Myositis in Germany: epidemiological insights over 15 years from 2005 to 2019.

BACKGROUND: The medical care of patients with myositis is a great challenge in clinical practice. This is due to the rarity of these disease … dition, myositis patients suffer more frequently from psychiatric disorders, such as depression and somatoform disorders. However, the ICD-10 catalogue only includes the specific coding of "dermatomyositis" and "polymyositis" and thus does not allow for a sufficient analysis of all idiopathic inflammatory myopathies subtypes. CONCLUSION: The current data provide a co …

Empirical assessment of functional somatic disorder (FSD): frequency, applicability, and diagnostic refinement in a population-based sample.

BACKGROUND: Persistent and troublesome physical symptoms are common and can, regardless of their cause, greatly impair patients' quality of life. Reflecting complex brain-body interactions, they are observed across all healthcare specialties, commonly overlap across them, and receive inconsistent diagnoses. In response, the international research network EURONET-SOMA has proposed a diagnostic classification for persistent and troublesome symptoms entitled "functional somatic disorder (FSD)". Focusing on symptom patterns across organ systems, the FSD approach aims to enhance diagnosis, treatmen

Can MMPI-2-RF confirm the structural validity of the somatoform spectrum of the Hierarchical Taxonomy of Psychopathology (HiTOP) in patients with chronic low back pain.

OBJECTIVES: This study aimed to evaluate the structural validity of the somatoform spectrum within the context of the Hierarchical Taxonomy of Psychopathology (HiTOP) model in patients with chronic low back pain. METHODS: 233 patients who were diagnosed with chronic low back pain (77.3 % female) completed the Oswestry Disability Index (ODI), the Health Anxiety Inventory-18 (HAI-18), the Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF), and the Patient Health Questionnaire (PHQ-15). Confirmatory factor analyses were conducted at two levels: spectra and symptom levels

Psychological risk factors of somatic symptom disorder: A systematic review and meta-analysis of cross-sectional and longitudinal studies.

OBJECTIVE: Current diagnostic concepts of somatic symptom disorder (SSD) in DSM-5 and bodily distress disorder (BDD) in ICD-11 require certain psychological criteria, but researchers have called for further specification. Therefore, in a first step, this systematic review and meta-analysis aimed to summarize the current evidence on psychological factors associated with SSD/BDD and/or disorder-relevant clinical outcomes such as symptom severity and impairment. METHODS: Psychological factors were systematically searched using Pubmed, Cochrane Library and Psycinfo via EBSCO. Studies providing or

Do labels matter? Implications of ongoing symptomatic chronic illnesses labeled as conventional diagnoses vs. functional somatic syndromes.

OBJECTIVE: A consideration of chronic illness according to illness labels that are medically understood as opposed to being outside of medical understanding may reveal unique differences in how individuals understand their illness and how such lay understandings relate to health-related quality of life. Study aims are framed according to the commonsense model of self-regulation with a focus on characterizing illness representations according to chronic illness diagnosis type. METHODS: Individuals suffering from symptomatic chronic illnesses (n  =  192) completed measures of illness representa

When things get complicated: At-risk attachment in children and adolescents with chronic pain.

Pain is a signal of danger, and danger activates the attachment system. When a parent responds to a child's pain with appropriate protection and comfort, more often than not, the pain resolves. But what happens in families when a child's pain becomes chronic and continues to signal a danger that has long passed? This study explored patterns of attachment in 48 children and adolescents with chronic functional pain and 48 healthy controls using structured attachment interviews. Patterns of attachment were identified using the Dynamic Maturational Model of Attachment. Compared to controls, childr

A review of recent case reports of cenesthopathy in Japan.

Idiopathic abnormal bodily sensations, or cenesthesic symptoms, are exhibited in a wide variety of mental illnesses. In Japan, patients with abnormal bodily sensations are often diagnosed with cenesthopathy. This study reviewed recent case reports of cenesthopathy. Of the 100 identified cases, young patients were more commonly men with predominant bodily cenesthesic symptoms, while older patients (≥40 years) were more commonly women with cenesthesic symptoms restricted to the oral cavity (oral cenesthopathy).

Non-specific symptoms as clues to changes in emotional well-being.

BACKGROUND: Somatic symptoms are a common reason for visits to the family physician. The aim of this study was to examine the relation between non-specific symptoms and changes in emotional well-being and the degree to which the physician considers the possibility of mental distress when faced with such patients. METHODS: Patients who complained of two or more symptoms including headache, dizziness, fatigue or weakness, palpitations and sleep disorders over one year were identified from the medical records of a random sample of 45 primary care physicians. A control group matched for gender an

Childhood abuse and neglect in body dysmorphic disorder.

OBJECTIVE: No published studies have examined childhood abuse and neglect in body dysmorphic disorder (BDD). This study examined the prevalence and clinical correlates of abuse and neglect in individuals with this disorder. METHODS: Seventy-five subjects (69.3% female, mean age=35.4+/-12.0) with DSM-IV BDD completed the Childhood Trauma Questionnaire and were interviewed with other reliable and valid measures. RESULTS: Of these subjects, 78.7% reported a history of childhood maltreatment: emotional neglect (68.0%), emotional abuse (56.0%), physical abuse (34.7%), physical neglect (33.3%), an

The role of core beliefs in the specific cognitions of bulimic patients.

Cognitive models of bulimia nervosa have stressed the importance of self-beliefs in the maintenance of the disorder. However, new findings show a deeper and more general level of beliefs also to play a role in eating disorders. These beliefs are long-standing, absolute, and unconditional. In the present study, the relations between such core beliefs and specific cognitions regarding eating, shape, and weight were examined. The sample consisted of 75 bulimic outpatients who started treatment. The patients completed the Schema Questionnaire, the Eating Disorders Inventory-2, the SCL-90-R, and th

Lessons from an Ukrainian woman with clubbing and back pain.

We present this case that we consider of particular interest in palliative care because of the difficulty we encountered in palliating the patient's symptoms. The difficulties derived, at least in part, from particular personality traits and complex social circumstances. We think this case is an excellent example to demonstrate the complexity of the process of pain and the different resources and techniques that can be used to manage it. We also wish to underline the importance of clinical signs in diagnosis. In fact, this patient showed signs compatible with Pierre-Marie disease (or pulmonary

Gastrin, cholecystokinin, and somatostatin in a laboratory experiment of patients with functional dyspepsia.

OBJECTIVE: To explore the pattern of gastrointestinal hormonal variations in plasma and to relate this to possible pathophysiological mechanisms in functional dyspepsia. METHOD: There were 25 patients, 12 men and 13 women, aged 24 to 50 years, with recurrent functional dyspepsia, compared with community control subjects pair-wise, matched for age and sex. The subjects participated in a laboratory stress experiment with timed provocations. At fixed intervals, 22 samples of blood were drawn from each subject and frozen for later peptide analyses. Levels of gastrin, cholecystokinin (CCK), and so

Affect and marital adjustment in women's rating of dyspareunic pain.

OBJECTIVE: To investigate the extent to which depressive symptomatology, anxiety, and marital adjustment mediate pain ratings in women suffering from dyspareunia. METHOD: Seventy-six women with dyspareunia were administered the depression and anxiety scales of Derogatis's Brief Symptom Inventory and the McGill Pain Questionnaire. They also underwent a manual-visual gynecological examination, an ultrasound, and a colposcopy in an attempt to identify potential evidence and type of organicity. RESULTS: Depressive symptomatology, anxiety, and marital adjustment together accounted for a significa

Somatic complaints in childhood tic disorders.

Twenty-six children diagnosed with chronic tic disorders (18 with Gilles de la Tourette syndrome and 8 with chronic motor tic disorder) were studied for unexplained physical complaints. Compared to normal controls, an excess of somatic complaints was found in the tic disorders group; this was similar to an excess of somatic complaints in a mixed psychiatric clinic group. Medication produced no significant effect on somatic complaints for patients in the tic and psychiatric clinic groups. Within the tic disorders group, no significant correlation was found between the increased somatic complain

Medically unexplained physical symptoms, somatization disorder and abridged somatization: studies with the Diagnostic Interview Schedule.

This paper reviews recent research that used the Diagnostic Interview Schedule (DIS) to characterize medically unexplained symptoms and their clustering in clinical and community populations. While the type of symptom(s) differed little across samples, the distribution of a less restrictive construct of somatization ('abridged' somatization) showed interesting differences across the various groups. The authors propose that in view of its relatively high prevalence, abundant psychiatric comorbidity and the simple and unintrusive nature of the assessment, use of the abridged construct may have p

Somatization disorder in family practice.

Although often difficult to recognize, somatization disorder--the presence of multiple physical symptoms but no apparent physical or psychiatric disease--is a common problem in primary care practices. Onset is usually before age 30. The disorder manifests as a myriad of symptoms, including gastrointestinal complaints, various types of pain, cardiopulmonary symptoms, pseudoneurologic complaints, sexual problems and complaints related to the female reproductive system. Vigilance is necessary, because somatization disorder may mimic numerous physical and psychiatric ailments.

Psychiatric illness and length of stay in general hospitals: do case finding methods matter?

OBJECTIVE: Several prior studies have investigated whether patients with "non-cognitive" mental disorders (i.e., organic disorders, substance abuse, delirium, and psychotic disorders excluded) have longer Length Of Stay (LOS) than mentally healthy individuals in nonpsychiatric hospital settings. These studies yielded contrasting results. The present paper aims to examine whether methods of psychiatric case finding can explain these differences. METHODS: Using the Clinical Interview Schedule (CIS) and the General Health Questionnaire (GHQ), 462 in-patients of medical, surgical, gynecological,
