Vancomycin-resistant Enterococci Colonization Without Clinical Infection in a High-acuity Pediatric Intensive Care Unit: Challenging the Rationale for Universal Surveillance.

BACKGROUND: Routine surveillance for vancomycin-resistant enterococci (VRE) colonization remains the standard practice in many intensive care units. However, in high-acuity pediatric intensive care units (PICUs), where clinical VRE infections are uncommon, universal screening may impose substantial financial and operational burdens with uncertain clinical benefits. This study evaluates the necessity and value of universal VRE surveillance by describing the clinical characteristics and risk profiles of asymptomatic VRE-colonized patients in a newly established, high-acuity, 54-bed PICU. METHOD

Revealing the benefit of eye motion for acuity under emulated cone loss.

Retinal degenerative diseases progressively erode the cone photoreceptor mosaic, reducing the retina's spatial sampling power, yet visual acuity is remarkably resilient to cone loss. Prior work has shown that clinically normal visual acuity (20/25 or better) can persist despite up to 50% of cone cells being lost (Ratnam et al. 2013, Foote et al. 2018). However, studies on individuals with retinal degeneration are limited by patient recruitment and cannot control for patients' stage of disease progression, creating the need for an experimental paradigm that can mimic these diseases in healthy s

Not just exotropia: a case of persistent visual complaints in a 10-year-old boy.

PURPOSE: Intermittent exotropia is a common childhood condition often considered benign. However, atypical features such as reduced distance visual acuity due to accommodative spasm, rapid progression without clear cause, or unusually late onset may point to an underlying neurological cause. METHODS: We report the case of a 10-year-old boy referred for a second opinion due to progressive intermittent exotropia and reduced distance visual acuity. RESULTS: At the time of referral, clinical examination revealed an accommodative spasm, which explained the reduced acuity. This spasm was initially

Visual and refractive outcomes after implantation of two models of trifocal intraocular lenses in eyes with previous corneal ablation to treat hyperopia.

BACKGROUND: To assess whether a trifocal intraocular lens (IOL) with neutral spherical aberration (SA) provides better visual and refractive outcomes than a trifocal IOL with negative SA after hyperopic corneal laser ablation. METHODS: This is a retrospective comparative study. Patients were classified according to the IOL implanted after cataract or clear lens phacoemulsification [group 1, PhysIOL FineVision Pod-F (negative SA); group 2, Rayner RayOne Trifocal (neutral SA)]. We evaluated uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), uncorrected intermedi

Reversible Hearing Impairment Due to Inferior Colliculi Compression by a Pineal Glial Cyst.

Pineal glial cysts associated with bilateral hearing impairment are very rare. Here, we present the case of a 13-year-old boy with a pineal cyst, which caused severe bilateral hearing impairment persisting from 6 years of age. When the patient was 6 years old, the bilateral hearing acuity was about 40 dB on audiometry. Upon admission to our otolaryngology department, his audiogram revealed a bilateral worsening of the hearing acuity (80 dB). Magnetic resonance imaging (MRI) revealed an abnormal pineal cyst with tectal compression from the left with hardly normal bilateral brainstem auditory ev

[Orbital decompression in Graves' orbitopathy-Experiences and results].

BACKGROUND: Graves' orbitopathy is the most frequent extrathyroidal manifestation of Graves' disease, affecting approximately 25-50% of patients. It leads to inflammation and swelling of orbital soft tissues. The treatment is mostly conservative. Surgical orbital decompression is indicated in severe cases with disfiguring exophthalmos or an acute steroid-refractive threat to vision, facilitating visual and cosmetic recovery. An important aspect in the quality of care is the avoidance of postoperative diplopia. OBJECTIVE: To report experiences and results from 100 cases of orbital decompressio

Visual disturbance following shunt malfunction in a patient with congenital hydrocephalus.

A 25-year-old woman presented with complaints of nausea and headache. She had been treated with a ventriculoperitoneal shunt for hydrocephalus when she was 7 months old. Her bilateral optic discs showed moderate atrophy. Right visual acuity allowed only perception of hand movement and left visual acuity was 0.02 (1.2). Computed tomography (CT) showed mild ventricular dilation but no periventricular lucency. Intracranial pressure (ICP) was not high when the shunt valve was punctured. Her visual acuity deteriorated 5 days after the consultation. She was referred again 8 days after the first cons

Endoscopically assisted transconjunctival decompression of traumatic optic neuropathy.

Traumatic optic neuropathy (TON) is a severe sequel after maxillofacial trauma. Recent clinical experience has suggested that optic canal decompression is beneficial in the treatment of TON. With the advent of endoscopy in orbital reconstruction, we extended this technique to decompress the optical canal and reported its outcome. This technique was applied to 30 patients with TON who did not improve after corticosteroids treatment. The average age of the patients at the time of injury was 26 years. Nine patients presented preoperative vision of light perception (LP) or better and 21 patients h
