Prophylactic minitracheotomy in lung resections. A randomized controlled study.

Thirty consecutive patients undergoing lung resections were randomized into two groups: Group A (n = 15) received minitracheotomy postoperatively and group B (n = 15) were control patients. Postoperative respiratory course was monitored by serial clinical assessments, chest x-ray examination, arterial blood gases, sputa bacterial cultures, and the patient's requirement and response to chest physiotherpy. The two groups were similarly matched in age (mean 58.5 years), smoking habits, pulmonary functions, and surgical procedures. Postoperative pulmonary complications of collapse/consolidation de…

[Angioedema in ENT practice].

Angioedema is characterized by subcutaneous or submucosal swelling, which often affects the lips, tongue and oral or pharyngeal sites. Laryngeal oedema can be potentially life-threatening. The pathogenesis of angioedema is thought to involve the release of histamine and other mediators with subsequent extravasation of fluid. Over 90% of cases of chronic angioedema are clinically idiopathic. The known causes are allergic and more commonly pseudo-allergic (IgE-independent) reactions, especially drug or food hypersensitivity. Pseudo-allergic angioedemas are mainly evoked by a direct release of me

[Results of an educational program on the autonomy of patients tracheotomized for chronic respiratory insufficiency].

In 1984 an educational programme was drawn up for patients with chronic respiratory failure and tracheotomies (IRCT) aimed at their obtaining independence and self-sufficiency. A prospective evaluation was made in parallel with the diary cards of the nurses (SI) measuring the level of dependence of care, defined by 15 characteristics and based on objective and analysable data. After excluding those patients who are already independent (22), decreased or transferred on account of decompensation (23) or removal of the tracheostomy tube (27), 92 patients (46 obstructive and 46 restrictive) coming

Therapeutic effects of tracheostomy in two cases of hypersomnia with respiratory disturbance during sleep.

Recently the association of hypersomnia and respiratory insufficiency without lesion in the respiratory organ has attracted attention of many investigators. Obese patients with such a condition have been called the Pickwickian syndrome. In this report, two non-obese patients with a similar condition were presented, one with micrognathia and frequent apneic episodes during sleep, and the other with laryngeal stenosis due to paralysis of the bilateral laryngeal nerves and chronic laryngitis. Tracheostomy had a prompt and long-lasting therapeutic effect to make their sleep stable and also to reli

Reinnervation of the paralyzed larynx: a review.

Our high-speed and increasingly aggressive society, coupled with a continuing rise in necessary surgery of the neck and adjacent areas, acco … ients, as demonstrated by their ability to sustain reasonable day-to-day activity without needing a tracheotomy tube and without manifesting further loss of voice. The unilateral technique has yielded promising results in a limited number of carefully selected patients.

Acute epiglottitis in a community hospital.

Acute epiglottitis is a fulminating pediatric emergency which should be managed with antibiotics and the establishment of an airway. Either tracheostomy or nasotracheal intubation is employed for airway intervention. Because of the morbidity associated with tracheostomy, recent reports have stressed the use of nasotracheal intubation. Experience in 21 cases of acute epiglottitis emphasizes nasotracheal intubation as the treatment of choice in a community hospital.

[Anaesthesia for the relief of dyspnoea caused by fibrin membranes in the larynx and trachea (author's transl)].

Three cases of life-endangering airway obstruction by fibrin membranes are reviewed which developed in a 17-year-old girl with virus pneumonia, in a 21-year-old girl with a history of thrombopathy after general anaesthesia with naso-tracheal intubation and in a 23-year-old woman after short anaesthesia with orotracheal intubation. Possible causal factors, the clinical symptoms and the therapeutic measures taken by the anaesthetist are discussed. Since these cases are generally in a state of severe respiratory collapse by the time treatment is initiated general anaesthesia with muscle relaxatio

Morphological changes of tracheobranchial mucosa in dogs caused by breathing through a surgically produced tracheal fistula.

The author studied the morphological changes developing in the tracheobronchial mucosa following a surgical tracheal fistula. The experiments were carried out on 19 dogs which were divided into 2 groups. Group I included 10 dogs, in which specimens were obtained by bronchoscopy; in group II, 9 dogs, specimens were obtained directly. Histological examinations were carried out by Gomori's reaction and hematoxylin-eosin staining method. Histo-chemical tests included PAS reaction and alcian blue stain. The results showed that after 24 hours morphological changes mainly involved the tracheal mucosa

Acquired tracheomalacia: etiology and differential diagnosis.

The noninvasive diagnosis of impaired mechanical integrity or compliance of the trachea is most accurately made by fluoroscopic observation recorded on video tape or cineradiography, with or without benefit of artificial contrast media (contrast tracheography). In particular, localized buckling, collapse or dilatation indicative of focal tracheomalacia may thus be diagnosed in vivo and correlated with diseases of the central airways. Congenital tracheomalacia is a rare, bu, well described entity. Cases of acquired tracheomalacia occur with increasing frequency, but are often not clearly recogn
