BACKGROUND: Inclisiran (ALN-PCSsc) is a long-acting RNA interference (RNAi) therapeutic agent that inhibits the synthesis of proprotein convertase subtilisin-kexin type 9 (PCSK9), a target for the lowering of low-density lipoprotein (LDL) cholesterol.
METHODS: In this phase 1 trial, we randomly assigned healthy volunteers with an LDL cholesterol level of at least 100 mg per deciliter in a 3:1 ratio to receive a subcutaneous injection of inclisiran or placebo in either a single-ascending-dose phase (at a dose of 25, 100, 300, 500, or 800 mg) or a multiple-dose phase (125 mg weekly for four dos

BACKGROUND: Testosterone deficiency is associated with increased serum cholesterol levels. However, how testosterone deficiency precisely affects cholesterol metabolism remains unclear. Therefore, in the current study, we examined the effect of testosterone deficiency on cholesterol metabolism and liver gene expression in pigs fed a high-fat and high-cholesterol (HFC) diet.
METHODS: Sexually mature male miniature pigs (6-7 months old) were randomly divided into 3 groups as follows: intact male pigs fed an HFC diet (IM+HFC), castrated male pigs fed an HFC diet (CM+HFC), and castrated pigs with

BACKGROUND: South Asian individuals bear a disproportionately high burden of atherosclerotic cardiovascular disease (ASCVD), characterized by earlier onset, greater severity, and higher mortality compared with Western populations. This is despite lower average low-density lipoprotein cholesterol (LDL-C) levels and is driven by a distinctive cardiometabolic phenotype, including adverse lifestyle factors, high prevalence of insulin resistance and diabetes, hypertriglyceridemia, increased small dense LDL particles, elevated lipoprotein(a), and low high-density lipoprotein cholesterol (HDL-C) leve

BACKGROUND: While proprotein convertase subtilisin/kexin type 9 (PCSK9) has emerged as an effective therapeutic target in secondary prevention of cardiovascular disease (CVD), its role in primary prevention remains unclear. Therefore, we examined the prospective association between PCSK9 and CVD risks in the general population.
METHODS: In the Gusu cohort, 1876 community members (mean age 57 years, 38% males) who were free of CVD in 2014 were included. Serum PCSK9 was measured by enzyme-linked immunosorbent assay kits at baseline and CVD events were followed every 2 years through 2020. Associ

BACKGROUND: Proprotein convertase subtilisin/kexin type 9 (PCSK9) is a critical therapeutic target for managing hyperlipidemia and atherosclerosis. We developed Cadd4, a synthetic proteolysis-targeting chimera (PROTAC) engineered to selectively induce proteasomal degradation of the PCSK9 protein. In this study, we investigate Cadd4's anti-atherosclerotic properties and concurrently evaluate the feasibility and safety of its long-term therapeutic administration.
METHODS: Cadd4-mediated PCSK9 degradation was assessed in mouse monocyte-macrophage leukemia cells(RAW264.7), mouse aortic vascular s

BACKGROUND: Acute ischemic stroke (AIS) carries high risks of recurrence, disability, and death despite standard therapy. The role of Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors has been shown to provide cardiovascular benefits, but their role in the acute neurovascular phase of AIS remains uncertain. METHODS: We systematically searched PubMed, MEDLINE, Embase, Web of Science, and the Cochrane Library from inception to August 31, 2025, for studies initiating PCSK9 inhibitors during hospitalization or within 14 days of AIS symptom onset. Primary outcomes were early recurren

Background: Intensive low-density lipoprotein cholesterol (LDL-C) reduction is essential for secondary prevention after acute coronary syndrome (ACS). Although randomized trials support the use of proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors, real-world evidence on long-term lipid control and the optimal timing of initiation remains limited. Objective: To evaluate lipid outcomes and atherosclerotic cardiovascular disease (ASCVD) events associated with evolocumab after ACS and to assess whether timing of initiation influences these outcomes in routine clinical practice. BACKGROUND: Proprotein convertase subtilisin/kexin type 9 (PCSK9) is a newly identified immunosuppressive regulator, but its mechanism of suppressing antitumor immunity remains ambiguous. This study aims to uncover the underlying mechanism by which PCSK9 promotes hepatocellular carcinoma (HCC) immune escape and to explore potential intervention strategies.
METHODS: Co-culture assay assessed the cytotoxicity of CD8+ T cells against PCSK9-knockout HCC cells. Hepa1-6, H22, and HepG2 cells were used to establish HCC mouse models. Tumor microenvironment changes were evaluated using flow cytometry

Chronic stress significantly impacts both physical and mental wellbeing, increasing risk of cardiovascular disease, immune dysregulation, and psychiatric conditions such as depression and anxiety disorders. The plasma proteome is a valuable source of biomarkers of health and disease, but the limited number of studies exploring the potential of the plasma proteome as a biomarker for stress-related disorders underscores the importance of further investigation of the effects of chronic stress on the plasma proteome. The aim of this study was to examine the effect of a 5-week chronic psychosocial

OBJECTIVE: Guidelines developed by the American College of Cardiology/American Heart Association (ACC/AHA) recommend lipid-lowering therapies (LLTs) to reduce low-density lipoprotein cholesterol (LDL-C) and atherosclerotic cardiovascular disease (ASCVD) risk. This study described LLT utilization patterns and LDL-C goal achievement (to <70 mg/dL) among patients with ASCVD in the United States.
METHODS: This retrospective study was conducted using Optum's de-identified Clinformatics Data Mart Database (CDM). Patients with their first ASCVD diagnosis (index date) in the CDM database between July

BACKGROUND: Increasing evidence highlights the importance of novel players in Alzheimer's disease (AD) pathophysiology, including alterations of lipid metabolism and neuroinflammation. Indeed, a potential involvement of Proprotein convertase subtilisin/kexin type 9 (PCSK9) in AD has been recently postulated. Here, we first investigated the effects of PCSK9 on neuroinflammation in vitro. Then, we examined the impact of a genetic ablation of PCSK9 on cognitive performance in a severe mouse model of AD. Finally, in the same animals we evaluated the effect of PCSK9 loss on Aβ pathology, neuroinfla

PURPOSE: Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors are novel drugs that have proven efficacy in improving cardiovascular outcomes. Roles for the PCSK9 molecule in metabolic pathways beyond LDL receptor processing and cholesterol homeostasis are well established. PCSK9 genetic variants associated with lower LDL-C levels correlate with a higher incidence of type 2 diabetes (T2DM), calling into question the appropriateness of these drugs in patients with T2DM and those at high risk of developing diabetes, and whether cardiovascular benefit seen with PCSK9 inhibitors might b

BACKGROUND AND PURPOSE: Statins were shown to increase hemorrhagic stroke (HS) in patients with a first cerebrovascular event in 2006 (SPARCL), likely due to off-target antithrombotic effects, but continued to sometimes be used in patients with elevated HS risk due to absence of alternative medications. Recently, the PCSK9Is (proprotein convertase subtilisin kexin 9 inhibitors) have become available as a potent lipid-lowering class with potentially less hemorrhagic propensity.
METHODS: We performed a systematic comparative meta-analysis assessing HS rates across all completed statin and PCSK9

BACKGROUND: Beyond their success in cardiovascular disease prevention, statins are increasingly recognized to have sex-specific pleiotropic effects. To gain additional insight, we characterized associations of genetically mimicked statins across the phenotype sex-specifically. We also assessed whether any apparently non-lipid effects identified extended to genetically mimicking other widely used lipid modifiers (proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors and ezetimibe) or were a consequence of low-density lipoprotein cholesterol (LDL-c).
METHODS: We performed a sex-speci

BACKGROUND: Familial hypercholesterolemia is characterized by an elevated level of low-density lipoprotein (LDL) cholesterol and an increased risk of premature atherosclerotic cardiovascular disease. Monoclonal antibodies directed against proprotein convertase subtilisin-kexin type 9 (PCSK9) have been shown to reduce LDL cholesterol levels by more than 50% but require administration every 2 to 4 weeks. In a phase 2 trial, a twice-yearly injection of inclisiran, a small interfering RNA, was shown to inhibit hepatic synthesis of PCSK9 in adults with heterozygous familial hypercholesterolemia.

BACKGROUND: Patients with acute coronary syndrome are at risk for peripheral artery disease (PAD) events and venous thromboembolism (VTE). PCSK9 (proprotein convertase subtilisin/kexin type 9) inhibitors reduce lipoprotein(a) and low-density lipoprotein cholesterol (LDL-C) levels. Our objective was to ascertain whether PCSK9 inhibition reduces the risk of PAD events or VTE after acute coronary syndrome, and if such effects are related to levels of lipoprotein(a) or LDL-C.
METHODS: This was a prespecified analysis of the ODYSSEY OUTCOMES randomized clinical trial (Evaluation of Cardiovascular
