Narcolepsy with cataplexy.

Narcolepsy with cataplexy is a disabling sleep disorder affecting 0.02% of adults worldwide. It is characterised by severe, irresistible daytime sleepiness and sudden loss of muscle tone (cataplexy), and can be associated with sleep-onset or sleep-offset paralysis and hallucinations, frequent movement and awakening during sleep, and weight gain. Sleep monitoring during night and day shows rapid sleep onset and abnormal, shortened rapid-eye-movement sleep latencies. The onset of narcolepsy with cataplexy is usually during teenage and young adulthood and persists throughout the lifetime. Pathoph

Narcolepsy: Comorbidities, complexities and future directions.

Patients with narcolepsy live with a lifelong sleep-wake disorder, impairing their quality of life, productivity, educational and employment outcomes. Clinicians are becoming aware that a significant aspect of the burden of this disease relates to frequent comorbid conditions, including aspects of the patient's emotional, metabolic, sleep and immune health. This review explores the literature describing the comorbidities seen in patients with narcolepsy, to enhance understanding of these often complex presentations. It hopes to encourage a multidisciplinary approach, to collaborate with patien

Modeling of sleep-induced changes in airway function: implication for nocturnal worsening of bronchial asthma.

Here we describe the model of sleep-induced worsening of airway function in patients with airway disorders. Our model is based on the noradrenergic pathways that link central neuronal structures responsible for alternating wakefulness and sleep with the neuronal networks regulating the activity of airway-related vagal preganglionic neurons (AVPNs). Our previous studies showed that cholinergic outflow to the airways depend on the activity of inhibitory inputs to AVPNs. Major inhibitory cell groups, regulating AVPNs discharge, include brainstem noradrenaline (NA)-containing cells receiving proje

Night-to-night variability in periodic leg movements in patients with restless legs syndrome.

BACKGROUND AND PURPOSE: Although a night-to-night variability in periodic leg movements (PLM) occurrence has been described in patients with primary PLM disorder and sleep apnea syndrome, no study has apparently considered the inter-night effect on PLM index during wakefulness and sleep in patients with Restless Legs Syndrome (RLS). Moreover, no study has examined the night-to-night variability in PLM index according to sleep stage and time of night. We therefore examined changes in PLM index during wakefulness and sleep during two consecutive nights in a group of untreated RLS patients. PATI

Sleep in frontotemporal dementia is equally or possibly more disrupted, and at an earlier stage, when compared to sleep in Alzheimer's disease.

BACKGROUND: Conversely to other neurodegenerative diseases (i.e., Alzheimer's disease, AD), sleep in frontotemporal dementia (FTD) has not been studied adequately. Although some evidence exists that sleep-wake disturbances occur in FTD, very little is known regarding sleep macrostructure and/or primary sleep disorders. OBJECTIVE: To investigate these issues in this population and compare them to similar issues in AD and in healthy elderly (HE). METHODS: Twelve drug-naïve behavioral-variant FTD (bvFTD) patients (7 men/5 women) of mean age 62.5 ± 8.6 years were compared to seventeen drug-naïve

Sleep and psychotic symptoms: An actigraphy and diary study with young adults with low and elevated psychosis proneness.

Experimental research has shown that poor sleep triggers psychotic experiences, even in healthy participants. This warrants an in-depth investigation of this mechanism in a naturalistic environment, an exploration of which particular aspects of poor sleep trigger psychotic symptoms, and a test for reverse effects of symptoms on sleep. For this purpose, we conducted a 14-day ambulatory assessment study with 82 young adults (age: M = 21.24 years, SD = 1.54; 64.6% female), half of which were characterized by elevated psychosis proneness. Objective sleep parameters (actigraphically-measured sleep

Sleep and behavioral problems in Down syndrome: differences between school age and adolescence.

BACKGROUND: Individuals with Down syndrome (DS) are at risk of developing sleep problems. In spite of the well-established knowledge on the presence of sleep difficulties in DS individuals and the associated emotional and behavioral problems, less is known about the possible differences in the kind of associations between sleep and emotional/behavioral problems across different age ranges. METHODS: In this retrospective study, we included 289 participants with DS aged 6-18 years with the aims to explore differences in the distribution of sleep problems between specific age groups (school age

Sleep disturbances in elderly: a subjective evaluation over 65.

Sleep in elderly people shows progressive changes caused by general aging processes. Several alterations are described in medical literature: changes of sleep/wake rhythm and modifications both in sleep duration and in sleep architecture. The aim of our study was to evaluate sleep disturbances in elderly people, with and without cognitive impairment, through a sleep questionnaire. Our population included 1000 subjects, over 65 years of age, stratified by sex and age. The first 600 interviews were included in this report. All patients underwent a mini mental state examination (MMSE) and a questi

In vivo marker of brainstem myelin is associated to quantitative sleep parameters in healthy young men.

The regional integrity of brain subcortical structures has been implicated in sleep-wake regulation, however, their associations with sleep parameters remain largely unexplored. Here, we assessed association between quantitative Magnetic Resonance Imaging (qMRI)-derived marker of the myelin content of the brainstem and the variability in the sleep electrophysiology in a large sample of 18-to-31 years healthy young men (N = 321; ~ 22 years). Separate Generalized Additive Model for Location, Scale and Shape (GAMLSS) revealed that sleep onset latency and slow wave energy were significantly associ

Agomelatine: An Astounding Sui-generis Antidepressant?

Major depressive disorder (MDD) is one of the foremost causes of disability and premature death worldwide. Although the available antidepressants are effective and well tolerated, they also have many limitations. Therapeutic advances in developing a new drug's ultimate relation between MDD and chronobiology, which targets the circadian rhythm, led to a renewed focus on psychiatric disorders. In order to provide a critical analysis about antidepressant properties of agomelatine, a detailed PubMed (Medline), Scopus (Embase), Web of Science (Web of Knowledge), Cochrane Library, Google Scholar, an

Heat Pump Noise: Scoping Overview of Emission Characteristics and Systematic Review of Human Effects.

OBJECTIVE: The aim of this work is two-fold: to characterize air-source heat pump (ASHP) noise emissions and to systematically evaluate the scientific evidence regarding non-auditory health effects, thereby assessing the technology's current and future public health impact. METHODS: The primary component is a PRISMA-guided systematic review on noise impact. Utilizing a population-exposure-outcome framework, eight bibliographic databases were searched for English and German records. Of 501 identified records, 26 reports (covering 25 unique studies) met eligibility criteria. Risk of bias (RoB)

Gut microbiota alterations in sleep disturbances: A systematic review and meta-analysis.

Emerging evidence links gut dysbiosis to sleep disturbances, yet a comprehensive synthesis defining specific and shared microbiota alterations across sleep disorders is lacking. In this systematic review and meta-analysis, we searched PubMed, Cochrane Library, Web of Science, Embase, and PsycINFO for articles published up to Sep 20, 2025. Data from 53 studies were included in the quantitative analysis. The alpha diversity was decreased in individuals with sleep disturbances compared to controls (Shannon index standardized mean difference [SMD] = -0.24, 95% CI: -0.34 to -0.15; Chao1 index SMD =

Sleep Alterations and Cognitive Decline.

Sleep disturbances and cognitive decline are intricately connected, and both are prevalent in aging populations and individuals with neurodegenerative disorders such as Alzheimer's disease (AD) and other dementias. Sleep is vital for cognitive functions including memory consolidation, executive function, and attention. Disruption in these processes is associated with cognitive decline, although causal evidence is mixed. This review delves into the bidirectional relationship between alterations in sleep and cognitive impairment, exploring key mechanisms such as amyloid-β accumulation, tau patho

Sleep Spindle Abnormalities as Neurophysiological Biomarkers of Schizophrenia Spectrum Disorders: From Cellular Mechanisms and Neural Circuits to Clinical Implications.

Although not included in current diagnostic criteria, sleep disturbances are common in patients with schizophrenia (SCZ) and significantly impact their cognitive function and clinical outcomes. Among sleep disturbances, abnormalities in sleep spindles-non-rapid eye movement thalamocortical oscillations essential for sleep stability and memory consolidation-have emerged as potential neurophysiological biomarkers of SCZ. Sleep spindle deficits, particularly reduced density and duration, are consistently observed across illness stages and correlate with cognitive impairments, including working me

Sleep alterations in amyotrophic lateral sclerosis.

PURPOSE OF REVIEW: This review summarizes recent evidence on sleep disturbances in amyotrophic lateral sclerosis (ALS), emphasizing their role as intrinsic features of the disease process rather than consequence of motor decline. RECENT FINDINGS: Emerging data suggest that sleep disturbances such as sleep fragmentation, rapid eye movement sleep (REM) and non rapid eye movement sleep (NREM) alterations and circadian changes often precede classic motor symptoms. Structural and functional hypothalamic changes have been observed in early ALS, suggesting a direct role in sleep-wake dysregulation.

Using Cannabis and CBD to Sleep: An Updated Review.

PURPOSE OF REVIEW: Cannabis sativa and constituent cannabinoids are in widespread use for the treatment of sleep disorders where many patients desire pharmacotherapy. Previous reviews highlight a lack of high-quality evidence to support the efficacy and long-term safety of cannabinoids in various conditions. We aim to provide an update of recent original research evaluating cannabinoid-based therapeutics in sleep disorders. RECENT FINDINGS: We identified twenty-one recent studies of cannabinoids for insomnia, subjective sleep impairment, obstructive sleep apnoea, rapid eye movement sleep beha
