Apical inflammatory root resorption: a correlative radiographic and histological assessment.

AIM: To assess the reliability of routine single radiographs in the diagnosis of inflammatory apical root resorption by correlating the radiographic and histological findings. METHODOLOGY: The material comprised serial and step serial sections of plastic-embedded root-apices with attached apical periodontitis lesions that were prepared for a previous study and the diagnostic radiographs. The histological sections of 114 specimens were analysed by light microscopy and categorized into three groups: (i) those without any resorption (0); (ii) those with moderate resorption (+); and (iii) those w

Anti-inflammatory effects of dexamethasone on periapical tissues following endodontic overinstrumentation.

An animal model was developed in which the anti-inflammatory effects of dexamethasone could be examined histologically in periapical tissues following endodontic overinstrumentation. Mandibular first molars containing vital or partially necrotic pulp tissue were instrumented beyond the apical foramen in rats. Fifty microliters of sterile saline or dexamethasone sodium phosphate (0.4 mg/ml) were administered in a blind manner by supraperiosteal infiltration into the buccal vestibule. The rats were killed after 6, 24, and 48 h. Block sections of the mandibular molars were processed for histologi

Recognizing Odontogenic Maxillary Sinusitis in the ENT Clinic: Four CT Findings That Should Prompt Dental Evaluation.

Objective: This retrospective cohort study evaluated the proportion of maxillary sinuses with radiographic findings consistent with an odontogenic contribution among patients with imaging-confirmed acute or chronic maxillary sinusitis presenting to a single academic ENT clinic over one year. Sixty-five CT scans (84 sinuses) underwent multidisciplinary review for prespecified radiographic features. By highlighting imaging findings indicative of dental origin this study aims to improve recognition of possible odontogenic disease and appropriate multidisciplinary evaluation. Methods: Sixty-five p

Influence of dealcoholized red wine on the healing of bone with induced apical periodontitis in rats.

Apical periodontitis (AP) is an inflammatory disease influenced by local and systemic factors. Nutritional compounds can modulate the inflammation, helping the healing process. The aim of this study was to evaluate the local and systemic effects of dealcoholized red wine during the repair of alveolar bone with induced AP. Thirty-five male Wistar rats with AP that had their teeth extracted were arranged into five groups according to the supplementation: no supplementation (NC); red wine (RW); dealcoholized red wine (DRW); and alcohol (AL). The extraction of healthy teeth from rats with no suppl

Protoporphyrin IX fluorescence for real-time visualization of bacterial persistence during root canal treatment.

INTRODUCTION: Apical periodontitis (AP) results from microbial infection within the root canal system. Although mechanical and chemical debridement techniques have advanced, real-time intraoperative visualization of bacterial persistence remains limited. This study evaluated whether fluorescence-aided root canal treatment (FARCT) using protoporphyrin IX fluorescence (PpIX-FL) could assist clinicians in visualizing bacterial persistence in real time during routine endodontic procedures. METHODS: Fifty patients (23 initial treatments, 27 retreatments) underwent root canal treatment. PpIX-FL was

Evaluating Dental Pain in Diverse Conditions: Findings From Short-Form McGill Pain Questionnaire Study.

INTRODUCTION AND AIMS: Making precise diagnoses of patients with dental pain is challenging. Thus, a screening system that can help accurately stratify patients depending on the severity of their condition is required. In this study, we compared the characteristics of dental pain in patients with different conditions using a Short-Form McGill Pain Questionnaire (SF-MPQ). METHODS: A total of 1541 patients admitted to the Peking University School of Stomatology between February 2022 and July 2023 were assessed. Demographic data and pain characteristics were collected. Pain was assessed using th

Present status and future directions - Guided endodontics.

Luxation injuries and other stimuli may lead to a pulp canal obliteration (PCO). Even though the apposition of tertiary dentine is a sign of a vital pulp, in some cases root canal treatment is indicated in the long term due to apical periodontitis or pulpitis. Depending on the extent of PCO, root canal treatment may be challenging even for experienced and well-equipped endodontic specialists. The 'guided endodontics' (GE) technique was introduced 6 years ago as an alternative to conventional access cavity preparation for teeth with PCO and apical pathosis or irreversible pulpitis. Using three-

Antibiotics for the urgent management of symptomatic irreversible pulpitis, symptomatic apical periodontitis, and localized acute apical abscess: Systematic review and meta-analysis-a report of the American Dental Association.

BACKGROUND: Patients with pulpal and periapical conditions often seek treatment for pain, intraoral swelling, or both. Even when definitive, conservative dental treatment (DCDT) is an option, antibiotics are often prescribed. The purpose of this review was to summarize available evidence regarding the effect of antibiotics, either alone or as adjuncts to DCDT, to treat immunocompetent adults with pulpal and periapical conditions, as well as additional population-level harms associated with antibiotic use. TYPE OF STUDIES REVIEWED: The authors updated 2 preexisting systematic reviews to identi

Healing of a cyst-like lesion involving an implant with nonsurgical management.

Numerous reports primarily in the implant literature outline the loss of an implant claimed to be as a consequence of endodontic pathosis from an adjacent tooth. This case report outlines the resolution of a large cyst-like periapical lesion that resolved with nonsurgical management when the canal was dressed with calcium hydroxide on 4 occasions because of persistent intracanal exudate. A favorable healing outcome was achieved for the affected tooth and the implant. This report adds to a growing body of evidence to suggest that implant retained crowns are not vulnerable when an endodontic inf
