ventilatory and carotid sinus nerve responses to isocapnic hypoxia in seven cats exposed to simulated altitude of 15,000 ft (barometric pressure = 440 Torr) for 48 h. A control group (n = 7) was selected for hypoxic ventilatory responses matched to the preacclimatized measurements of the experimental group. Exposure to 48 h of hypobaric hypoxia produced acclimatization manifested as decrease in en

A 14-year-old girl developed right peroneal nerve palsy following diet for weight reduction. Peroneal neuropathy started after sitting and leg-crossing. The main electrophysiologic findings showed a severe slowing of conduction velocity in the above-to-below capitulum fibulae segment and a striking reduction in the size of the compound motor action potential when the nerve was stimulated above the knee. These features are consistent with focal demyelination and conduction block of the nerve fibres. The left peroneal nerve showed the same results but less severe. There were no electrophysiologi

Within the sampled population of cutaneous unmyelinated afferent neurons (n = 94), only the C-polymodal nociceptor population was reactive to the pruritogen cowhage. Of 62 C-polymodal neurons tested, 11 were unresponsive to cowhage. No C-polymodal neurons were more responsive to inactive, than to active, cowhage (n = 17) and all were responsive to mechanical (n = 62) stimuli and noxious heat (n = 24). The range of conduction velocities obtained by single-unit recording techniques was similar to that found by signal averaging the activity from larger strands of nerve. Hence, it is concluded tha

The study was performed on 2 lines of rat selected for high (H) and low (L) excitability threshold of the nervous-muscle apparatus. H-line rats have higher values of spike conduction velocity in caudal and tibial nerves, as compared to those obtained in L-line rats. It is suggested that behavioral differences between these two lines of rat may be connected with changes of neurological characters both of peripheral and central nervous system, as a result of selection.

A 40-year-old man sustained a circumferential crush injury to his right forearm. Four months after injury, he experienced the onset of numbness and tingling in the distribution of the median nerve after exercise. Elevated compartment pressures of the palmar forearm and slowing of median nerve conduction after exercise suggested chronic exertional compartment syndrome. A flexor fasciotomy led to complete relief of symptoms, which allowed the patient unrestricted activity.

Six patients with the syndrome of "painful legs and moving toes" were studied. Although clinical differences were nonspecific, the EMG disclosed two subgroups: one with a simple, erratic pattern of spontaneous activities in foot and leg muscles, and the other with a complex alternating pattern in antagonistic muscles. In the first group, the physiopathologic mechanism is thought to act in the periphery: in the lumbar roots when local anesthesia of the posterior tibial nerve suppresses spontaneous discharges, or in the nerve trunk when it is ineffective. In the second group, the symptomatology

The reliability of the electrophysiological criterion of conduction block in determining the presence of focal demyelination in a peripheral nerve has been studied in an animal model. Demyelination was produced in the rat tibial nerve by one or two closely spaced microinjections of lysophosphatidylcholine (LPC). Histological and electrophysiological data were obtained on the acute lesion (up to 6 days), and during recovery (up to 11 weeks). Single LPC injections produced a lesion of very variable severity. Double injections more reliably produced a severe lesion with marked conduction block. S

A chronic alcoholic who had also been submitted to partial gastrectomy developed a syndrome of continuous motor unit activity responsive to phenytoin therapy. There were signs of minimal distal sensorimotor polyneuropathy. Symptoms of the syndrome of continuous motor unit activity were fasciculation, muscle stiffness, myokymia, impaired muscular relaxation and percussion myotonia. Electromyography at rest showed fasciculation, doublets, triplets, multiplets, trains of repetitive discharges and myotonic discharges. Trousseau's and Chvostek's signs were absent. No abnormality of serum potassium,

When the membrane potential was manually clamped, the non-cholinergic excitatory potential was associated with either a sustained increase, an initial decrease followed by a prolonged increase or no apparent change in neuronal input resistance. In the majority of neurons studied, the amplitude of non-cholinergic depolarization was augmented upon conditioning hyperpolarization, whereas it was attenuated in a low Na solution. The results are consistent with the suggestion that the non-cholinergic depolarization may be generated by a change of multiple conductances that may include GNa activation

A double blind study of partial thiamin restriction was undertaken in 19 volunteer male medical students for a period of 4 to 5 weeks. Each day the subjects followed a diet supplying 500 micrograms thiamin and took 1 capsule randomly assigned to contain 5 mg thiamin hydrochloride or thiamin placebo. This capsule also supplied physiological doses of other vitamins. At the end of the study all were repleted with thiamin hydrochloride. Ten thiamin-depleted subjects were correctly identified by low urinary thiamin excretion, decreasing erythrocyte transketolase activity, and elevation of the thiam

Observations have been made on a series of 228 patients with hereditary motor and sensory neuropathy, comprising 120 index cases and 108 affected relatives. These could be separated into genetically distinct type I and type II categories depending upon whether motor nerve conduction velocity in the median nerve was below or above 38 m s-1. These disorders constitute separate genetic subgroups within the clinical spectrum of 'peroneal muscular atrophy'. Type I cases were more numerous. Most were of autosomal dominant inheritance, but a proportion were sporadic. Four families with probable autos
