Distortion of aortic valve from mechanical traction imposed by the mitral valve prosthesis: The three-dimensional transesophageal echocardiographic perception.

Iatrogenic injury to the aortic valve is a rare but frequently reported complication during mitral valve surgeries. Intraoperative 2-dimensional transesophageal echocardiography (2D TEE) has a major impact in diagnosing these injuries, so that timely intervention is possible. However, 2D TEE has lot of limitations during the perioperative period, which can be overcome by the three dimensional echocardiography (3D-TEE). We report a case where 3D TEE has undoubtedly delineated the cause for distortion of aortic sinus after mitral valve replacement and helped in the successful outcome.

Efficacy and safety of traction-assisted endoscopic submucosal dissection for superficial gastric neoplasms: a meta-analysis of randomized controlled trials.

BACKGROUND: Endoscopic submucosal dissection (ESD) remains technically demanding for superficial gastric neoplasms, particularly during submucosal layer dissection which is often time-consuming and associated with procedural risks. While traction-assisted ESD has been proposed to enhance visualization and facilitate safer dissection, current evidence regarding its efficacy and safety remains inconsistent. METHODS: We conducted a comprehensive meta-analysis of randomized controlled trials (RCTs) comparing traction-assisted versus conventional ESD for superficial gastric neoplasms. Primary outc…

Regional mechanical matching dictates vertebral rotation improvement in severe rigid idiopathic scoliosis with halo-pelvic traction: a finite element analysis.

PURPOSE: Halo-pelvic traction (HPT) safely improves spinal deformities by applying continuous, controlled axial traction between the skull and pelvis, creating favorable conditions for subsequent surgery. The regional mechanical relationship between spinal stress and improvement of vertebral axial rotation in severe rigid idiopathic scoliosis with HPT was clarified in this study. METHODS: A finite element (FE) model of C3-S1 spine was constructed to simulate three displacement conditions of HPT (5 cm, 10 cm, 15 cm). The regional mechanical relationship between spinal stress and the improvemen…

Tumor-phrenic nerve spatial relationship and intraoperative traction-induced injury during mediastinal tumor resection: implications for real-time protective strategies.

BACKGROUND: Phrenic nerve injury during mediastinal tumor resection can lead to significant postoperative diaphragmatic dysfunction. Current intraoperative protection techniques are imprecise and lack real-time feedback. We aimed to develop and validate a quantifiable, multimodal neuroprotective strategy. METHODS: A total of 370 patients undergoing minimally invasive mediastinal tumor resection were enrolled in a two-phase cohort. Tumor-nerve spatial parameters, traction force, and intraoperative temperature were measured. The intervention group received automated cold saline irrigation and p…
