Assessment of Treatment Response by Bone SPECT-CT in a Case of Dermatomyositis with Calcinosis Cutis.

Calcinosis of soft tissue is a rare but known complication of dermatomyositis (DM), mostly associated with juvenile DM and rare in adult DM. Bone scan with Tc-99m Methylene diphosphonate is useful in disease mapping and has high sensitivity to know the extent of calcinosis. However, there is scanty literature available on the utility of bone scan in treatment response evaluation in DM. Rituximab has been found useful in adult DM with calcinosis unresponsive to conventional treatment. We describe an interesting case showing partial response to rituximab on bone scan with single-photon emission

Ameloblastoma on the Maxillary Sinus: Cause of Unilateral Nasal Obstruction.

Ameloblastoma is a rare, benign (99%) or malignant (1%) tumour, which has derived from dental mesenchyme. We present a case of a patient mainly complaining about obstruction on the right nasal cavity. Endoscopy and computed tomography revealed a soft tissue mass occupying the maxillary sinus to the middle meatus causing complete obstruction of the right nasal cavity. Endoscopic en block removal of the lesion and biopsy confirmed follicular ameloblastoma. Literature review confirms the extremely rare frequency of ameloblastoma. The aim of the present study is to expand on our knowledge of a rar

An atypical manifestation of idiopathic retroperitoneal fibrosis: A case report.

A 58-year-old male patient, previously healthy, presented with right flank pain of few hours duration. Initial workup showed an elevated creatinine and right hydroureteronephrosis. Computed tomography imaging revealed a retroperitoneal soft tissue mass encasing the right common iliac artery which is a first reported case in its anatomical distribution. Patient was referred to interventional radiology service where a percutaneous nephrostomy tube was placed followed by double J stenting. Subsequently, treatment with steroids was started. Follow up imaging a few months later revealed progressive

Clinical Examination and Radiographic Assessment of the Cavus Foot.

The purpose of the clinical examination is to detect subtle cavus or cavovarus deformity, assess the severity and type of deformity, differentiate between idiopathic versus secondary etiologies of cavus foot deformity, and evaluate for other associated abnormalities. The clinical examination should begin with a gait analysis. The neurologic examination reveals peripheral neuropathy or central nervous system etiology for the foot deformity. On plain radiographs, forefoot-driven deformity can be assessed using the Meary angle, and hindfoot-driven deformity can be measured by the calcaneal pitch.

A Thymic Hyperplasia Case without Suppressing on Chemical Shift Magnetic Resonance Imaging.

A 22-year-old woman with myasthenia gravis (MG) presented with ptosis and mild muscle weakness symptoms for one year. Computed tomography (CT) presented a diffuse bilobulate enlargement gland with a high density of soft tissue. Magnetic resonance imaging (MRI) showed the gland with no suppression on the opposed-phase chemical shift. After the thymic tumor diagnosis, she underwent thoracoscopic surgery for tumor resection. The postoperative histopathological finding was thymic lymphoid hyperplasia. This case suggests chemical shift MRI is not enough in distinguishing, and supplementary examinat

Unusual findings on infantile cortical hyperostosis: A case report.

BACKGROUND: Caffey's disease is a rare syndrome, usually self-limiting, affecting newborn and young infants. On radiological exams, the cortical hyperostosis is always present, associated or not to soft tissue swelling. Other radiographic presentations are described as lytic areas. AIM: This article has the objective to relate computed tomography (CT) findings of Caffey's disease, where lytic lesion on mandibular angle was the principal radiological manifestation. METHODS AND RESULTS: Three-dimensional reconstructions were performed to demonstrate the initial aspect and the healing process.

Jejunal Carcinoid Tumors Presenting as Small Bowel Obstruction.

Carcinoid tumors are the most common form of gut neuroendocrine tumors, however, they rarely present with small bowel obstruction. We present a case of a 65-year-old woman without prior abdominal operations who presented with symptoms of small bowel obstruction. Computed tomography (CT) showed multiple air fluid levels and a transition point in the left mesentery with two soft tissue densities at the same level. The patient was taken to the operating room for surgical exploration, which showed two intramural masses in the mid and distal jejunum, which surgical pathology showed to be stage IIIB

Radiologic Approach to Musculoskeletal Infections.

Imaging is often used to establish a diagnosis of musculoskeletal infections and evaluate the full extent and severity of disease. Imaging should always start with radiographs, which provide an important anatomic overview. MRI is the test of choice in most musculoskeletal infections because of its superior soft tissue contrast resolution and high sensitivity for pathologic edema. However, MRI is not always possible. Alternative imaging modalities including ultrasound scan, computed tomography, and radionuclide imaging may be used. This article reviews the individual imaging modalities and disc

Ectopic thymic carcinoma presenting as an intrathoracic mass.

An asymptomatic 83-year-old man was found to have a right intrathoracic tumor. Computed tomography demonstrated a soft-tissue density mass measuring 55 × 25 × 22 mm adjacent to the right anterior chest wall. At surgery, the tumor was found to adhere to the diaphragm and right lung, contiguous with the mediastinal fat tissue. Histology of the resected specimen demonstrated proliferation of spindle and sarcomatous cells with multinucleated giant cells. Thus the tumor was diagnosed as undifferentiated thymic carcinoma and was considered to have arisen from ectopic thymic tissue. At 2 years postop

The Role of Multidetector CT in the Diagnosis of Retroperitoneal Fibrosis: Report of a Case.

Herein, we report a 40-year old man who presented with flank and abdominal pain with dilatation of the bilateral pyelocalyceal system detected in ultrasonography. Computed Tomography (CT) scan showed a soft tissue mass at the level of the fourth and fifth lumbar vertebrae in the retroperitoneal region. There were no blood flow signals in 64-slice multidetector CT (MDCT) which confirms the Retroperitoneal Fibrosis (RPF). Pathological examination showed infiltration of plasma cells, macrophages, lymphocytes and eosinophils accompanied by fibrosis, which is consistent with idiopathic RPF. In conc

Imaging of male pelvic trauma.

Prompt imaging plays an important role in the evaluation of male pelvic soft tissue trauma. Using appropriate imaging modalities, with optimization of contrast administration when appropriate, is essential for accurate diagnosis. Traumatic bladder rupture, either extraperitoneal or intraperitoneal, is diagnosed with high accuracy using computed tomography cystography. Suspicion of urethral injury warrants evaluation with retrograde urethrography to evaluate for the presence of injury and injury location. Early identification of laceration of the testicular tunica albuginea is essential. Unders

Intrathoracic xanthoma mimicking lung cancer in a patient with familial hypercholesterolemia type II: a case report.

Xanthomas are benign soft-tissue lesions commonly occurring on the skin, subcutis, or tendon sheaths of patients. The lung and thoracic cavity is a rare location for xanthomas. We present a 39-year-old woman who was admitted to our hospital with complaints of dyspnea, cough, and chest pain. She had a prior diagnosis of type II familial hypercholesterolemia. Chest x-ray film and computed tomography scans revealed a large tumor-like mass in the right hemithorax. Thoracal mass and narrowed tracheal segments were removed using cardiopulmonary bypass. Histopathologic findings were consistent with x

An orthognathic simulation system integrating teeth, jaw and face data using 3D cephalometry.

A method for simulating the movement of teeth, jaw and face caused by orthognathic surgery is proposed, characterized by the use of 3D cephalometric data for 3D simulation. Computed tomography data are not required. The teeth and facial data are obtained by a laser scanner and the data for the patient's mandible are reconstructed and integrated according to 3D cephalometry using a projection-matching technique. The mandibular form is simulated by transforming a generic model to match the patient's cephalometric data. This system permits analysis of bone movement at each individual part, while

Case of optic nerve involvement in metastasis of a gastrointestinal stromal tumor.

BACKGROUND: The gastrointestinal stromal tumor (GIST) belongs to the leiomyosarcoma or leiomyoma group, and metastasis of leiomyosarcoma to the orbit is extremely rare. The most common sign of the metastasis of leiomyosarcoma to the orbit is proptosis; the primary lesion is usually found in skin or soft tissue. CASE: A 60-year-old man, who had a history of multiple metastases of GIST, suffered sudden unilateral blindness. OBSERVATIONS: His history and computed tomographic scans suggested that the lesion was a metastasis of GIST to the cavernous sinus around the root of the optic nerve. CONC

The role of CT, MRI and 3D imaging in the diagnosis of temporomandibular joint and other orofacial disorders in children.

Advances in medical imaging techniques add a new third dimension (3D computer reconstruction of CT, MR and Ultrasound images) to the diagnostic armamentarium of practitioners in orofacial medicine, surgery and Orthodontics. In particular, Computerized Tomography and Magnetic Resonance Imaging with, to a lesser extent, Nuclear Medicine Imaging and Sonography provide more accurate and detailed information on abnormalities and disorders of craniofacial osseous and soft tissues, especially the temporomandibular joint, than was previously available from plain films or tomography.
