Body sway test which examines the vestibulo-spinal reflex is sensitive for evaluating vestibular dysfunction in patients with vertigo, dizziness or unsteadiness. With the results of VS test (%) we were able to classify patients into three groups as follows: Group I, hyper range of visual suppression, VS (%) = 100-90; Group II, normal range of VS (%) = 89-45; Group III, reduced or abolished range of VS (%) = less than 44. In order to easily detect endolymphatic hydrops and to avoid the side effects, we applied a furosemide VOR test in 24 patients with unilateral Meniere's disease, 6 patients with delayed endolymphatic hydrops, and 2 patients with labyrinthine syphilis. Three cases of cerebellar hemorrhage and 5 of cerebellar infarction, diagnosed by brain CT, were examined from the early phase of the onset. All of our cases were mild to moderate in severity and were cured satisfactorily within 3 to 8 weeks of medical treatment. A case report on a 26 years old woman, who was admitted for a pontobulbar vascular accident. She took oral anticoncipients since 10 years without any control. Vertebral angiography confirmed the basilar artery thrombosis. Otoneurologic investigation was three times performed. The cochleovestibular alterations were in accordance with the neurologic signs and demonstrated the level and extension of the pathologic process. The autopsy confirmed the clinical diagnosis.

In a group of 26 patients with a hyperventilation syndrome, 77% showed vestibular hyperreactivity of velocity step responses, mostly due to an increase in gain of the vestibulo-ocular reflex (VOR), apparent from an increase in initial velocity, but also due to an increase in the time constant. Similar effects were found among 11 normal subjects after forced hyperventilation. In another group of 44 patients whose primary complaint was dizziness and who showed vestibular hyperreactivity without having any other demonstrable abnormality, a hyperventilation syndrome was found in 75%. In 1982 and 83, we performed cochleosacculotomies on 12 patients, of which nine have been retained for the present study. The operation was carried out on patients affected by unilateral Ménière's disease with a hearing loss stabilized at 50 dB or more at speech frequencies. All patients were suffering incapacitating vertigo which was perfectly controlled in all the cases operated so far. This article reports on a training programme for vestibular diseases, involving exercises of the vestibulo-vestibular, the visuo-vestibular, the cervico-vestibular and the spino-vestibular reflexes. Twenty-five patients were examined; all patients but two showed good improvement of the vestibular complaints as demonstrated by vestibular diseases, electronystagmography and posturography.
