Nasopharyngeal carcinoma: posttreatment changes of imaging findings.

OBJECTIVE: The purpose of this study was to determine the radiologic results of nasopharyngeal carcinoma (NPC) that showed complete responses on follow-up imaging studies after radiation therapy. MATERIALS AND METHODS: This study is a retrospective review of 23 patients (18 male, 5 female, aged 15-71 years; mean age, 48.5 years) affected with NPC, from August 1995 to July 2000, who were examined with magnetic resonance imaging or computed tomography scan before and after either radical radiotherapy or chemoradiotherapy. The median follow-up was 24.7 months and ranged from 12 to 48 months. We

Skull base osteomyelitis interpreted as malignancy.

Lesions in the skull base may present difficulties in diagnosis primarily because the access needs to be invasive and one has to rely on imaging that may still be misleading. The case presented here illustrates this example in that the patient had abnormalities on computed tomography (CT) and magnetic resonance image (MRI) scans quite convincing of malignancy but which with time proved, essentially through clinical judgment, to be skull base osteomyelitis secondary to malignant external otitis (OME).

The Le Fort I osteotomy as a surgical approach for removal of tumours of the midface.

Tumours of the midface and maxillary sinuses have been removed via multiple approaches. The most common approaches are those using incisions in the facial skin, especially in the case of malignant tumours. The Le Fort I procedure via an intraoral incision as described by Sailer in 1986 is a versatile alternative. Combined with a coronal approach and various osteotomies of the upper face it also allows removal of tumours extending into the orbits, the nasoethmoidal complex and the skull base. The versatility of the Le Fort I osteotomy as a surgical approach was analysed in 17 cases. This method

Associations between perioperative lung ultrasound score and postoperative pulmonary complications in patients with skull base tumor resection: a retrospective single-center cohort study.

BACKGROUND: Postoperative pulmonary complications (PPCs) are common after major surgery and may be particularly frequent after skull base neurosurgery. This study aimed to investigate the incidence of PPCs following skull base tumor resection and the relationship between lung ultrasound scores (LUS) and PPCs in these patients. METHODS: This is a retrospective, single-center, cohort study. Demographic, clinical, and laboratory data were collected from the Hospital Information System. Perioperative lung ultrasound was routinely performed in patients undergoing skull base tumor resection. Only t

Clival Chordoma Presenting as Airway Obstruction in a Neonate With Tuberous Sclerosis and Good Response to sirolimus.

We report the case of a 50-day-old infant with a nasopharyngeal mass causing respiratory distress at birth. Imaging revealed a mass from the clivus and features indicative of tuberous sclerosis complex (TSC), including cortical tubers and subependymal nodules. The mass was surgically excised, with histopathology confirming chordoma. Genetic analysis identified pathogenic mutation in TSC2 gene. Postsurgery, he had residual disease and was treated with mammalian target of rapamycin (mTOR) inhibitor sirolimus, leading to complete resolution. This case is notable as it represents the first instanc

Phosphaturic mesenchymal tumor of the skull base presenting with tumor-induced osteomalacia and multiple fractures: a case report.

BACKGROUND: This case reports a rare disease in which the clinical symptoms are completely inconsistent with the primary site. As the occurrence of this disease is partly rare, the challenges in the diagnosis of phosphaturic mesenchymal tumors are discussed. It also provides a new clinical treatment option, that is, the combined treatment with anlotinib capsules after surgery and the long-term follow-up to observe the effect of the combined treatment. CASE PRESENTATION: A 27-year-old Chinese woman presented with generalized pain and multiple fractures for 2 years. The present case was initial

Surgical outcomes in the treatment of temporal bone cerebrospinal fluid leak: A systematic review.

OBJECTIVE: Temporal bone CSF leak represents a rare condition that may be associated to some particular symptoms as hearing loss, otorrhea and tinnitus. In the opinion of many authors a surgical treatment is mandatory to avoid serious consequences as meningitis. Middle cranial fossa (MCF), transmastoid (TM) and combined approaches are all described to manage this condition. The objective of this paper was firstly to analyze the overall outcomes of this surgery. Our second aim was to make a comparison between different surgical modalities on the basis of their rate of success and complications.

Repair of Anterior Skull Base Defect by Dual-Layer/Split-Frontal Pericranial Flap.

INTRODUCTION: Normal skull base structures are destroyed either by the skull base pathology itself or during surgery that results in cerebrospinal fluid (CSF) leak. Skull base repair is usually performed transnasally by using a nasal septal flap. But when NSF is not available and failed repeated transnasal repair, a cranionasal repair with frontal pericranial flap (PF) can be attempted to achieve the highest successful chance. We performed a dual layer/split PF repair of a skull base defect using cranionasal approach and here we describe the technique. CASE DESCRIPTION: A 74-year-old man suff

Probabilistic Tractography of the Cranial Nerves in Vestibular Schwannoma.

OBJECTIVE: Multiple recent studies have reported on diffusion tensor-based fiber tracking of cranial nerves in vestibular schwannoma, with conflicting results as to the accuracy of the method and the occurrence of cochlear nerve depiction. Probabilistic nontensor-based tractography might offer advantages in terms of better extraction of directional information from the underlying data in cranial nerves, which are of subvoxel size. METHODS: Twenty-one patients with large vestibular schwannomas were recruited. The probabilistic tracking was run preoperatively and the position of the potential d

Following the canyon to reach and remove olfactory groove meningiomas.

BACKGROUND: Olfactory groove meningiomas (OGMs) represent approximately 10% of all intracranial meningiomas. They arise in the olfactory fossa, a variable depression delimited by the lateral lamella and perpendicular plate. The cribriform plate with the lateral lamella and ethmoidal and orbital roof could be viewed as a 'canyon' with the frontal sinus as the main entrance. METHODS: Between January 2000 and December 2013, 32 consecutive patients underwent removal of OGMs through this 'canyon' at the Department of Neurosurgery of Brescia and Turin. Complete removal was achieved in all patients

Resection of large epidermoid tumors ventral to the brainstem: techniques to expand the operative corridor across the basilar artery.

Epidermoid tumors comprise about 1% of all intracranial tumors. They are congenital lesions that arise from paramedian cisterns within the posterior fossa. These tumors present as heterogeneous hyperintense lesions on FLAIR and homogenous hyperintense lesions on DWI. Surgical resection remains the most accepted form of therapy, but epidermoid tumors may recur. These tumors are well exposed through a traditional retrosigmoid approach. The tumor can be removed relatively easily as it is avascular. However, the propensity of this tumor type to fill the small spaces within basal cisterns and attac

Using fixed anatomical landmarks in endoscopic skull base surgery.

BACKGROUND: The identification of anatomic landmarks in endoscopic skull base or revision sinus surgery can be challenging. Normal anatomy is significantly altered with many paranasal tumors. Traditional endoscopic surgical landmarks extrapolated from inflammatory disease, such as the superior turbinate, may have been previously removed or involved in pathology. A frequently used rule to enter the sphenoid, "stay below or at the level of the orbital floor as dissection proceeds posteriorly and one will avoid the skull base," is assessed anatomically. METHODS: The maxillary sinus roof height,

Pseudogout mimicking an infratemporal fossa tumor.

BACKGROUND: The infratemporal fossa is a well-concealed, complex anatomical space. Neoplasms arising in this area are heterogenous in nature and have an insidious onset and usually present late. METHODS AND RESULTS: We present a case of a 71-year-old woman who presented with worsening facial pain, trismus, and a large mass in her infratemporal fossa with minimal associated temporomandibular joint destruction. She underwent a surgical excision of the mass, which revealed a pseudogout deposit. CONCLUSION: The clinical and radiological features of patients with tophaceous pseudogout frequently

Expanded endoscopic endonasal approach for treatment of clival chordomas: early results in 12 patients.

OBJECTIVE: We report our recent experience with an expanded purely endoscopic endonasal approach for the treatment of clival chordomas. METHODS: Twelve patients underwent an expanded endoscopic approach for excision of cranial base chordomas at Toronto Western Hospital. Two patients had undergone a previous craniotomy for excision of a significant lateral intracranial extension of the tumor. All other patients had mainly centrally located lesions. Three patients had recurrent tumors. This study focused on the surgical approach, results, and complications associated with this approach. RESULT
