Sixty-three congenital idiopathic clubfeet were clinically and radiographically evaluated following a single Turco-type complete posteromedial soft tissue release with internal fixation. Average age at the time of surgery: 12.4 months; average postoperative follow-up: 67.2 months. A new rating system that weighs dynamic functional results more heavily was used to compare our findings. The results were rated as excellent in 29 feet (46%), good in 15 feet (24%), fair in 8 feet (13%), and poor/failures in 11 feet (17%). Ninety-five percent of the parents were satisfied with the current results, a

The treatment of Johnson stage 2 posterior tibial tendon insufficiency remains controversial. Because the deformities remain flexible in stage 2, bony osteotomies are preferable over fusion operations. It is our contention that operative intervention should address all of the components of the pes planovalgus deformity that exists in stage 2 disease. Seventeen patients with 20 cases of stage 2 posterior tibial tendon insufficiency were treated with heel cord lengthening, flexor digitorum longus to medial cuneiform tendon transfer, lateral column lengthening, and medial displacement calcaneal o…

PURPOSE: Volar locking plate (VLP) of distal radius fractures (DRF) carries a number of surgical risks including flexor pollicis longus (FPL) rupture secondary to irritation from prominent hardware. This study reviews FPL reconstruction (with interposition graft or tendon transfer) after VLP-related rupture.
METHODS: This retrospective case series reports the experience at one tertiary center with FPL reconstruction after DRF. Patients treated between January 1, 2013, and June 12, 2023, were eligible for inclusion. Preoperative radiographs were analyzed for Soong grading and fracture reductio…

BACKGROUND: Functional outcomes and quality-of-life measures following salvage reconstruction of composite Achilles/posterior leg defects are limited. We present our experience with combined Achilles defect reconstruction utilizing free tissue transfer with vascularized neotendon constructs.
METHODS: Between 2011 and 2012, six patients underwent vascularized reconstruction of complex Achilles defects by a single surgeon. Demographic and functional data were collected for each patient. Subjective evaluation and quality-of-life measures were obtained preoperatively and postoperatively using Ame

Two cases of spontaneous extensor pollicis longus tendon rupture are presented. One occurred in a 17-year-old diver and the other in a 69-year-old physician tennis player. Neither patient experienced pain, and only the diver had premonitory wrist discomfort. The diver was treated by an extensor indicis proprius transfer. The physician denied any disability and elected to have no treatment.

A cavovarus deformity results from muscle imbalances in the foot. There are several etiologies of a cavovarus foot including congenital, neu … modification, physical therapy, and bracing. Operative management consists of soft tissue releases, tendon transfers, osteotomies, arthrodesis, and repair/reconstruction of lateral ankle ligaments and peroneal tendons.

Deficit if the functional integrity of the limbs as a consequence of the definitive lesions of the peripheral nerves necessitates surgical t … truction of nerves increases the probability for choice of adequate surgical techniques. Muscle and tendon transfers are the methods of choice in most of the patients. Free microneurovascular muscle flaps are applied in the most complicated cases.

BACKGROUND: The results of operative treatment of an unreduced elbow dislocation have been regarded with pessimism. Suggested procedures have included tendon-lengthening, tendon transfer, or reconstruction of ligament or bone.
METHODS: Three women and two men (average age, forty-nine years) with an unreduced dislocation of the elbow without associated fractures were treated with open relocation of the joint and hinged external fixation at an average of eleven weeks (range, six to thirty weeks) after the initial injury. The lateral soft tissues, including the origin of the lateral collateral l…

An integrated concept is presented for the treatment of lesions of the brachial plexus. In such patients, nerve repair is useless as a single method of therapy. Only the combination of nerve repair with both conventional and new methods of muscle and tendon transfer, including the free muscle transplant innervated by vascularized nerve grafts, can give the patient functional use of the paralyzed arm.

Lateral elbow tendinopathy, or "tennis elbow," is a pathology that affects around 1.3% of the general population. Capacitive-resistive electric transfer therapy aims to provoke temperature and current flow changes in superficial and deep tissues. The aim of this in vitro study was to analyze the thermal behavior and transmission of electric current on the superficial and deep tissues of the elbow during the application of different modalities of a capacitive-resistive electric transfer treatment protocol for chronic elbow tendinopathy. A cross-sectional study was designed; five fresh cryoprese

Lower extremity contracture associated with stroke commonly results in a nonreducible, spastic equinovarus deformity of the foot and ankle. Rigid contracture deformity leads to gait instability, pain, bracing difficulties, and ulcerations. The classic surgical approach for stroke-related contracture of the foot and ankle has been combinations of tendon lengthening, tendon transfer, osteotomy, and joint fusion procedures. Recovery after traditional foot and ankle reconstructive surgery requires a period of non-weightbearing that is not typically practical for these patients. Little focus has be…

Dynamic anterior stabilization using the long head of the biceps tendon (LHBT) has emerged as a soft-tissue alternative for treating anterior shoulder instability in the setting of subcritical glenoid bone loss. This Technical Note describes an inlay dynamic anterior stabilization technique using soft-tissue anchors designed to enhance tendon-to-bone healing and optimize the length-tension relation of the transferred LHBT. The procedure combines a modified Bankart repair with trans-subscapularis LHBT transfer and fixation within a 5-mm anterior-inferior glenoid bone tunnel using inlay soft anc

The Latarjet procedure was first described in 1954. It is a nonanatomic procedure that requires transfer of the coracoid process, together with the conjoint tendon, to the anterior glenoid margin. The aim is to prevent recurrent anterior shoulder dislocation through a "triple blocking" effect, which includes restoring bone loss, providing a dynamic sling effect, and performing capsulolabral repair. Despite the long history of the Latarjet procedure, studies evaluating its impact on scapulothoracic kinematics are sparse. However, there is a concern that scapulothoracic dyskinesia may occur owin

The factors associated with failed operative intervention in the treatment of equinovarus foot deformity in children with cerebral palsy (CP) were evaluated after long-term follow-up. One hundred eight children with CP who had surgery on the posterior tibialis tendon (split tendon transfer, intramuscular lengthening, or Z-lengthening) on 140 feet were reviewed at a mean age of 16.8 years with 7.3 years of follow-up. The surgery was considered a failure when a 10 degrees or greater varus or valgus hindfoot deformity was present or if an additional operative intervention was required or planned.…
