PURPOSE: Some patients bedridden from various causes such as stroke or spinal cord injury experience poor control of bowel movement. This causes fecal leakage and diarrhea, increases the risk of perianal excoriation and bed sores, and is a burden on caregivers. A total of 128 pediatric pyogenic perianal infections were recorded prospectively in 1984-1995; over 70% arose in children below the age of 2 years. A quarter of all perianal abscesses developed a recurrence between 3 weeks and 3 years after the initial operation, while five children had an anal fistula after deroofing of anal abscesses. The early age of onset and frequent anomaly of anal crypts (12%) suggests a partly congenital etiology due to impaired fusion of the proctodaeal membrane or rudimentary anorectal duplications for both conditions.

A prospective study of 122 patients with anorectal abscesses was carried out to elucidate the value of abscess drainage with primary fistulectomy, when fistulas were identified. Patients were followed from 2 1/2 to 3 1/2 years with a mean follow-up of three years. Fistulas manifesting in the perianal region but originating in the abdomen often have their true origin not recognized for extended periods. Examples of seven such complex cases are presented and discussed. Removal of the abdominal abnormality is mandatory before any cure can be effected; the perianal portion usually heals without any special local treatment.

Screening for rectal chlamydia was not routinely offered in our department until we had a local outbreak of lymphogranuloma venereum (LGV) in men who have sex with men (MSM) in 2004/2005. We later decided to extend the screening to high-risk women (women who had receptive anal sex, contacts of gonorrhoea, women, with anorectal symptoms, women who had been sexually assaulted). An experience with treatment of 3160 patients with acute paraproctitis under conditions of a specialized department is presented. Rational curative methods allow to achieve healing of 2280 patients (72.2%) with small amount of complications (22 cases). The duration of treatment was shortened to 11.2 days. No recurrences were noted.

A 51 year old Yugoslavian patient was admitted to our hospital in reduced general condition with distinct hypocalcemia, osteomalacia, and with rectum stenosis. Our investigations led to the diagnosis of a malabsorption syndrome due to non-tropical sprue. The most likely cause of the rectum stenosis is an abuse of ergotamine-containing suppositories for several years. A gluten-free diet and the interruption of the use of the suppositories improved her general condition remarkably.

Classical enterobacterial strains are sensitive to the working concentrations of pervomur, dioxydine, resorcinol, sodium sulfacyl, iodopyrone, chlorhexidine and boric acid, resistant to the action of cetyplyridinium chloride, rivanol, roccal and ethonium. In enterobacterial populations strains with acquired resistance to chloramine B, iodopyrone, chlorhexidine and resorcinol are present. Hospital strains of enterobacteria are characterized by higher, in comparison with extrahospital strains, resistance to ethonium, sodium sulfacyl, lodopyrone, chloramine B and resorcinol.
