Dynamic sonographic evaluation of peroneal tendon subluxation.

OBJECTIVE: Peroneal tendon subluxation is a major cause of posttraumatic lateral ankle pain. Because peroneal subluxation often occurs only … lso scanned. Sequential axial and longitudinal sonograms of the peroneus longus and peroneus brevis tendons at rest were obtained and evaluated for tendinitis and tendon tears. Real-time axial sonograms were then obtained while the foot was dorsiflexed and everted. Of the 13 patients, 12 had sonographic findings of peroneal tendon subluxation and underwen …

Percutaneous repair of acute Achilles tendon rupture.

Various studies have shown that the operative treatment of a freshly ruptured Achilles tendon is generally considered to be more appropriate than a nonoperative regimen. However, complications in open reconstructions are reported to occur in 11-29%. The method used in this study reduced the risk of complications arising from operation, but simultaneously allowed early postoperative mobilization and functional treatment. It was a percutaneous repair of the Achilles tendon, using two Lengemann extension wires for coadaptation of the ruptured tendon. To fix the rupture site, the authors used a fi

Interventions for treating acute Achilles tendon ruptures.

BACKGROUND: There is lack of consensus on the best management of the acute Achilles tendon (TA) rupture. Treatment can be broadly classified into operative (open or percutaneous) and non-operative (cast immobilisation or functional bracing). Post-operative splintage can be with a rigid cast (above or below the knee) or a more mobile functional brace. OBJECTIVES: To identify and summarise the evidence from randomised controlled trials of the effectiveness of different interventions in the treatment of acute Achilles tendon ruptures. SEARCH STRATEGY: We searched multiple databases including th

MR imaging of tendon lesions of the hand and wrist.

MR imaging of the hand and wrist tendons has greatly benefited from the use of dedicated surface coils, which allow fine depiction of the intricate anatomy of these structures, owing to high spatial resolution images as well as superb soft tissue contrast. MR imaging of the wrist and hand is obtained in the axial, sagittal, and coronal planes. The axial and sagittal planes provide most of the information necessary, however, to assess the tendons at the wrist and hand. The axial images are optimal for evaluating tendon morphology, longitudinal splits, tendon sheath fluid, and adjacent soft tiss…

The accessory extensor tendon of the first metatarsophalangeal joint.

The accessory extensor tendon of the first metatarsophalangeal joint (1st MTPJ) was evaluated in 32 cadaver feet. Eighty-one percent of feet possessed an accessory tendon to the 1st MTPJ. Of those feet possessing an accessory tendon to the 1st MTPJ, approximately 92% originated from the extensor hallucis longus muscle-tendon unit, while approximately 8% originated from the tibialis anterior muscle-tendon unit. All accessory tendons inserted onto the dorsal/dorsomedial capsule of the 1st MTPJ. Accessory tendons were found to be bilateral in the majority (87.5%) of specimens. Differences in sex …

[Secondary reconstruction of extensor pollicis longus tendon].

BACKGROUND: Secondary reconstruction of the extensor pollicis longus tendon (EPL) can be performed using an intercalated tendon graft (ITG) or a tendon transposition, preferably the extensor indicis transposition (EIT), since a secondary suture of the EPL tendon is not possible in most cases due to degenerative changes or defect injuries. A direct comparison of ITG and EIT in one single patient collective based on a standardized follow-up protocol has not been performed yet. PATIENTS AND METHOD: For the first time, these two operative procedures were directly compared in a retrospective analy

[Clinic applications of vascularized plantaris tendon grafting].

OBJECTIVE: To study a new method of the vascularized plantaris tendon transferring. METHODS: The vascularized plantaris tendon was elevated with the pedicle of the fascial perforating vessel or a small segment of the posterior tibial artery, which was transplanted to the wrist to repair the tendon defects of the flexor pollicis longus or the flexor digitorum muscle resulted from electric injury. According to the defects, the plantaris tendon can be used together with the skin, fascia, or both as a composite flap. 7 cases received the operation with complete survival of the flap. RESULTS: Pos

[The Mantero technique for flexor tendon repair - an alternative?].

PURPOSE: Regaining free tendon gliding after reconstruction of flexor tendons is essential to restore full function to the affected finger. Mantero et al. described a pull-out suture technique for the repair of flexor digitorum profundus (FDP) lesions in zone 1, allowing early postoperative active mobilisation and thus minimizing the risk of tendon adhesions. In a retrospective study we examined the results after Mantero tendon repair and compared these with the results after different reconstructive procedures in the literature. METHODS AND MATERIAL: Between 1995 and 2001, the FDP tendon in …

Vascular malformation within the flexor tendon sheath.

We report an unusual location of a capillary vascular malformation found entirely within the flexor tendon sheath of an 8-year-old girl. Vascular malformations have been found in many locations throughout the hand but have, to our knowledge, not been reported to involve the vinculum of the flexor tendon sheath. The vascular malformation was contained completely within the sheath. Therapeutic management involved surgical excision of the vascular malformation.

Arthroscopic management of calcific tendinitis of the popliteus tendon.

Calcific tendinitis is seen most commonly affecting the rotator cuff but has not been previously reported affecting the popliteus tendon. Symptoms mimic a tear of the posterior horn of the lateral meniscus. Arthroscopic debridement of the calcific deposit results in resolution of symptoms.

Avulsion of the triceps tendon during acceleration stress.

The authors present a rare case of triceps tendon avulsion associated with +5 G-force and muscle straining. Although failure of musculoskeletal system, mainly the cervical spine, secondary to high-G exposure has been reported, this is the first report they have found of a triceps tendon avulsion. Furthermore, unlike other traumatic triceps avulsions reported, the pilot involved had no known predisposing factors. Awareness of the possibility of such an injury must be maintained when a pilot of high performance aircraft presents with elbow pain.

Patellar tendon length--the factor in patellar instability?

Patellar tendon length has been compared in 42 knees with a history of patellar dislocation, and 51 control knees. A lateral X-ray and a magnetic resonance image (MRI) were taken of each knee. The mean radiological patellar tendon length was 46 mm in the controls and 53 mm in the dislocation group. From MRI images, the mean was 44 mm in controls and 52 mm in the dislocation group. This means that the patellar tendon is significantly (P<0.0001) longer in patients with a history of patellar dislocation on both MRI and X-ray. There is no significant difference (P=0.52) between X-ray and MRI measu

Treatment of tendon disorders. Is there a role for corticosteroid injection?

Tendon injuries and other tendon disorders are a source of major concern in competitive and recreational athletes and in many working conditions requiring repetitive movements. The exact etiology, pathophysiology, and healing mechanisms of the various tendon complaints are, however, only partly known and even origin of pain in the chronic tendon disorders is unknown. Thus, the treatment strategies recommended for tendon complaints vary considerably and the given treatment is frequently based on empirical evidence only. Corticosteroid injections are one of the most commonly used treatments for

Complications of achilles and posterior tibial tendon surgeries.

Surgery of the Achilles and posterior tibial tendons requires finesse to achieve optimal function. Although superficial in anatomic location, various critical neurovascular structures exist in close proximity. The superficial blood supply is fragile, requiring a delicate touch to prevent problems with wound healing. Damaged tendon tissue often is friable, making it difficult to sew or anchor. Adjusting the tendon length to the appropriate tension requires good judgment and affects long-term outcome through power and range of motion. Finally, surgery on these two tendons significantly affects f…

Fibroma of a tendon-sheath presenting as toe deformity.

We describe a rare manifestation fibroma in a tendon-sheath in an 83-year-old man. The patient complained initially of a slowly progressive spreading, apart of the right second and third toes. A mass was found in the plantar aspect of the foot. It was completely excised at surgery. Histological examination revealed sparse spindle or stellate cells with slit-like vessels in the dense collagenous matrix. There had been no recurrence at follow-up 17 months after surgery.

Quinolone-associated tendonitis: a potential problem in COPD?

BACKGROUND: Quinolones have traditionally had limited application in the area of community-acquired respiratory tract infections due to poor cover against Streptococcus pneumoniae. This trend is changing with the broader spectrum of newer fluoroquinolones. A rare serious side effect of fluoroquinolones is tendinopathy. AIMS: This study describes two cases of levofloxacin-associated tendinopathy in patients with severe chronic obstructive pulmonary disease (COPD) and the implications and mechanisms involved are discussed. CONCLUSIONS: The finding of two cases of levofloxacin-induced tendinopa

Resection of the common digital extensor tendon in a gelding.

A four-year-old gelding was lame owing to a chronic septic common digital extensor tendon and sheath. The horse had been treated by open surgical lavage but the sepsis had recurred after three months. Physical, ultrasonographic, cytological and histological examinations confirmed chronic septic tenosynovitis and tendonitis. The entire intrathecal component of the common digital extensor tendon was resected under general anaesthesia and the synovial lining of the sheath was ablated. Postoperatively the horse regained good limb function and became sound.
