[Open reduction and internal and external fixation combined with anchor repair of intercarpal ligament for the treatment of peripheral injury of lunate bone].

OBJECTIVE: To evaluate the feasibility of internal and external fixation after open reduction combined with suture anchors to repair the intercarpal ligaments for the treatment of perilunate injury, and to explore its operative techniques and therapeutic efficacy. METHODS: From September 2012 to September 2016, 11 patients with perilunate injury were surgically treated with Kirschner wires in intercarpal articulations, 3.0 mm cannulated screws for scaphoid fracture, absorbable anchor for intercarpal ligament repair, together with fixators at intercarpal joints, among whom 6 suffered from peri

The long-term effects of force-transmission in an injured wrist, measured with dual-energy X-ray absorphometry: a case report.

PURPOSE: To investigate the feasibility of using a dual-energy X-ray absorphometry (DEXA) scan to predict long-term force-transmission patterns in wrists. MATERIAL AND METHODS: Both wrists of a man with morbid Kienböck stage IIIa disease of his left wrist (avascular necrosis of the lunate) were examined by a DEXA scan to determine the differences in bone density in the distal radius. RESULTS: In the distal radius of the injured wrist, a shift in bone density was seen toward the scaphoid fossa, which resembles the shift in force-transmission pattern described in force-transmission studies of

Concomitant Avascular Necrosis of the Lunate and Proximal Pole of the Scaphoid in a Thalassemia Minor Patient.

Avascular necrosis (AVN) of multiple carpal bones is a very rare condition. This case report presents a patient with concomitant AVN of the lunate and proximal pole of the scaphoid that had led to severe joint space narrowing and degenerative arthritis in the radiocarpal and distal radioulnar joints (DRUJs). She was known to have thalassemia minor hemoglobinopathy. Wrist arthrodesis and distal ulna hemiresection-interposition arthroplasty (Bowers technique) for a DRUJ were offered and performed. The patient was satisfied with the clinical outcomes.

Carpal coalition with radioscaphoid synostosis and hypoplastic thumb.

Carpal coalition is an anomaly that is usually discovered as an incidental finding on roentgenograms. The most common site is between the lunate and the triquetrum, though fusion of almost every combination of carpal bones has been reported. Carpal coalition can be isolated but has also been associated with numerous congenital malformation syndromes. In this article, we report the case of a 12-year-old boy with left-sided asymptomatic fusion of the trapezoid and trapezium, fusion of the radius and scaphoid, and hypoplasia of the thumb.

[Long-term results of Russe modified scaphoid screw fixation II].

Proximal pole scaphoid nonunion with a small necrotic proximal fragment presents therapeutical problems. Cannulated screw fixation as a modification of the Russe II procedure combines the advantages of minimal surgical exposure and trauma, preserving the blood supply of the distal fragment, and at the same time achieving rigid fixation of the autogenous bone graft. Six out of seven patients had satisfactory subjective and objective results after a mean follow-up of 12.9 years, although radiographic findings were less favorable.

[Pathomorphology of the wrist joint in malunion of the distal radius after osseous fracture healing].

Morphological changes subsequent to malunion of distal radius fractures involve altered bone density along articular surfaces and wrist ligaments. The inclination of articular surfaces of the scaphoid and lunate in frontal and sagittal planes frequently leads to a dorsal dislocation of the distal fracture fragment. X-ray densitometry of subchondral mineralization shows zones of higher density along the palmar articular border, resulting from excentric load transmission. Typical morphological changes involving wrist ligaments are demonstrated and analyzed.

Percutaneous Screw Fixation for Distal Oblique Scaphoid Fractures: A Computed Tomography-Based Evaluation of the Dorsal and Volar Approach.

BACKGROUND: The scaphoid height-to-length (H/L) ratio assesses reduction quality, with lower values indicating better anatomic restoration and reduced malunion risk. We hypothesized that dorsal percutaneous fixation of acute, unstable distal oblique fractures yields a lower postoperative H/L ratio compared with volar fixation. METHODS: We retrospectively reviewed all Herbert B1 fractures treated with percutaneous screw fixation at a single, tertiary referral center over 10 years. Dorsal and volar approaches followed standardized protocols, with guidewire placement under fluoroscopy and headle

Minimally Invasive Arthroscopic-Assisted Treatment for Isolated Scaphoid Fossa Die Punch Nascent Malunion.

A die punch fracture is an intra-articular fracture of the distal radius characterized by a localized depression of the articular surface, typically occurring in the lunate fossa. Misdiagnosis or incorrect management can lead to articular malunion. While die punch fractures in the scaphoid fossa are less common, their nascent malunions pose significant treatment challenges. Traditionally, these fractures are treated with an open dorsal and volar approach, combined with osteotomy and internal fixation. Arthroscopic-assisted osteotomy has emerged as a promising technique due to its precision and

Reconstruction of scaphoid waist non-unions using a radial approach: Preliminary outcome analyses.

This study aimed to evaluate clinical and radiographic outcomes in a consecutive series of patients who underwent open surgery via a radial approach for reconstruction of scaphoid waist non-union. Over a 2-year period, 16 males with scaphoid waist non-union received surgery using a radial approach; we performed limited styloidectomy, biconcave curettage, ovoid bone grafting from the styloidectomy fragment, and placement of a retrograde radial-entry screw. Mean patient age was 25.1 years (range 17-56 years). Bony union was determined on radiographs and computed tomography. Radiographic variable

Vascularized thumb metacarpal periosteal pedicled flap for scaphoid nonunion: An anatomical study and pediatric case report.

PURPOSE: Through an anatomical review, the primary aim of this study was to delineate the dorsal thumb metacarpal (TM) periosteal branches of the radial artery (RA). In addition, we report here the clinical utility of a vascularized TM periosteal pedicled flap (VTMPF), supplied by the first dorsal metacarpal artery (FDMA), in a complex case of scaphoid nonunion. METHODS: Ten latex-colored upper limbs from fresh human cadavers were used. Branches of the RA were dissected under 3x loupe magnification, noting the periosteal branches arising from the FDMA. The VTMPF was measured for both length (

[Scaphoid fracture in motocross riders].

Motocross racing is a demanding motorcycling discipline with significant physiological and psychological demands. Upper extremity injuries are frequently encountered. Interestingly, motocross riders present with a significantly stronger left arm, even if the left hand is not dominant. This difference is attributed to the use of the clutch lever with the left hand, which is more frequent in motocross than in Enduro or desert rally. The wrist has been reported to be involved especially among motocross racers in contrast to road racing. Besides wrist fractures, scaphoid fractures have been previo

Vascularized bone grafting using the 1,2 intercompartmental supraretinacular artery for the treatment of Preiser's disease.

INTRODUCTION: Preiser's disease refers to a rare idiopathic avascular necrosis of the scaphoid that often leads to pole collapse and poor clinical outcomes when managed conservatively. Vascularized bone grafting (VBG) using the 1,2 intercompartmental supraretinacular artery (ICSRA) flap has been used to restore perfusion, but its treatment efficacy across different disease stages remains unclear. METHODS: We retrospectively reviewed eight women (mean age 60.6 years) who were treated using 1,2 ICSRA VBG between 2006 and 2022. Disease stage was determined using the Herbert radiographic classifi

Scaphoid waist fractures in the presence of carpal coalitions: a consistent biomechanical weak point?

Carpal coalitions may alter intercarpal kinematics and load transfer. Reported coalition-associated scaphoid fractures show an exclusive predilection for the scaphoid waist, raising the possibility of a non-random biomechanical mechanism with focal stress concentration. These injuries have been associated with delayed union and nonunion. This pattern may justify CT-based assessment and consideration of a lower threshold for early surgical stabilization, including percutaneous screw fixation even in selected nondisplaced waist fractures. However, the currently available evidence is limited to c

Carpal fractures: epidemiology, classification and treatment of 6542 fractures from the Swedish Fracture Registry.

This study describes the age and sex distribution, trauma mechanism, treatment and influence of patient-reported outcomes of 6542 carpal fractures from the Swedish Fracture Registry (SFR). The most commonly fractured carpal bone was the scaphoid (60%), followed by the triquetrum (25%), hamate (5%) and trapezium (4%). The mean age at injury was 41 years, and 69% of patients were male. The age and sex distribution of carpal fractures differed substantially between the different carpal bones. Men were more likely to sustain a carpal fracture after high-energy trauma and were more likely to be tre

Triquetrum Excision in Radioscapholunate Arthrodesis for Posttraumatic Radiocarpal Osteoarthritis With Ulnar Impaction Syndrome.

Dependent on fracture type, concomitant carpal injuries, and operative treatment, distal radius fractures can lead to symptomatic radiocarpal osteoarthritis. In addition, radial shortening can cause ulnar impaction syndrome. Radiocarpal arthrodesis and ulnar shortening osteotomy are known and frequently applied surgical procedures for each of those pathologies. There are limited data concerning treatment options for a combined disorder. The presented technique in this article demonstrates that radioscapholunate arthrodesis with distal pole scaphoidectomy and total triquetrum excision successfu

Dynamic repair of scapholunate dissociation with dorsal extensor carpi radialis longus tenodesis.

This study investigates the results of a technique using an extensor carpi radialis longus (ECRL) tenodesis for symptomatic scapholunate instability. Symptomatic scapholunate instability has been corrected so far either by limited wrist fusion or by various techniques of soft tissue repair. Limited wrist fusion greatly reduces wrist motion and increases the probability of osteoarthritis in the remaining mobile wrist segments. On the other hand, most types of soft tissue repair are technically difficult to perform and have disappointing results due to the inherent laxity. The presented dynamic

Acute coronal plane scaphoid fracture and scapholunate dissociation from an axial load: a case report.

Coronal fractures of the scaphoid are rare and can be difficult to diagnose. Axial load injuries that result in a complete coronal fracture of the scaphoid associated with an acute scapholunate dissociation are exceedingly rare. In our patient the radiographic finding of wide scapholunate dissociation was obvious; however, the coronal scaphoid fracture was not recognized initially nor suspected. During surgery the coronal scaphoid fracture was identified, reduced anatomically, and fixed with a compression screw. The scapholunate ligament also was repaired. A good result was obtained with retur
