Surgical treatment for avulsion injuries of the humeral lesser tuberosity apophysis in adolescents.

BACKGROUND: There is little published information regarding avulsion fractures of the humeral lesser tuberosity in adolescents, and no consensus exists on optimal treatment. The purpose of this study was to investigate the demographics, injury mechanisms, and results of operative treatment of lesser tuberosity avulsion fractures in skeletally immature patients. METHODS: Eight patients were treated with open reduction and internal fixation (ORIF) for lesser tuberosity avulsion fractures from 2000 through 2010. Data were collected regarding patient demographics, mechanisms of injury, operative

[Hemiarthroplasty for management of proximal humeral fractures].

PURPOSE OF THE STUDY: With the advent of angle-stable implant systems in surgical treatment of proximal humeral fractures, the number of indications to shoulder hemiarthroplasty decreased dramatically, because these modern implants provide certain fixation of osteoporotic bone fragments in elderly patients. MATERIAL AND METHODS: The authors report on their experience with shoulder replacement surgery in 29 patients, of which 26 underwent urgent surgery. The indications for acute hemiarthroplasty included humeral head fractures in which destruction of the articular surface exceeded 40 %, and f

Areal and volumetric bone mineral density and risk of multiple types of fracture in older men.

Although many studies have examined the association between low bone mineral density (BMD) and fracture risk in older men, none have simultaneously studied the relationship between multiple BMD sites and risk of different types of fractures. Using data from the Osteoporotic Fractures in Men study, we evaluated the association between areal BMD (aBMD) by dual-energy X-ray absorptiometry (DXA) and volumetric BMD (vBMD) by quantitative computed tomography (QCT) measurements, and different types of fractures during an average of 9.7years of follow-up. Men answered questionnaires about fractures ev

Functional outcome after shoulder arthroplasty for primary osteoarthritis: a multicenter study.

Shoulder arthroplasty outcomes have been reported in many case series. Typically, these series have followed either a single prosthesis used to treat a variety of arthritic disorders of the shoulder or experience in a single institution. In contrast, this report of a prospective study summarizes the experience of several surgeons with a single prosthetic design for treatment of primary osteoarthritis of the shoulder. A prospective, multicenter clinical outcome study evaluated 176 shoulders in 160 patients with primary osteoarthritis. This study evaluated a single prosthetic design (Global Shou

Outcomes of shoulder arthroplasty in Olmsted County, Minnesota: a population-based study.

Most studies on shoulder arthroplasty include a diverse group of patients presenting to a tertiary care center. Little information is available regarding outcomes in a community setting. We reviewed 98 residents (110 shoulders) of Olmsted County, Minnesota who had shoulder arthroplasties from 1976 to 2000. There were 65 total shoulder arthroplasties and 45 humeral head replacements. The most common indications were osteoarthritis for total shoulder arthroplasties (48/65) and acute fracture for hemiarthroplasties (27/45). The Neer ratings were excellent or satisfactory in 92% of total shoulder

Combination of Anatomical Plates and Rotator Cuff Reinforcement for Minimally Invasive Treatment of Greater Tuberosity Fractures of the Humerus.

Greater tuberosity fractures of the humerus can be challenging to manage due to their complex anatomy and the potential for compromised shoulder function. We present a novel technique for treating greater tuberosity fractures utilizing specialized anatomical plates and rotator cuff reinforcement. The technique involves the use of an anatomically T-shaped plate designed specifically for the greater tuberosity region of the humerus, allowing for precise fixation and stability. Additionally, rotator cuff reinforcement is performed using sutures to enhance structural integrity and promote early mo

Functional Outcomes After Operative Management of Extra-Articular Glenoid Neck and Scapular Body Fractures.

BACKGROUND: This study's purpose was to assess patient-based functional outcomes following open reduction and internal fixation (ORIF) of displaced scapular body and glenoid neck fractures. This series represents a 9-year experience at a level-I trauma center and referral destination for this injury. METHODS: A database was established to record surgical and functional outcomes of scapular fractures treated with ORIF. For this report, the cases of all patients who had a glenoid neck or scapular body fracture (AO/OTA 14-A3 or 14-C1) without intra-articular involvement were reviewed. Operative

Humeral offset as a predictor of outcomes after reverse shoulder arthroplasty.

INTRODUCTION: The Grammont-style reverse shoulder arthroplasty (RSA) relies on medialization and distalization of the shoulder center of rotation. Lateralized designs have recently gained popularity. The amount of lateralization, however, remains a controversial topic. The purpose of this study was to correlate the change in humeral offset (HO) with outcomes and complications following RSA. We hypothesized that a lateralized HO following RSA would be associated with improved range of motion (ROM), better patient-reported outcomes (PROs), and fewer complications. MATERIALS AND METHODS: A conse

Computed tomography of joint injuries.

CT has become the imaging modality of choice in most acute and chronic injuries involving the articular surfaces. Today's technology allows high resolution, fast scanning, and rapid reformatting, all of which contribute to prompt orthopedic evaluation and decision making. Our orthopedic colleagues have come to rely on high-quality CT scans in order to evaluate calcaneal and tibial plateau fractures, stress fractures, osteochondritis dissecans, acute and chronic shoulder trauma, and problems of the painful wrist--especially the distal radioulnar joint.

Global Excision of Severe Heterotopic Ossification of the Shoulder: A Case Report.

CASE: A 21-year-old man sustained a closed glenohumeral fracture/dislocation as a pedestrian struck by a motor vehicle. He was treated nonoperatively and developed severe post-traumatic heterotopic ossification (HO) with near-complete shoulder ankylosis. We present our technique for safe surgical excision. CONCLUSIONS: Excision led to improvements in motion and quality of life at 1 year postoperatively. Recommendations for successful HO excision around the shoulder include excision after at least 180 days, appropriate preoperative imaging to include cross-sectional imaging and a 3D model, int

Base of coracoid process fracture with acromioclavicular dislocation in a child.

Fracture of the coracoid process is a rare injury. It can be easily missed when associated with other injuries to the shoulder girdle, for instance, acromioclavicular joint (ACJ) dislocation. Clinical attention is easily drawn to the more obvious ACJ dislocation, hence, the need for further radiological evaluation. We report an unusual case of fracture of the base of coracoid process associated with a true acromioclavicular joint dislocation in a 12 year old boy, with no separation of the epiphyseal plate, as one might expect. Treatment also remains controversial. Our patient underwent open re

A Scapular Spine Fracture Defined on the Basis of a Bisphosphonate: a Case Report and Review of the Literature.

Bisphosphonates are commonly used in the treatment of osteoporosis. Long-term use without drug holiday causes the risk of atypical fractures. Subtrochanteric and femoral stress fractures are among the frequently described complications. In our case report; a stress fracture of the scapular spine, a previously undescribed adverse effect of bisphosphonates, is presented. Key words: bisphosphonates, scapular spine, stress fracture, drug holiday.

Clavicle shaft fractures: are children little adults?

Most clavicle shaft fractures in children are nondisplaced, minimally shortened or angulated, and are well treated with nonoperative management. Indications for operative fixation include open fractures and impending open fractures. Relative indications for operative fixation include fractures in multi-trauma patients, floating shoulder injuries, comminuted fractures, and shortened fractures in older adolescents. Controversies exist in preferred methods of fixation and include superior or anteroinferior plating, locked or unlocked plating, and intramedullary fixation. Retrospective studies of

Office Diagnosis of Shoulder Disorders.

In brief Complex tests and procedures brief are not usually needed to identify shoulder injuries that are common in clinical practice: rotator cuff injuries, shoulder instability, and impingement syndrome, among others. The history can differentiate causes of pain or instability with questions about symptom patterns. Performing targeted physical tests such as the drop-arm test and the apprehension test establishes presence and degree of impingement or instability. A standard radiographic series that includes rotation, axillary, and supraspinatus outlet views can identify presence of associated

Early atraumatic recurrent dislocation after reverse total shoulder arthroplasty despite optimal implant positioning: two case reports.

INTRODUCTION: Postoperative instability following reverse total shoulder arthroplasty (RTSA) is one of the most common complications and remains a significant clinical challenge. Although implant positioning, design, and surgical technique are known to influence stability, there is no universally accepted management strategy when dislocation recurs. This study reports two rare cases of early, atraumatic recurrent dislocation after RTSA, highlighting the complexity of this complication. METHODS AND RESULTS: Between 2014 and 2023, 182 reverse total shoulder arthroplasties (RTSAs) were performed

Percutaneous Intramedullary Nailing of Complex Humeral Shaft Fractures: A Retrospective Case Series.

BACKGROUND: Humeral diaphyseal fractures have been traditionally stabilized with plates and screws. However, surgical morbidity can be quite extensive, particularly in more complex segmental and comminuted fracture patterns. An intramedullary nail (IMN) has the biomechanical advantage of being a load-sharing device and can be placed with a more minimally invasive technique. The purpose of this study was to evaluate the clinical and radiographic outcomes of complex humeral shaft fractures treated with an IMN utilizing a percutaneous surgical technique. METHODS: A retrospective review was perfo
