Management of skin disease in patients with lupus erythematosus.

Skin disease in patients with lupus erythematosus may be subdivided into two broad categories - those represented by a 'specific' histopathology, the interface dermatitis, and those with changes that are not specific to lupus erythematosus, for example, vasculitis, mucin infiltration, etc. The specific skin lesions that are most common are discoid lupus erythematosus (DLE) and subacute cutaneous lupus erythematosus (SCLE). Evaluation will allow the treating physician to assign a prognosis. Cutaneous lesions can generally be managed with standard therapies. Patients with discoid LE and subacute

Vaccination practices among North American trauma surgeons in splenectomy for trauma.

BACKGROUND: The purpose of this study was to examine trama surgeons' practice patterns regarding immunization of splenic injury patients. METHODS: Data were analyzed from surgeons responding to a survey sent to 557 adult trauma surgeons in the United States and Canada. The survey queried the timing and use of vaccinations in splenic injury patients. RESULTS: Three hundred four (54.6%) surgeons responded to the survey, with 43 no longer active. Of the 261 active surgeons, 99.2% immunize their splenectomized patients, whereas 15.7% immunize those who undergo splenorrhaphy and 8.4% immunize tho

Routine prophylactic antibiotic use in the management of snakebite.

BACKGROUND: Routine antibiotic prophylaxis following snakebite is not recommended but evidence suggests that it may be common practice in Zimbabwe. This study set out to determine and describe the extent of this practice at Parirenyatwa Hospital, a large teaching hospital in Zimbabwe METHODS: A retrospective case review (1996 to 1999 inclusive) of all cases of snakebite was undertaken at Parirenyatwa Hospital. Cases with a diagnosis of snakebite, presenting within 24 hours of the bite and with no complications or concurrent illness were defined as "routine prophylactic antibiotic use". RESUL

Antidepressant side effects and physician prescribing patterns.

We studied the following questions: (1) Do physicians preferentially prescribe antidepressants (ADs) with the least side effects (SEs) of sedation, orthostatic hypotension, and anticholinergic activity; (2) Have there been any recent changes in prescribing patterns; and (3) Do patterns differ for psychiatrists and nonpsychiatrists. Data on psychiatrist and non-psychiatrist outpatient prescribing of specific ADs were obtained from the National Prescription Audit (NPA) for 1986 and 1989. In 1986, physicians did not minimize the use of ADs with the most of these SEs. The 1989 data indicate that p

Management of urinary incontinence in gynecological practice in Norway.

In order to investigate how Norwegian gynecologists manage female urinary incontinence in their daily practice, a questionnaire about six typical case histories was sent to the members of the Norwegian Gynaecological Association. The questionnaire comprised questions about investigations, use of laboratory tests and treatment options. Of 268 questionnaires sent out, 190 were returned (71%). Frequency-volume charts were recommended by 42% of the doctors and pad-weighting tests by 12%. Urodynamic examination was recommended for 55% of the cases, but mainly in mixed incontinence (76 and 77%). Pel

Development, Pilot Testing, and Preliminary Validation of a Parenteral Nutrition Macronutrient Assessment Tool.

INTRODUCTION: Parenteral nutrition is an intravenous form of nutrition support for patients unable to meet nutritional needs through the gastrointestinal tract. While guidelines exist for parenteral nutrition macronutrient prescription, variability in clinical practices highlights the need for an assessment tool to evaluate clinician decision-making. This study aimed to develop, pilot test, and perform preliminary validation of a parenteral nutrition macronutrient assessment tool in a sample of clinicians to evaluate prescribing practices. METHODS: Eight clinical scenario-based questions were

Association Between Socioeconomic Conditions and Biologic Prescriptions for Inflammatory Bowel Diseases in the Brazilian Public Healthcare System: An Ecological Study.

PURPOSE: Inflammatory bowel diseases (IBD) are chronic conditions affecting millions of people globally. Brazil's public healthcare system provides free access to IBD medications. However, socioeconomic conditions may affect prescribing patterns. This study examined associations between income inequality, human development, and biologic prescription rates for IBD in the Brazilian public health system (SUS). METHODS: Data from the SUS Outpatient Information System (2008-2022) were analyzed. The biologic prescription rate (percentage of biologic prescriptions among all IBD prescriptions) was th

Evaluating dispensing patterns of Parkinson's medications from 2012 to 2024 in Australia: insights from the Pharmaceutical Benefits Scheme concessional database.

INTRODUCTION: Parkinson's disease is a neurodegenerative disorder that affects patients' lives by deteriorating their motor and non-motor functions through the loss of dopaminergic neurons. Medications remain the preferred option to manage Parkinson's disease symptoms. In Australia, the Pharmaceutical Benefits Scheme (PBS) supports access to essential medicines by subsidising their cost, making long-term treatment more affordable. AIM: This study analyses the dispensing patterns of Parkinson's medications in Australia from 2012 to 2024, focusing on utilisation trends over time and shifts in p

Establishing an endoscopic enucleation of the prostate practice: a survey of strategies and challenges by successful early career urologists.

INTRODUCTION: Endoscopic Enucleation of the Prostate (EEP) remains a size independent technique for the treatment of Benign Prostatic Hyperplasia (BPH) with many benefits. Adoption has lagged behind other interventions due to a purported steep learning curve and high cost of equipment. The Prostate Enucleation Collaborative (PEC) is a group of early career urologists which formed in 2024 to promote EEP education and research. Here we report the results of a survey of PEC members addressing common practice patterns and member experiences early in their learning curve to help aspiring non-EEP pr

Variations in Paediatric Intussusception Radiologic Enema Reduction Practices Across Aotearoa New Zealand: Results From a Nationwide Cross-Sectional Study.

INTRODUCTION: Most children with intussusception are successfully treated with radiologic enema reduction. Our recent nationwide study revealed significant variation in enema reduction success and the requirement of secondary operative intervention. This may be due to variation in the management of intussusception. As such, radiologic enema management patterns across Aotearoa New Zealand (AoNZ) were investigated. METHODS: A snapshot survey of all AoNZ radiologists who had managed paediatric intussusception in the preceding 5 years was performed. Survey answers were collected anonymously via R

Current practice patterns for cleaning tonometry tips and diagnostic or laser lenses.

In an online survey of >1000 ophthalmologists, most reported using alcohol wipes or swabs to clean tonometry tips (73%) and diagnostic (63%) or laser (50%) contact lenses. Use of disposable devices or high-level disinfection (HLD) was uncommon and driven largely by facility policies or regulatory requirements. Only 19% of those using HLD did so because they believed it to be safer. Ocular surface toxicity and device damage were commonly seen with HLD. Reports of infection transmission were exceedingly rare, with 99% indicating they had never encountered a case. More than 75% of respondents wer

Current management and treatment of patients with myasthenia gravis in France: A survey of the neurologist's perspective.

Myasthenia gravis (MG) is a rare autoimmune disease in which immunoglobulin G autoantibodies targeting post-synaptic receptors at neuromuscular junction play a central role. Due to recent therapeutic developments, there is a need to update current guidelines. FILNEMUS (the French network for rare neuromuscular diseases) conducted a survey in neurologists treating patients with MG to better understand the current clinical and therapeutic management of MG and to identify difficulties and unmet needs. The 45-questions survey assessed various aspects of MG management: including characteristics of
